The social and economic impact of epilepsy in Zambia: a cross-sectional study.

BACKGROUND: Among the 40 million people with epilepsy worldwide, 80% reside in low-income regions where human and technological resources for care are extremely limited. Qualitative and experiential reports indicate that people with epilepsy in Africa are also disadvantaged socially and economically, but few quantitative systematic data are available. We sought to assess the social and economic effect of living with epilepsy in sub-Saharan Africa. METHODS: We did a cross-sectional study of people with epilepsy concurrently matched for age, sex, and site of care to individuals with a non-stigm

Neurologic intensive care unit monitoring.

Monitoring modalities unique to the neurologic intensive care unit include intracranial pressure monitors and neuroelectrophysiologic monitors. Each modality fullfills criteria for accuracy, responsivity during clinical change, and stability over time for trend analysis. Intracranial pressure monitoring may be accomplished by any of three approaches--ventricular catheter, subarachnoid bolt, or epidural pressure transducer. Intracranial pressure control has proved beneficial in at least three different illnesses--acute closed head injury, acute noncommunicating hydrocephalus, and Reye's syndrom

Social support moderates association between posttraumatic growth and trauma-related psychopathologies among victims of the Sewol Ferry Disaster.

Disasters have numerous harmful effects on the mental health status of trauma-exposed people. We investigated the differences in the association between trauma-related psychopathologies and posttraumatic growth according to the perceived social support level among victims of the Sewol Ferry disaster on April 16, 2014, in South Korea. Data from 241 bereaved family members, survivors, and family members of survivors were used. The Duke-UNC Functional Social Support Questionnaire, Posttraumatic Growth Inventory, PTSD Checklist-5, Posttraumatic Embitterment Disorder Self-Rating Scale, Patient Heal

Successful Resuscitation: Novel Partnership between Paramedics and U.S. Coast Guard.

In this article, we present a notable case that illustrates a novel partnership between a United States Coast Guard small boat station and a civilian paramedic response unit. Patients who experience medical emergencies in remote environments are at particularly high risk for morbidity and mortality. For the most serious conditions, delayed contact with Advanced Life Support (ALS) has grave results. Typically, these circumstances involve small groups of individuals and cannot be easily predicted. The waters off the coasts of Maui, Hawaii, however, host millions of residents and visitors annuall

Migrant deaths along the California-Mexico border: an anthropological perspective.

California shares a 150-mile international border with Mexico. Traditionally, this border has seen non-stop illegal migration. In the 1990s, the Border Patrol began a concerted effort to establish and maintain control of the border, beginning in urban San Diego. This heightened law enforcement presence, known as Operation Gatekeeper, changed the westernmost segment of the border from the most permeable to the least permeable. This enforcement pushed migrants into more dangerous crossing areas in eastern San Diego and Imperial Counties, making their trip longer and more physically challenging a

The Capacity Profile: a method to classify additional care needs in children with neurodevelopmental disabilities.

The aim of this study was to determine the interrater reliability and stability over time of the Capacity Profile (CAP). The CAP is a standardized method for classifying additional care needs indicated by current impairments in five domains of body functions: physical health, neuromusculoskeletal and movement-related, sensory, mental, and voice and speech, in children from 3 to 18 years of age. The intensity of care in each domain is defined from 0 (no need for additional care) to 5 (needs help with every activity). The intensity of additional care in each of the five separate domains indicate

Risk factors for severe and fatal childhood unintentional injury: a systematic review protocol.

BACKGROUND: Unintentional injuries are a leading cause of death among children aged 1-19 years worldwide. Systematic reviews assessing various risk factors for different childhood injuries have been published previously. However, most of the related literature does not distinguish minor from severe or fatal injuries. This study aims to describe and summarize the current knowledge on the determinants of severe and fatal childhood unintentional injuries and to discuss the differences between risk factors for all injuries (including minor injuries) and severe and fatal injuries. The study also ai

Scedosporium apiospermum infection of the lumbar vertebrae: A case report.

BACKGROUND: Scedosporium apiospermum (S. apiospermum) is a clinically rare and aggressive fungus mainly found in contaminated water, wetlands, decaying plants, stagnant water, and potted plants in hospitals. The lung, bone, joint, eye, brain, skin, and other sites are easily infected, and there is a marked risk of misdiagnosis. There have been few case reports of infection by S. apiospermum of the lumbar vertebrae; most reports have focused on infection of the lung. CASE SUMMARY: An otherwise healthy 60-year-old man presented with a 4-mo history of lumbosacral pain, stooping, and limited walk

Homeless and unclaimed persons' deaths in north India (Jan 2008-Nov 2012): a retrospective study.

OBJECTIVES: Homeless and unclaimed dead bodies are a social as well as legal stigma on a country's development index. In addition, homeless people are exposed to increased incidence of diseases, accidents and mortality. Lucknow city, a city situated in the heart of the great Gangetic plain in the northern region of India, is surrounded by rural towns and villages and has seen tremendous growth in the past few years; with this the problem of migrants and homelessness has also increased. This has resulted in a spectrum of problems relating to law and order, including frequent incidences of uncla

Presence of alcohol in suicide victims.

A number of studies have established a strong connection between acute inebriation, alcohol addiction and suicides, as the last act of alcoholism or an act of desperation in an alcoholic's family, an act of escape from restraints in state of depression or as a way of self-destruction. In recent years in average 600 people per year committed suicide. Slovenia is a country with extremely high and variable suicide tendencies and harmful alcohol use levels, as well as a high level of alcohol-related troubles. The aim of our research was to ascertain some typical features, especially those connecte

Childhood behavior disorders and injuries among children and youth: a population-based study.

CONTEXT: While an association between pediatric behavioral disorders and injuries is generally acknowledged, no studies have measured the risk for injury in the context of a large, population-based study that is free of cohort biases. OBJECTIVES: To examine the association between childhood behavior disorders ([CBDs] as indicated by prescription for methylphenidate [MPH]) and a variety of injury outcomes, and to evaluate the risk for injury among these children after controlling for known demographic correlates. DESIGN: Population-based database analysis of all children in British Columbia (

Effect of cimetidine on the pharmacokinetics of the metabolites of metamizol.

Metamizol (dipyrone) is hydrolyzed in the gastrointestinal tract to the pharmacologically active metabolite 4-methyl-amino-antipyrine (4-MAA), which is transformed by both, oxidation to 4-formyl-amino-antipyrine (4-FAA) and demethylation to 4-amino-antipyrine (4-AA). 4-AA is acetylated to 4-acetyl-amino-antipyrine (4-AcAA). The aim of the present study was to investigate whether cimetidine will alter the pharmacokinetics of the metabolites of metamizol due to cimetidine-induced inhibition of the metabolic transformation of 4-MAA. The study was carried out in 12 patients with duodenal ulcer tre

Retrospective investigation of medico-legal autopsy cases involving mentally handicapped individuals.

Mentally handicapped individuals were involved in some way in 64 (17.7%) of the total of 362 medico-legal autopsy cases during the past 13 years. Among these 64 cases, 32 were suicides committed by mentally handicapped persons (18 males and 14 females). The most common method of suicide was drowning (65.6%). Five cases were homicides suddenly and unexpectedly committed by mentally handicapped persons. In four of the five cases, the victim was a member of the assailant's family; this was considered to be a characteristic of homicides committed by mentally handicapped persons. On the other hand,

Quantitative EEG predicts outcomes in children after cardiac arrest.

OBJECTIVE: To determine whether quantitative EEG (QEEG) features predict neurologic outcomes in children after cardiac arrest. METHODS: We performed a single-center prospective observational study of 87 consecutive children resuscitated and admitted to the pediatric intensive care unit after cardiac arrest. Full-array conventional EEG data were obtained as part of clinical management. We computed 8 QEEG features from 5-minute epochs every hour after return of circulation. We developed predictive models utilizing random forest classifiers trained on patient age and 8 QEEG features to predict o

Social inequities in injury-attributable emergency department visits across Alberta: an ecological study.

BACKGROUND: Injury is a leading cause of death and disability in Canada, and injury rates are disproportionately higher in Alberta. Limited evidence suggests that social inequities, including income inequality, may contribute to injury risk through differential exposure to hazards and unequal access to prevention and care. The objective of this study is to examine the ecological association between income inequality and rates of injury-attributable emergency department (ED) visits in Alberta from 2013 to 2022. METHODS: Data on social inequities were from the 2016 Census and injury-attributabl

The effect of bi-annual community-directed treatment with ivermectin on the incidence of epilepsy in onchocerciasis endemic villages in South Sudan: a study protocol.

BACKGROUND: Nodding syndrome (NS) is a devastating epileptic illness of unknown aetiology mainly affecting children 5-15 years of age. Head nodding distinguishes NS from other forms of epilepsy. Other manifestations of the illness include mental and physical growth retardation. Many children die as a result of falling in fires or drowning. Recently, it was shown that NS is only one of the phenotypic presentations of onchocerciasis associated epilepsy (OAE). Despite the strong epidemiological association between epilepsy and onchocerciasis, the causal mechanism is unknown. After implementation
