V-A ECMO for neonatal coxsackievirus B fulminant myocarditis: a case report and literature review.

BACKGROUND: Neonatal (enteroviral) myocarditis (NM/NEM) is rare but unpredictable and devastating, with high mortality and morbidity. We report a case of neonatal coxsackievirus B (CVB) fulminant myocarditis successfully treated with veno-arterial extracorporeal membrane oxygenation (V-A ECMO). CASE PRESENTATION: A previously healthy 7-day-old boy presented with fever for 4 days. Progressive cardiac dysfunction (weak heart sounds, hepatomegaly, pulmonary edema, ascites, and oliguria), decreased left ventricular ejection fraction (LVEF) and fractional shortening (FS), transient ventricular fib

[Feasibility study of therapeutic exposure in virtual reality for pathological gamblers].

UNLABELLED: The experiment presented is the result of a partnership between the Paris-Nanterre University (CLIPSYD laboratory, EA 4430) and the CSAPA Imagine (Simone Veil Hospital, GH Eaubonne-Montmorency). Virtual reality immersion exposure for pathological gamblers is beneficial in the context of cognitive and behavioral interventions. It can be used to evoke the desire to play and to have access to automatic thoughts and mistaken beliefs of the players when they find themselves in a gambling context. Exposure is used until the urge to play diminishes and disappears which is the process of d

The Use of Virtual Reality to Facilitate Mindfulness Skills Training in Dialectical Behavioral Therapy for Spinal Cord Injury: A Case Study.

Introduction: Paralysis from a spinal cord injury (SCI) increases risk of psychological problems including suicide attempts, substance use disorder, negative emotions (e.g., anger), depression, anxiety, ASD/PTSD. Dialectical Behavioral Therapy® (DBT®) has been shown to be effective for treating similar psychological symptoms in non-SCI patient populations. The current study explored for the first time, the feasibility and clinical potential of using Immersive Virtual Reality (VR) enhanced DBT® Mindfulness skills training to help reduce psychological symptoms (negative emotions and anxiety, ASD

Simple identification of the third segment of the extracranial vertebral artery by extreme lateral inferior transcondylar-transtubercular exposure (ELITE).

PURPOSE: The exposure of the third segment of the extracranial vertebral artery (V3) is an important step in the extreme lateral inferior transcondylar-transtubercular exposure (ELITE) approach. The muscular suboccipital triangle provides one of the landmarks to identify the V3 segment; however, identification of this triangle and dissection of the V3 segment is not always straightforward in the actual surgery. Blind dissection below the level of the foramen magnum can lead to vertebral artery injury. While the surgeon may be able to readily define the V3 segment of the vertebral artery by fee

Human MT+ mediates perceptual filling-in during apparent motion.

During apparent motion, spatially distinct items presented in alternation cause the perception of a visual stimulus smoothly traversing the intervening space where no physical stimulus exists. We used fMRI to determine whether the perceptual 'filling-in' that underlies this phenomenon has an early or late cortical locus. Subjects viewed a display comprised of concentric rings that elicited apparent motion (two concentric rings presented in alternation), flicker (the same rings presented simultaneously), or real motion. We independently localized the cortical regions corresponding to the path o

Cortical representation of inward and outward radial motion in man.

We have used positron emission tomography to investigate the cortical areas of the normal human brain involved in processing inward (Expansion) and outward (Contraction) radial motion simulated with an optic flow stimulus. The optical flow display was made out of dots moving radially away from or toward the center of the display monitor. In the Control condition, the dots' motion was randomized in order to remove any sensation of radial motion. In the Expansion condition, several loci of activation were observed: visual areas V2-V3 and the superior parietal lobule (BA 7), predominantly in the

The effect of virtual body swapping with mental rehearsal on pain intensity and body perception disturbance in complex regional pain syndrome.

The present study investigated the effects of virtual body swapping with mental rehearsal on pain intensity and body perception disturbance (BPD) in patients with complex regional pain syndrome. Patients with complex regional pain syndrome were assigned randomly to three experimental groups: the 'virtual body swapping with mental rehearsal' (VBS) group (n=13), the 'watching movement only' group (n=13), and the 'mental rehearsal only' (n=13) group. Then, pretreatment pain intensity and BPD were compared with post-treatment perceptions. The results showed that pain intensity did not decrease sig

The effect of tactile discrimination training is enhanced when patients watch the reflected image of their unaffected limb during training.

In patients with phantom limb pain or complex regional pain syndrome (CRPS), sensory discrimination training increases tactile acuity, normalises cortical reorganisation and decreases pain. In healthy people, sensory cortical response, and tactile acuity, are greater if the participant looks towards the body part being stimulated. Does this effect enhance tactile training in CRPS patients? Ten patients underwent a 30-min tactile discrimination training session under four conditions (order randomised) in a 2 x 2 design: looking towards or away from the stimulated limb and seeing or not seeing s

Selective visual responses to expansion and rotation in the human MT complex revealed by functional magnetic resonance imaging adaptation.

Many neurons in the macaque visual area MSTd are sensitive to the global structure of a pattern of moving dots, responding to optic flow components such as expansion and rotation. Direct evidence for neurons with similar properties in humans has been lacking. We have explored sensitivity to optic flow in the human occipital cortex using an event-related functional magnetic resonance imaging adaptation paradigm. On each trial, two brief random-dot kinematograms were presented sequentially. Attention was controlled with a demanding task at fixation. In human MST, the compound response was smalle

The specificity of cortical region KO to depth structure.

Functional MRI studies have identified a cortical region designated as KO between retinotopic areas V3A/B and motion area V5 in human cortex as particularly responsive to motion-defined or kinetic borders. To determine the response of the KO region to more general aspects of structure, we used stereoscopic depth borders and disparate planes with no borders, together with three stimulus types that evoked no depth percept: luminance borders, line contours and illusory phase borders. Responses to these stimuli in the KO region were compared with the responses in retinotopically defined areas that

Chronic somatoparaphrenia: a follow-up study on two clinical cases.

Somatoparaphrenia consists in abnormal or bizarre verbal reports about some parts of the body. Such a pathological condition usually lasts for days or weeks and is variably associated with other cognitive defects. In the present paper we describe exceptionally long-lasting somatoparaphrenia in two focal brain-damaged patients: GA who had a right hemorrhagic fronto-parieto-temporal stroke and AC who developed a left ischemic parieto-occipital lesion. The presence and severity of somatoparaphrenia did not change in either patient during a 2-year follow-up, whereas the two patients showed differe

Dysfunction of early-stage visual processing in schizophrenia: harmonic analysis.

Schizophrenia is associated with severe neurocognitive deficits that constitute a core feature of the disorder. Deficits have been most extensively studied in relationship to higher-order processes. This study evaluated the integrity of early visual processing in order to evaluate the overall pattern of visual dysfunction in schizophrenia. Steady-state visual-evoked potentials (ssVEPs) were recorded over the occipital cortex (Oz) in patients with schizophrenia and schizoaffective disorder (N=26) and in age-matched comparison volunteers (N=22). Two stimuli were used: windmill-dartboard and part

Elementary visual hallucinations and their relationships to neural pattern-forming mechanisms.

An extraordinary variety of experimental (e.g., flicker, magnetic fields) and clinical (epilepsy, migraine) conditions give rise to a surprisingly common set of elementary hallucinations, including spots, geometric patterns, and jagged lines, some of which also have color, depth, motion, and texture. Many of these simple hallucinations fall into a small number of perceptual geometries-the Klüver forms-that (via a nonlinear mapping from retina to cortex) correspond to even simpler sets of oriented stripes of cortical activity (and their superpositions). Other simple hallucinations (phosphenes a

Clinical features of the localized girdle sensation of mid-trunk (false localizing sign) appeared in cervical compressive myelopathy patients.

Cervical compressive myelopathy patients sometimes show localized girdle sensation in the mid trunk (so-called false localizing sign). This symptom often confuses physicians, but the clinical features and mechanism of this symptom are still unclear. We investigated the clinical features and possible mechanism. In each of five cases of cervical compressive myelopathy disease with and without mid-truncal girdle sensation, the clinical features, degree and shape of cord compression were analysed. The girdle sensation was expressed as a vague or burning sensation, and was localized with a width of

"I know a little, but I can't do more": a COM-B-based qualitative study of CPAP use and sleep behaviour change among hospitalised patients with COMISA in China.

BACKGROUND: Co-morbid insomnia and sleep apnoea (COMISA) is a highly prevalent condition associated with worse health outcomes and greater treatment complexity than either disorder alone. Patients with COMISA demonstrate lower acceptance and use of continuous positive airway pressure (CPAP) therapy compared with those who have obstructive sleep apnoea (OSA) alone, yet the subjective mechanisms underlying this adherence penalty remain poorly understood. Qualitative research on CPAP use and sleep behaviour change in patients with COMISA is scarce, the COM-B (Capability, Opportunity, Motivation-B
