[Laser Doppler vibrometry: a new tool for diagnosing hearing loss with an intact eardrum].

The diagnosis of hearing loss with an intact eardrum frequently requires an entire battery of hearing tests, without the guarantee of an exact diagnosis. The techniques frequently provide only orientation for it, without establishing the site of the lesion and the etiology of the hearing loss. Laser Doppler vibrometry is a new technique, which has recently proved capable, of partially resolving this problem. The method is based on the study of the sound-induced vibration of the eardrum in humans in vivo, using a laser Doppler vibrometer. The method proved to be useful in the diagnosis of the p

[Reinforcing tympanoplasty with cartilage mosaic (differences from the palisade technique)].

OBJECTIVES: To demonstrate the interest of cartilage mosaic tympanoplasty in the reconstruction of the tympanic membrane. MATERIALS AND METHODS: Retrospective series of 103 patients; the authors analyzed their results after cartilage mosaic tympanoplasty, with a mean follow up of 3.5 years (3 to 7 years). They explain the surgical technique in detail. The average gain was calculated on the difference of the pre-and post-operative thresholds in air conduction on the four frequencies 500 Hz, 1000 Hz, 2000 Hz, 4000 Hz. RESULTS: The authors detail and analyze their anatomical and audiologic resu

Tympanosclerosis: long-term hearing results after ossicular reconstruction.

OBJECTIVE: The study goal was to analyze long-term hearing results and factors likely to affect outcome after ossicular reconstruction in patients with tympanosclerosis, particularly tympanosclerotic stapes fixation. METHODS: We conducted a retrospective chart review of the past 10 years. The short- and long-term (up to 9(1/2) years) hearing results of first-stage and second-stage operations for middle ear tympanosclerosis in 203 consecutive patients, performed in a tertiary referral otologic private practice, are presented. There were 160 tympanoplasties and 43 tympanoplasties with mastoidec

[Studies of sound condition in the reconstructed middle ear with a hydrophone. Initial results].

The acoustic result following reconstruction of the middle ear is often disappointing and unpredictable. Apart from biologic factors, this is due to insufficient knowledge of the mechanism of sound transmission through the operated middle ear. Therefore, temporal bone experiments with simulated middle ear operations were set up to investigate the sound transmission following different prosthesis' designs, materials, etc. We developed a tiny hydrophone which has a frequency range from 100 Hz to 10 kHz in order to measure the sound pressure in the cochlear fluid. By comparing the sound pressure

[Deafness in patients with osteogenesis imperfecta].

This study deals with 8 cases of progressive deafness in patients with abnormalities in growth and skeletal disease, suffering from Osteogenesis imperfecta (5 of them are a familial case). The work is both and audiological and radiological research of the temporal bone, done in order to uncover the reasons of the patient's deafness. This curses just like otosclerosis with a conductive component (fixation of the stapes) and a sensitive one (cochlear demineralization), starting up in adolescence as bilateral and symmetric process. Because of its progressive nature the AA, recommend as treatment

Fully implantable hearing aid in the incudostapedial joint gap.

A fully implantable hearing aid is introduced which is a combined sensor-actuator-transducer designed for insertion into the incudostapedial joint gap (ISJ). The active elements each consist of a thin titanium membrane with an applied piezoelectric single crystal. The effectiveness of the operating principle is verified in a temporal bone study. We also take a closer look at the influence of an implantation-induced increase in middle ear stiffness on the transducer's output. An assembly of the transducer with 1 mm thickness is built and inserted into six temporal bones. At this thickness, the

Preoperative Predictors in Chronic Suppurative Otitis Media for Ossicular Chain Discontinuity: A Cross-Sectional Study.

OBJECTIVE: Ossicular discontinuity may result from chronic suppurative otitis media and is usually detected intraoperatively. Our objective is to determine whether a preoperative audiogram can preoperatively predict the presence or absence of ossicular discontinuity. METHODS: A cross-sectional study was prospectively run on our patients, aged 12-75 years, ultimately operated on for chronic suppurative otitis media. Preoperative audiograms were analyzed to measure frequency-specific air-bone gap (ABG) cutoff values. Intraoperatively, ossicular chain integrity was carefully checked. Logistic re

Relationship of preoperative findings and ossicular discontinuity in chronic otitis media.

BACKGROUND: Ossicular discontinuity may result from chronic otitis media, with or without cholesteatoma, and usually cannot be determined unless an operation is performed. If preoperative information can be used to determine whether or not the ossicular chain is intact, the patient can be better informed and the surgeon can be better prepared before surgery. OBJECTIVE: To examine the correlation between preoperative findings and the status of the ossicular chain at surgery. STUDY DESIGN: Retrospective. SETTING: A tertiary referral center. PATIENTS: Data from 190 patients (212 ears) who wer

Oto-palato-digital syndrome type II.

We report a patient with a sporadic case of oto-palato-digital (OPD) syndrome type II. Parents and five previous sibs are normal. At 26 years of age the patient had conductive hearing impairment, cleft palate, a prominent forehead, a flat facies, and a broad nasal base resulting in the characteristic "pugilistic" appearance. Extension and supination were limited at the elbows; thumbs and halluces were broad. Many radiological abnormalities were noted: malformations of the cervical spine, pelvic abnormalities, bilateral coxa valga, genu valgum, small fibulae, pes equino varus, and 15 carpal bon

[Tympanoplasty with incus interposition].

We report on a 5-year experience with 44 patients (1980-1985) with incus interposition using a modelled or sculptured incus, either autograft or homograft, to correct ossicular discontinuity when a functional malleus and stapes are present. After a period of 21 months, 75% of our patients achieved an air-bone gap closure between 1 and 10 dB. The postoperative hearing results of patients with trauma and middle ear atelectases were better than the hearing results of patients with cholesteatomas. These long-term results are dependent upon intact malleus attic ligaments and an intact tensor tympan

Modifications of the stapedius muscle reflex under spontaneous and experimental brain-stem impairment.

An oscilloscopic plus a graphic method of recording of the stapedius muscle reflex has revealed peculiar modifications in the latency, threshold, and shape of development of the reflex in cases of brain-stem impairment. Many subjects have been examined who were affected by nucleo-reticular vestibular syndrome, vascular insufficiency, disseminated sclerosis, tumours, etc. and a comparison has also been made in normal subjects and in the same subjects during a temporary impairment of the brain-stem structures following barbiturate administration. The greatest semeiological value is attributed to
