Functional hearing results in revision stapes surgery.

OBJECTIVE: The object of our study was to review the results of 63 revision stapes surgeries performed from 1978 to 1994. RESULTS: The most common cause of failure was the displacement of the prosthesis, followed by ossicular chain problems and oval window fibrosis. Postoperative hearing improvement within a 20-dB air-bone gap was achieved in 58.7% of the patients. Hearing gain was closely linked to the operative findings. Better results occurred when prosthesis problems were found. Evaluation of the hearing results by using Glasgow benefit plot gave evidence of symmetric normal hearing in on

Otosclerosis--an inflammatory disease of the otic capsule of viral aetiology?

Fragments of otospongiotic and otosclerotic footplates were investigated by immunohistochemical methods. Antibodies IgG, IgA, IgM were found to be bound to the vascular connective tissue of the resorption lacunae, IgG also to osteocytes. The application of antibodies against mumps, measles and rubella antigens showed the expression of the relevant viral antigens in the large cells of the resorption lacunae, in the vascular connective tissue, and in osteocytes, osteoclasts and chondrocytes, present in or around the otospongiotic areas. In the sclerotic stage only the perivascular connective tis

Hearing function and ossicular deformities and fractures in the oim mouse model of brittle bone disease.

Hearing loss is a prevalent symptom of osteogenesis imperfecta (OI), a group of collagen type I-related skeletal disorders, commonly known as brittle bone disease. Clinical manifestation of hearing loss in OI often presents with stapes footplate fixation and hypodense foci in the otic capsule. However, the etiology and evolution of OI-hearing loss and its relation to bone abnormalities are still unknown. This study investigates the onset, severity, and progression of hearing loss in the homozygous oim mouse model of severe OI Type III, which is reported to exhibit hearing loss at 11-12 weeks o

Prolyl 3-hydroxylase-1 null mice exhibit hearing impairment and abnormal morphology of the middle ear bone joints.

Prolyl 3-hydroxylase1 (P3H1) is a collagen modifying enzyme which hydroxylates certain prolines in the Xaa position of conventional GlyXaaYaa triple helical sequence. Recent investigations have revealed that mutations in the LEPRE1 (gene encoding for P3H1) cause severe osteogenesis imperfecta (OI) in humans. Similarly LEPRE1 knockout mice display an OI-like phenotype. Significant hearing loss is a common problem for people with osteogenesis imperfecta. Here we report that hearing of the P3H1 null mice is substantially affected. Auditory brainstem responses (ABRs) of the P3H1 null mice show an

Study on normal and otosclerotic bone cell cultures: an advance in understanding the pathogenesis of otosclerosis.

The authors first reviewed the main theories concerning the pathogenesis of otosclerosis and studied the morphologic and functional characteristics of cell cultures derived from normal and otosclerotic bones. Light transmission and scanning electron microscopy did not permit definite identification of the cultured cells as predominantly osteoblasts, nor did these techniques show significant differences between cultured cells derived from normal and pathologic bone. Functional tests of the cell cultures proved more interesting. First, the bony nature of the cultured cells was demonstrated by st

Stapes anomaly and cochlear sensory cell changes. A scanning electron microscope study.

This paper reports the pathological findings in the cochleae of a 66-year-old man examined under a scanning electron microscope (SEM). Columella-shaped stapes, which were the only anomalous changes in the middle ear cleft, were found in both ears. Beside this, giant hair formation and fusion of stereocilia in the inner sensory cells were also observed in the apical part of the lower basal turn. Since the patient had no history of ototoxic drug use or other ear disease episodes, it is suggested that these inner hair cell changes might have been caused by the conductive disorder accompanying the

The role of the stapedius reflex in poststimulatory auditory fatigue.

The human ear is very resistant against noise-induced damage in the low frequency range. The aim of the present study was to investigate whether or not the stapedium reflex is of any importance for this resistance. Subjects with peripheral facial palsy (Bell's palsy) including unilateral stapedius muscle paralysis were exposed to several different levels of narrow band noise centered at 0.5 and 2.0 kHz. Temporary threshold shift (TTS) at 0.75 kHz was significantly higher in the affected than in the nonaffected ear after 0.5 kHz noise at and above 110 dB SPL. After the exposure with 2.0 kHz nar

Profile of paediatric tuberculosis mastoiditis - a case series.

BACKGROUND: Tuberculosis (TB) otitis media is an uncommon site of extrapulmonary TB and can primarily present as a complicated TB mastoiditis (TBM). This complication is rare in children, even in TB endemic areas but necessitates early identification as delays can lead to severe morbidities. We describe the clinical characteristics as a case series to raise awareness of the condition, and highlight fundamentals related to diagnosis and management. METHODS: A retrospective chart review of clinical and radiological information of five children with TBM seen at the Red Cross War Memorial Childre

Novel variant in Sp7/Osx associated with recessive osteogenesis imperfecta with bone fragility and hearing impairment.

Osteogenesis imperfecta (OI) is a connective tissue disorder characterized by low bone density and recurrent fractures with a wide genotypic and phenotypic spectrum. Common features include short stature, opalescent teeth, blue sclerae and hearing impairment. The majority (>90%) of patients with OI have autosomal dominant variants in COL1A1/COL1A2, which lead to defects in type 1 collagen. More recently, numerous recessive variants involving other genes have also been identified. Sp7/Osx gene, is a protein coding gene that encodes a zinc finger transcription factor, osterix, which is a member
