Tissue-engineered polymer-based periosteal bone grafts for maxillary sinus augmentation: five-year clinical results.

PURPOSE: Augmentation of the maxillary sinus with allogenic or alloplastic materials, as well as autologous bone grafts, has inherent disadvantages. Therefore, the aim of our study was to evaluate the long-term clinical repair effect of autologous periosteal bone grafts on atrophic maxillary bone. PATIENTS AND METHODS: In the present retrospective cohort study, augmentation of the edentulous atrophic posterior maxilla was performed using autologous tissue-engineered periosteal bone grafts based on bioresorbable polymer scaffolds and, in a 1-step procedure, simultaneous insertion of dental imp

Surgical and prosthodontic treatment of a patient with significant trauma to the middle and lower face secondary to a gunshot wound: a clinical report.

Large defects of dentofacial structures may result from trauma, disease (including neoplasms), and congenital anomalies. The location and size of the defects are related to difficulties that patients report relative to speech, mastication, swallowing, facial esthetics, and self-image. This article reports on the evaluation and treatment of a patient who suffered significant trauma to the lower and mid-face secondary to a gunshot injury. It describes the initial presentation, life-saving procedures, and subsequent bone grafts, implant placement, and prosthetic treatments required to rehabilitat

Long-term evaluation of implant survival in augmented sinuses: a case series.

This study compared bone grafting regimens and different implant surfaces used for sinus augmentation and presented long-term implant success rates in augmented sinuses. Two hundred fifty-seven consecutive patients with 625 implants were evaluated retrospectively. In phase 1, 188 sinuses were grafted with (1) autograft alone; (2) autograft + demineralized freeze-dried bone allograft (DFDBA) + absorbable hydroxyapatite (AHA) in a ratio of approximately 1:3:3; or (3) DFDBA + AHA + nonabsorbable HA (NHA) in a ratio of approximately 1:1:1. In phase 2, grafting regimen 3 (combination of DFDBA + AHA

Sinus floor augmentation with recombinant human growth and differentiation factor-5 (rhGDF-5): a pilot study in the Goettingen miniature pig comparing autogenous bone and rhGDF-5.

AIM: The aim of this study was to test the hypothesis that recombinant human growth and differentiation factor-5 (rhGDF-5) in combination with a beta-tricalcium phosphate (beta-TCP) scaffold material results in superior bone formation in sinus floor augmentations in miniature pigs compared with a particulated autogenous bone graft combined with the scaffold material. MATERIAL AND METHODS: Six adult female Goettingen minipigs underwent a maxillary sinus floor augmentation procedure. In a split-mouth design, the sinus floors were augmented with beta-TCP mixed with autogenous cortical bone chips

Precise communication is critical to the delivery of functional and aesthetic restorations. Interdisciplinary therapy integrates individual disciplines to function as a comprehensive unit and consistently provide predictable treatment results. Thorough diagnosis and treatment planning must be executed between every member of the perio-aesthetic-restorative team to ensure accurate communication and provision of the patient's desires. This two-part article compares the multidisciplinary treatment approach with the interdisciplinary approach in the replacement of congenitally missing lateral inci

Esthetic considerations when replacing missing maxillary incisors with implants: a clinical report.

Restoring missing maxillary incisors with implants is both challenging and demanding for the clinician. Decisions concerning the number, position, and diameter of implants are crucial to achieve an optimal esthetic result, especially in patients with increased vertical and horizontal overlap. This clinical report presents a treatment where 2 narrow implants, placed in the maxillary lateral incisor positions of the maxilla, support the restoration of the 4 missing incisors. Despite meticulous surgical and restorative treatment procedures, the use of gingiva-colored veneering material was requir

Clinical record-keeping for monitoring treatment outcomes in implant dentistry: a case study.

Sound clinical practice in implant dentistry depends upon defining the methodology that can successfully be used. This can be achieved by identifying literature relating to a specific technique prior to its use. However, clinical observations of outcomes within one's own practice are also very effective. This is particularly true as it becomes specific, by eliminating numerous confounding factors. Appropriate documentation, both clinical and radiographic, permits the practitioner to review clinical cases and establish the efficacy of the methodology used. This is most effective in terms of eva

Inlay-onlay grafting for three-dimensional reconstruction of the posterior atrophic maxilla with mandibular bone.

This prospective study describes and evaluates a surgical approach for 3D reconstruction of the posterior maxilla with autogenous mandibular bone in 16 patients (mean age 51 years). Bone blocks were harvested from the mandible and used as lateral or vertical block grafts (onlay); they were also partially milled and used for sinus elevation (inlay). In 4 cases, anorganic bovine bone was added at the periphery of the blocks. 4 months after grafting, implants were placed in a second operation and loaded after 12 weeks. Lateral and vertical augmentations were measured immediately after grafting an

Maxillary sinus augmentation following removal of a maxillary sinus pseudocyst after a shortened healing period.

PURPOSE: Dome-shaped radiopacities on the floor of the maxillary sinus are commonly interpreted as a sinus cyst on radiographs during dental implant planning. They might present an obstacle in sinus grafting, leading to bone graft failure or implant loss later. The therapeutic approaches to the removal of such cystic lesions and the following sinus augmentation are still controversial. The purpose of this article is to present a modified technique that can be used for predictable removal of a maxillary sinus cyst and sinus augmentation after a shortened healing period in patients with maxillar

Clinical failures and shortfalls of immediate implant procedures.

The esthetic outcome of dental implants in the esthetic zone is a major treatment concern in modern dentistry, especially in single-step procedures, which unite extraction, implantation, and augmentation into one surgical intervention, and are gaining popularity. Although these procedures offer advantages concerning early soft tissue preservation, they also show a high risk of complications when careful patient selection is neglected. Thus, the aim of this article is to illustrate the complexity and limitations of immediate implant procedures using two case reports. The clinical cases and scie
