Normative Values of Tibial Tubercle-Trochlear Groove Distance and Tibial Tubercle-Posterior Cruciate Ligament Distance in Children.

BACKGROUND: Patellar instability is a common knee pathology in skeletally immature patients. In skeletally mature populations, a tibial tubercle-trochlear groove (TT-TG) distance of ≥20 mm is generally considered a pathological value. However, as pediatric patients grow and as the TT-TG distance varies with age, applying the same cutoff value as adult patients to them is unreasonable. PURPOSE/HYPOTHESIS: This study aimed to analyze the normative values of the TT-TG and tibial tubercle-posterior cruciate ligament (TT-PCL) distances in children with no patellofemoral instability and to propose

Excellent clinical and radiological outcomes after both open flake refixation and autologous chondrocyte implantation following acute patella dislocation and concomitant flake fractures.

PURPOSE: To investigate clinical and magnetic resonance (MR) imaging results of patients undergoing patella stabilization with either open flake refixation (oFR) or autologous chondrocyte implantation (ACI) and concomitant soft tissue patella stabilization after sustaining primary, acute patella dislocation with confirmed chondral and/or osteochondral flake fractures. It was hypothesized that refixation will lead to better results than ACI at mid-term follow-up. METHODS: A retrospective chart review was conducted to identify all patients undergoing oFR or ACI after sustaining (osteo-)chondral

Adding a tibial tubercle osteotomy with anteromedialisation to medial patellofemoral ligament reconstruction does not impact patient-reported outcomes in the treatment of patellar instability.

OBJECTIVES: An isolated medial patellofemoral ligament (MPFL) reconstruction (MPFLR) has been demonstrated to be an effective treatment option in the prevention of patellar instability, but there is growing support for performing a tibial tubercle osteotomy (TTO) in patients with an elevated tibial tubercle-trochlear groove distance. The purpose of this study was to evaluate the impact of adding a TTO to MPFLR on patient-reported outcomes. METHODS: A retrospective review of patients who underwent MPFLR with or without TTO with a minimum of 12-month follow-up was performed. Patients in both gr

Modified adductor sling technique: A surgical therapy for patellar instability in skeletally immature patients.

BACKGROUND: Due to open femoral physis the therapy of patellar instability in skeletally immature patients is challenging. We developed a modified surgical technique of the 'Adductor-Sling-Approach' by Sillanpää. The purpose of this study was to evaluate the clinical outcome of the operative technique and to analyse the reasons for failure. METHODS: Thirty 'modified adductor sling' reconstructions in 28 patients were included in the study. From 2010 to 2016 modified adductor sling reconstruction was performed by looping the gracilis or semitendinosus tendon around the adductor magnus tendon a

Treatment for lateral patellar impingement syndrome with arthroscopic lateral patelloplasty: a bidirectional cohort study.

BACKGROUND: Anterior knee pain is one of the most common musculoskeletal complaints of young patients. We notice that some patients had normal femoral trochlear, medial and lateral patellar retinaculum, and special patellar morphology, which resulted in a series of symptoms in the flexion of the knee due to the impingement of the lateral articular surface of the patella with the femur. We firstly termed this pathologic process as lateral patellar impingement syndrome (LPIS). This ambispective cohort study was to explore the curative effect of arthroscopic lateral patelloplasty for early LPIS.

Variation in position of the medial fabella in West Highland white terriers and other dogs.

OBJECTIVES: To investigate whether West Highland white terriers exhibit significant variation in the position of the medial fabella compared to both small and large breed dogs. METHODS: Criteria for the normal location of the medial fabella on a caudocranial radiograph were established. A retrospective analysis was undertaken of a consecutive series of bilateral caudocranial stifle joint radiographs from 70 West Highland white terriers, 100 small and 100 large control dogs by three reviewers. Medial fabellar location, cruciate ligament disease and medial patellar luxation were examined within

[Treatment of patellar instability in children and adolescents].

To address recurrent patellar instability in children and young adolescents a variety of therapeutic options exist either as non-operative or operative treatment. Recent options, such as reconstruction of the medial patellofemoral ligament have evoked a new focus of attention on this topic. The intention of this article is to present diverse therapeutic options and to evaluate them by reference to the current literature.

Distal alignment procedures for patellofemoral instability: comprehensive review of the literature.

Patellofemoral disorders are a common cause of complaint in adolescent patients. Several distal realignment procedures performed in isolation or combination with proximal alignment have been described. To clarify the role of distal alignment for patellofemoral instability, a systematic review of the literature was conducted. Two independent reviewers accessed the following databases: PubMed, Medline, CINAHL, Cochrane, EMBASE and Google Scholar. A total of 1478 patients with a mean age of 22.78 years were included. The mean follow-up was 86.53 months. The average Kujala score improved from 57.6

Return to sports and short-term follow-up of 101 cases of medial patellofemoral ligament reconstruction using gracilis tendon autograft in children and adolescents.

INTRODUCTION: An acute patellofemoral dislocation is the most common acute knee injury in children. Recent studies suggest up to 40% of immature patients may develop recurrent instability. MPFL reconstruction has become a mainstay for the treatment of patellofemoral instability. Aim of the recent study is to show our experiences of MPFL reconstruction in patellar instability in immature patients using a gracilis autograft in respect to return to sport, growth plate disorders and short-term results. MATERIALS AND METHODS: A total of 101 patients (50 females, 51 males) were included in this ret

The quadriceps active ratio: a dynamic MRI-based assessment of patellar height.

INTRODUCTION: Patella alta (PA) is one of the primary correctable risk factors for patellofemoral instability (PFI). Both an accurate diagnosis of PA and a clinically relevant target for correction are necessary for optimal treatment. An ideal test for PA should relate the position of the patella to the femur rather than tibia, should do so with the quadriceps contracted and the patellar tendon under tension and should have good sensitivity and specificity. None of the currently used radiographic tests PA meet these criteria, most of which are based on the position of the patella relative to t

The patellar insertion of the medial patellofemoral ligament in children: a cadaveric study.

BACKGROUND: The purpose of this study was to evaluate the medial patellofemoral ligament (MPFL) patellar insertion in skeletally immature anatomic specimens. METHODS: Nine pediatric cadaveric knee specimens were examined through gross dissection. Metallic markers were placed at the MPFL patellar insertion footprint. Computed tomographic scans for each specimen were analyzed. The MPFL insertion footprint width, patellar height, and patellar width were measured. The distance from the MPFL insertion footprint center to the midline of the patella was assessed. The proportion of the patella that t

What activities do patients with patellar instability perceive makes their patella unstable?

Patellar instability is a disabling musculoskeletal condition. Whilst previous texts have suggested that twisting activities may cause patients to experience instability symptoms, no studies have assessed which activities are related to the patient's perceived instability. The purpose of this study was to determine which activities and with what frequency patients with patellar instability symptoms, perceive their patella to be unstable. Ninety patients referred because of recurrent patellar instability were asked to assess the frequency with which they perceived patellar instability for 19 ev

Osseous and myotendinous injuries about the knee.

Osseous injuries may be articular, extra-articular, or physeal, and may be related to direct trauma, avulsion forces, or chronic microtrauma. Muscle and tendon injuries about the knee may occur alone or in association with more significant osseous and ligament injuries. Radiographs or computed radiography images remain an important screening technique for patients who have suspected knee injuries. After initial radiographs, MR imaging has become the most common modality for detection and complete evaluation of osseous and soft-tissue injuries about the knee when those radiographs and other tec

Biomechanical Evaluation of Medial Patellofemoral Ligament Reconstruction Grafts Fixed at Nonanatomic Femoral Insertion Points: MPFL Reconstruction And Femoral Tunnel Location.

BACKGROUND: Improved patient outcomes and decreased patellar instability have been reported after medial patellofemoral ligament (MPFL) reconstruction for recurrent lateral patellar dislocation; however, there is a lack of comparative evidence on functional outcomes associated with different femoral attachment sites for the MPFL graft. PURPOSE: To identify differences in MPFL reconstruction graft isometry with femoral tunnel malpositioning, specifically evaluating isometric differences as the femoral position is moved anterior, posterior, proximal, and distal relative to the Schöttle point, t

A Word of Caution for Future Studies in Patellofemoral Pain: A Systematic Review With Meta-analysis.

BACKGROUND: Patellar maltracking is widely accepted as an underlying mechanism of patellofemoral pain. However, methodological differences in the literature hinder our ability to generate a universal quantitative definition of pathological patellofemoral kinematics (patellar maltracking) in patellofemoral pain, leaving us unable to determine the cause of patellofemoral pain. PURPOSE: To systematically review the literature to provide evidence regarding the influence of confounding variables on patellofemoral kinematics.
