Vertical translocation. Part II. Outcomes after surgical treatment of rheumatoid cervical myelopathy.

This is a prospective observational study in 116 patients with rheumatoid arthritis and vertical translocation who underwent cervical spine surgery after developing symptomatic myelopathy. These patients, whose mean age was 62 years, had suffered from rheumatoid arthritis for almost 25 years. Surgery was performed via a combination of anterior (67 transoral decompressions) and posterior approaches. Surgical morbidity was recorded in 39% of patients, with a 30-day mortality rate of 10.3%, which was largely related to poor preoperative neurological grade. Neurological improvement of at least one

Minimally Invasive Percutaneous C1-C2 Fixation Using an Intraoperative Three-Dimensional Imaging-Based Navigation System for Management of Odontoid Fractures.

BACKGROUND: Odontoid fractures are the most common lesions of the upper cervical spine in patients >70 years old. These fractures are associated with frequent comorbidities, and minimally invasive techniques might have the potential to reduce postoperative complications while ensuring better precision. The aim of this study was to report an innovative technique and preliminary results of minimally invasive atlantoaxial fixation using a three-dimensional intraoperative navigation system and percutaneous approach. METHODS: This retrospective study included 5 consecutive patients with an acute t

Improvements in Neck Pain and Disability Following C1-C2 Posterior Cervical Instrumentation and Fusion for Atlanto-Axial Osteoarthritis.

OBJECTIVE: Symptomatic Atlanto-axial (C1-2) osteoarthritis (AAOA) is a common phenomenon in elderly patients; however, there is a paucity of data on the effectiveness of posterior atlanto-axial fusion (PAAF) for this condition. To this end, here we assess changes in patient-reported outcomes and neck-related disability in adult patients undergoing PAAF for symptomatic C1-2 AAOA. METHODS: In this retrospective study, the clinical records of consecutive patients with symptomatic AAOA who underwent PAAF between 2004 and 2017 were reviewed. Patient demographics, comorbidities, intraoperative and

Minimally invasive atlantoaxial fusion.

BACKGROUND: C1-C2 fusion has significantly advanced from predominantly wiring/cable modalities to more biomechanically stable screw-rod techniques. Minimally invasive surgical techniques represents the most recent modification of atlantoaxial fixation. The indications, rationale, and surgical technique of this novel procedure are described. METHODS: Six patients requiring C1-C2 fusion (5 type II odontoid fractures and 1 os odontoideum) underwent minimally invasive C1-C2 fusion over a 2-year period. The cohort consisted of 5 men and 1 woman with a mean age of 51 years (age range, 39-64 y). All

Craniocervical tuberculosis: protocol of surgical management.

OBJECTIVE: Craniovertebral junction tuberculosis (CVJ-TB) is rare and occurs in only 0.3 to 1% of patients with tuberculous spondylitis. In the available literature, the treatment options offered for this entity have ranged from a purely conservative approach to radical surgery without well-defined guidelines. In this study, we attempt to establish the most effective strategy for the management of this condition. METHODS: Twenty-five patients with CVJ-TB were treated during the past 8 years. Severe neck pain, restricted neck movement, and myelopathy were the predominant symptoms. The patients

Use of allograft bone for posterior C1-2 fusion.

OBJECT: An iliac crest autograft is the gold standard for bone grafting in posterior atlantoaxial arthrodesis but can be associated with significant donor-site morbidity. Conversely, an allograft has historically performed suboptimally for atlantoaxial arthrodesis as an onlay graft. The authors have modified a bone grafting technique to allow placement of a bicortical iliac crest allograft in an interpositional manner, and they evaluated it as an alternative to an autograft in posterior atlantoaxial arthrodesis. METHODS: The records of 89 consecutive patients in whom C1-2 arthrodesis was perf

Percutaneous C1C2 transarticular screw fixation for atlantoaxial osteoarthritis with 3D navigation: technical note and functional outcomes.

PURPOSE: Atlantoaxial osteoarthritis is a debilitating condition often resistant to conservative management. In refractory cases, C1-C2 fusion becomes a surgical option. Recent advancements, particularly in 3D navigation, have allowed for more precise and minimally invasive approaches, though there are limited reports on isolated percutaneous C1-C2 arthrodesis for osteoarthritis. This study aims to evaluate the feasibility, accuracy, and clinical outcomes of percutaneous C1-C2 transarticular screw fixation using 3D navigation in patients with disabling atlantoaxial osteoarthritis. METHODS: Te

Minimally invasive posterior atlantoaxial fusion: a cadaveric and clinical feasibility study.

OBJECTIVE: Minimally invasive surgical (MIS) techniques have several potential advantages for the patient over open techniques, including decreased loss of blood, postoperative pain, and recovery time. We aimed to demonstrate atlantoaxial fusion by using the MIS techniques, shown previously in only two reports. A smaller operating corridor with MIS may increase difficulty of instrumentation placement and risk of instrumentation-related complications. METHODS: Five cadaveric specimens were studied presurgically with computed tomography (CT) to determine bony anatomy, vertebral artery course, a
