Increased medial meniscus extrusion led to worse clinical outcomes after medial opening-wedge high tibial osteotomy.

PURPOSE: Given that no studies have assessed the correlation between improvements in medial meniscus extrusion (MME) and clinical outcomes after medial opening-wedge high tibial osteotomy (MOWHTO), the present study aimed to measure the improvement in MME after MOWHTO and to investigate the correlation between the remaining postoperative MME and MOWHTO clinical outcomes by subgroup analysis. METHODS: This study included 79 patients (80 knees) who underwent MOWHTO with a minimum follow-up of 2 years. MME was measured pre- and postoperatively through magnetic resonance imaging after an average

Survivorship and outcomes of intra- and perimeniscal corticosteroids injections with structured physiotherapy for degenerative medial meniscus tears.

BACKGROUND: Degenerative medial meniscus tears are a common pathology in the general population. Recent randomized trials demonstrated non-superiority of arthroscopic partial meniscectomy over conservative management. However, there is a paucity of information regarding the outcomes of combined conservative treatments. HYPOTHESIS: It was hypothesized that combined intra- and perimeniscal corticosteroid injections with structured physiotherapy, for degenerative medial meniscus tears, would result in high surgery-free and second injection-free survivorship. METHODS: A retrospective review of 6

Deep-learning-assisted diagnosis for knee magnetic resonance imaging: Development and retrospective validation of MRNet.

BACKGROUND: Magnetic resonance imaging (MRI) of the knee is the preferred method for diagnosing knee injuries. However, interpretation of knee MRI is time-intensive and subject to diagnostic error and variability. An automated system for interpreting knee MRI could prioritize high-risk patients and assist clinicians in making diagnoses. Deep learning methods, in being able to automatically learn layers of features, are well suited for modeling the complex relationships between medical images and their interpretations. In this study we developed a deep learning model for detecting general abnor

Joint distraction attenuates osteoarthritis by reducing secondary inflammation, cartilage degeneration and subchondral bone aberrant change.

OBJECTIVE: Osteoarthritis (OA) is a progressive joint disorder. To date, there is not effective medical therapy. Joint distraction has given us hope for slowing down the OA progression. In this study, we investigated the benefits of joint distraction in OA rat model and the probable underlying mechanisms. METHODS: OA was induced in the right knee joint of rats through anterior cruciate ligament transaction (ACLT) plus medial meniscus resection. The animals were randomized into three groups: two groups were treated with an external fixator for a subsequent 3 weeks, one with and one without joi

Arthroscopy of the knee in children.

Diagnostic arthroscopy was performed in 174 consecutive patients under the age of 18 years. Eight arthroscopies were bilateral. The clinical diagnosis was confirmed in only three of 17 knees in children under aged 14 years and in 44 per cent of the knees in older children. The most commonly disproved diagnoses were torn meniscus and chondromalacia patella. Arthroscopy seldom changed the diagnosis in cases of osteochondritis dissecans. In 38 per cent of the knees, arthroscopy revealed no surgical treatable abnormality, and unnecessary arthrotomy was avoided. Arthroscopy in acute hemarthrosis re

Predictors of persistent complaints after a knee injury in primary care.

BACKGROUND: Prognosis of persistent complaints after knee injury is based on secondary care populations. In a primary care setting, however, no studies have addressed this issue. AIM: To identify possible predictors of persistent complaints 1 year after a knee injury. These predictors are important for guiding the GP's therapeutic management, and giving advice to patients about work and/or sports-related activities. DESIGN AND SETTING: Primary care prospective cohort study with a 1-year follow-up period in five municipalities in the southwest region of the Netherlands. METHOD: Patients who

Opening-Wedge Distal Femoral Osteotomy With Concomitant Medial Meniscal Root Repair.

BACKGROUND: Valgus malalignment leads to excessive loading of the lateral compartment and has been associated with lateral compartment meniscal injuries and early-onset knee osteoarthritis. Distal femoral osteotomy (DFO) is a treatment option that allows correction of the mechanical access to offload the lateral compartment. Lateral opening-wedge distal femoral osteotomy and medial meniscal root repairs are both aimed to address the pathologic biomechanics of the tibiofemoral joint and when successful can delay or even prevent progression to end-stage osteoarthritic changes in the lateral and

Will patients with meniscal tear who continue to experience pain following a course of Physical Therapy (PT) enroll in a trial comparing further nonoperative therapy with surgery? a qualitative study.

BACKGROUND: We aimed to conduct a qualitative prospective preference assessment (PPA) to assess participants’ willingness to enroll in a planned randomized controlled trial (RCT). The planned RCT would enroll participants with meniscal tear with persistent knee pain following a course of physical therapy (PT) and would compare outcomes of arthroscopic partial meniscectomy (APM) vs. enhanced conservative care. METHODS: We identified participants 45–85 years old with suspected meniscal tear who were referred to PT. After 10 weeks of PT, participants were sent a questionnaire that assessed knee p

Maternal omega-6 to omega-3 conversion reduces osteoarthritis pain-related behaviors in adult female offspring.

BACKGROUND: A parental high-fat diet (HFD) enriched in inflammatory omega-6 polyunsaturated fatty acids (n-6 PUFAs) increases the severity of post-traumatic osteoarthritis (PTOA) in adult offspring, a risk factor which lacks therapeutic interventions. This study investigated whether rebalancing the maternal omega-6 to omega-3 fatty acid (n-6/n-3 FA) ratio during pregnancy and lactation could mitigate PTOA severity in adult offspring. METHODS: Wildtype (WT) and fat-1 female mice, which endogenously convert n-6 to n-3 PUFAs, were fed either a control or n-6-enriched HFD and mated to control-fed

Extrusion of tibial tunnel bioabsorbable screw 15 months after anterior cruciate ligament reconstruction.

The use of bioabsorbable implants in orthopaedic surgery has become a common practice. Despite their many theoretical advantages, recent reports have proposed certain concerns and complications associated with their use. We report the case of a bioabsorbable screw that extruded from the tibial insertion site 15 months after anterior cruciate ligament reconstruction.

Clinical, radiological, and morphological evaluations of posterior horn tears of the lateral meniscus left in situ during anterior cruciate ligament reconstruction.

BACKGROUND: Recent studies have shown that lateral menisci have a higher healing potential and that they can be treated successfully without symptoms by being left in situ during anterior cruciate ligament (ACL) reconstruction. However, few studies have reported morphological results. HYPOTHESIS: Stable posterior horn tears of the lateral meniscus left in situ during ACL reconstruction could be healed spontaneously and would result in not only successful clinical outcomes but also morphological restoration. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Among 367 patients who un
