Autogenous bone grafts in the esthetic zone: optimizing the procedure using piezosurgery.

Soft and hard tissue defects pose a therapeutic challenge in modern implant dentistry. There are a multitude of surgical techniques available, and it is necessary to match the problem with the solution. This report describes the reconstruction of the alveolar ridge in the esthetic zone with the help of autogenous bone blocks harvested from the chin that were shaped to fit and stabilized at the recipient site. The procedures were performed using Piezosurgery, which made it possible to introduce surgical modifications and had a significant impact on the accuracy of the procedure. An observation

Effect of sinus membrane perforation on dental implant integration: a retrospective study on 128 patients.

A common complication of sinus augmentation is perforation of the sinus membrane during augmentation and/or implant placement. This retrospective study examines the effect of sinus membrane perforation with regard to graft survival and implant integration. A total of 175 sinuses were augmented with 115 of the membranes being reported intact at the time of surgery. A total of three infections occurred in patients who sustained perforated sinuses and one infection occurred in a patient who had an intact sinus. All four infections resolved after culture sensitivity and placement of the patient on

Evaluation of maxillary sinus anatomy by cone-beam CT prior to sinus floor elevation.

PURPOSE: Surgical complications during sinus floor elevation are frequently caused by septa in the maxillary sinus. In this study, the prevalence of septa was retrospectively determined via three-dimensional imaging to assess the necessity for appropriate imaging prior to sinus floor elevation. MATERIALS AND METHODS: The cone-beam computed tomography scans of 1,029 consecutive patients were evaluated to assess the prevalence of septa, taking into account patient age and sex as well as the number and orientation of septa and their locations. If septa were present, the height was measured and t

Morbidity after chin bone harvesting--a retrospective long-term follow-up study.

BACKGROUND: Resorption of the alveolar bone after tooth extraction may result in insufficient bone volume for implant placement. Augmentation of the resorbed site using autogenous bone grafts harvested from the maxillofacial region, for example, the chin, is a common method; however, it also involves donor site morbidity. Chin graft morbidity involves impaired sensibility in the frontal teeth, the gingival, and skin postoperatively. MATERIALS AND METHODS: A group of 60 patients with partial edentulism in the maxilla and insufficient bone volume for implant therapy were augmented with bone gra

Computer-guided implant placement in a patient with severe atrophy.

Computer software associated with imaging techniques facilitates diagnosis, planning, and management in cases of severe maxillary atrophy, by reducing the incidence of complications and improving the postoperative course and functional outcome. This article reports on a case of a 66-year-old woman with maxillary atrophy. Computer software was used to plan and position 6 maxillary and 4 mandibular implants, taking maximum advantage of the remaining bone. Bicorticalization was sought by angulation and implant fixation in the buttresses, while rehabilitation was carried out by means of a fixed sc

Predictable synthetic bone grafting procedures for implant reconstruction: part two.

When teeth are missing, the surrounding bone and soft tissue is challenged as a result of the natural resorptive process or from traumatic destruction subsequent to extraction. The diminished structural foundation for prosthetic reconstruction with or without implants can therefore be compromised. Recent technological innovations in computer hardware and software have given clinicians the tools to determine 3-dimensional anatomy, quality, and density of bone, which can aid in the diagnosis and treatment planning for reparative or augmentative grafting procedures. Advanced synthetic bioactive r

Aesthetic recovery of alveolar atrophy following autogenous onlay bone grafting using interconnected porous hydroxyapatite ceramics (IP-CHA) and resorbable poly-L-lactic/polyglycolic acid screws: case report.

BACKGROUND: Onlay bone grafting techniques have some problems related to the limited volume of autogenous grafted bone and need for surgery to remove bone fixing screws. Here, we report a case of horizontal alveolar ridge atrophy following resection of a maxillary bone cyst, in which autogenous onlay bone grafting with interconnected porous hydroxyapatite ceramics (IP-CHA) and bioresorbable poly-L-lactic/polyglycolic acid (PLLA-PGA) screws was utilized. CASE PRESENTATION: A 51-year-old man had aesthetic complications related to alveolar atrophy following maxillary bone cyst extraction. We per

[Massive sinus-lift procedures with beta-tricalcium phosphate: long-term results].

INTRODUCTION: The use of bone substitutes in massive sinus-lift pre-implant procedures remains controversial. The aim of our study was to evaluate the long-term reliability of pure-phase beta-tricalcium phosphate (betaTCP, Cerasorb, Curasan, Kleinostheim, Germany) used in this particular indication. MATERIAL AND METHOD: Twenty patients (33sinus) presenting with severe sinus floor atrophy (class 4 to 6 according to Cawood) and having undergone a sinus lift procedure by mean of betaTCP were followed in a prospective study between January 2002 and May 2008. The surgical approach was classical (u

Interposed bone grafts to accommodate endosteal implants for retaining mandibular overdentures. A 1-7 year follow-up study.

The results are reported on 34 edentulous patients, who underwent interposed autogenous bone graft augmentation in the symphysis of the mandible, combined with bone-hydroxylapatite onlay augmentation of the area posterior to the mental foramina. Two to four implants were placed in the grafted symphysis after 3-5 months. An overdenture was constructed three months later. The follow-up period ranged from one to seven years. An average loss of mandibular bone height of 10-13% was observed. The data provided no evidence of a further time-dependent resorption from two-and-half to seven years.
