External auditory canal stenting utilizing a useful rolled, tapered silastic sheet (RTSS) post middle ear surgery.

OBJECTIVE: To overcome the demerits of conventional postoperative aural packing, we developed a useful protocol for postoperative stenting of the external auditory canal after middle ear surgery which enables transcanal drainage and simultaneously allows for visual inspection and treatment of the canal, as necessary. METHODS: Twenty-four surgeries, 21 patients underwent tympanoplasty with a postaural incision. At the end of all surgical procedures, the external auditory canal was packed with a 0.3mm thickness rolled, tapered silastic sheet (RTSS) with antibiotic ointment applied to one surfac

Congenital incudostapedial anomalies in adult stapes surgery: a case-series review.

OBJECTIVES: The management of developmental anomalies of the ossicular chain and otic capsule in stapes surgery tests the otologist's diagnostic skills and flexibility in technique and prosthesis choice. We review our experience in managing various incudostapedial developmental anomalies discovered during middle ear exploration for suspected otosclerosis in adult patients. DESIGN: A retrospective case-series review was used. SETTING: The study was set at a military tertiary referral center. PATIENTS AND OTHER PARTICIPANTS: A review of 185 stapes procedures yielded 8 cases of identified cong

Preservation of the saccular nerve and of the vestibular evoked myogenic potential during vestibular schwannoma surgery.

OBJECTIVE: The aim of the study was to present a case report demonstrating that vestibular evoked myogenic potentials originate from the saccular nerve. STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral center. INTERVENTION: Vestibular evoked myogenic potentials are shown before and after operation in a patient with a surgically confirmed superior vestibular schwannoma (1.3 cm). MAIN OUTCOME MEASURES: Preoperative and postoperative audiometry, auditory brainstem response, caloric test, vestibular evoked myogenic potentials, and magnetic resonance imaging outcomes. RESULTS

External auditory canal closure in cochlear implant surgery.

OBJECTIVE: To evaluate surgical techniques and complications associated with external auditory canal (EAC) closure in cochlear implant surgery. STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral center with a large cochlear implant program. PATIENTS: Twenty-eight patients (8 adults and 20 children) underwent multichannel cochlear implantation with EAC closure. The follow-up periods ranged between 1 and 10 years. Nucleus 21-channel auditory brainstem implant in patients with previous tumour removal.

Three patients with neurofibromatosis type 2 who had undergone previous cochlear nerve tumour removal were implanted with the Nucleus 21-channel auditory brainstem implant (ABI). The time intervals between tumour removal and implantation were 4, 5, and 7 years, respectively. Total bilateral deafness was confirmed before implantation. One patient was also blind after acute intracranial hypertension. The translabyrinthine approach was used in all cases. The choice of side for implantation depended on pre-operative magnetic resonance imaging study, the facial nerve function, the presence of recur

The vestibulo-ocular reflex and subjective balance after vestibular schwannoma surgery.

OBJECTIVES/HYPOTHESIS: To evaluate the vestibulo-ocular reflex and its relationship with subjective balance in a long-term follow-up after vestibular schwannoma surgery. STUDY DESIGN: Retrospective cohorts study in a tertiary referral hospital. Forty-nine consecutive patients on which vestibular schwannoma surgery was performed at least 1 year before. METHODS: Patients are classified in accordance with the morphology of the vestibulo-ocular reflex (VOR) into two groups: Group A, in which covert and overt saccades always occur in an organized fashion; and group B, in which covert and overt sa

Preoperative, intraoperative, and postoperative results of bacterial culture from patients with chronic suppurative otitis media.

BACKGROUND: Cultures obtained from preoperative middle ear swabs from patients with chronic suppurative otitis media (CSOM) have traditionally been used to guide antibiotic selection. However, little is known about changes in the bacterial flora during surgery. OBJECTIVE: To analyze preoperative, intraoperative, and postoperative bacterial cultures of CSOM patients who underwent tympanomastoidectomy. METHODS: We retrospectively reviewed the medical records of 244 patients (113 male and 131 female subjects; 252 ears) diagnosed with CSOM between January 2006 and December 2008. Middle ear swabs

Perioperative complications after translabyrinthine removal of large or giant vestibular schwannoma: Outcomes for 123 patients.

CONCLUSION: Large vestibular schwannomas are benign but dangerous tumors. The translabyrinthine approach allows the surgeon to limit vital and functional complications due to the disease itself or to its surgical removal. OBJECTIVE: Morbi-mortality study focused on large vestibular schwannoma surgically treated by translabyrinthine removal. METHODS: This was a retrospective review of prospectively collected data in a series of 123 patients who underwent translabyrinthine removal of a large vestibular schwannoma (>4 cm in the cerebellopontine angle, stage IV). All surgical and medical complic

Vibrant Soundbridge for hearing restoration after chronic ear surgery.

INTRODUCTION: Middle ear surgery is primarily concerned with resolving the discharging pathology, in the case of chronic otitis media (COM), or with complete eradication, in case of cholesteatoma. Either of these procedures may require repeated surgeries, often resulting in severe mixed hearing impairment. A middle ear implant may be indicated in these cases instead of a hearing aid because the anatomical conditions in such cases often impede an adequate acoustic coupling. The objective of this study was to evaluate MED-EL Vibrant Soundbridge (VSB) implantation in patients with severe conducti

Nerve of origin, tumor size, hearing preservation, and facial nerve outcomes in 359 vestibular schwannoma resections at a tertiary care academic center.

OBJECTIVE: To determine nerve of origin, tumor size, hearing preservation rates, and facial nerve outcomes in a retrospective cohort study of patients undergoing translabyrinthine (TL), middle cranial fossa (MCF), and retrosigmoid/suboccipital (SO) approaches to vestibular schwannomas (VS). STUDY DESIGN: Retrospective. METHODS: Chart review. RESULTS: Patient charts from 231 TL, 70 MCF, 53 SO, and 5 combined TL/SO procedures for VS were evaluated in 356 patients. The inferior vestibular nerve (IVN) was the nerve of origin in 84 of 359 cases (23.3%), while the superior vestibular nerve (SVN)
