Orbitofrontal epilepsy: Case series and review of literature.

BACKGROUND: Orbitofrontal epilepsy (OFE) is less known and is poorly characterized in comparison with temporal lobe epilepsy, partly because it is rare and possibly because it is unrecognized and therefore underestimated. OBJECTIVE: This paper aimed to better characterize seizure semiology, presurgical findings, and surgical outcomes in patients with OFE. METHODS: We retrospectively reviewed all confidently established OFE cases from six Canadian epilepsy monitoring units between 1988 and 2014, and in the literature between 1972 and 2017. Inclusion criteria were identification of an epilepto

Perceptual Integration Deficits in Autism Spectrum Disorders Are Associated with Reduced Interhemispheric Gamma-Band Coherence.

UNLABELLED: The integration of visual details into a holistic percept is essential for object recognition. This integration has been reported as a key deficit in patients with autism spectrum disorders (ASDs). The weak central coherence account posits an altered disposition to integrate features into a coherent whole in ASD. Here, we test the hypothesis that such weak perceptual coherence may be reflected in weak neural coherence across different cortical sites. We recorded magnetoencephalography from 20 adult human participants with ASD and 20 matched controls, who performed a slit-viewing pa

Inattentional Deafness: Visual Load Leads to Time-Specific Suppression of Auditory Evoked Responses.

UNLABELLED: Due to capacity limits on perception, conditions of high perceptual load lead to reduced processing of unattended stimuli (Lavie et al., 2014). Accumulating work demonstrates the effects of visual perceptual load on visual cortex responses, but the effects on auditory processing remain poorly understood. Here we establish the neural mechanisms underlying "inattentional deafness"--the failure to perceive auditory stimuli under high visual perceptual load. Participants performed a visual search task of low (target dissimilar to nontarget items) or high (target similar to nontarget it

A randomized controlled trial investigating the neurocognitive effects of Lacprodan® PL-20, a phospholipid-rich milk protein concentrate, in elderly participants with age-associated memory impairment: the Phospholipid Intervention for Cognitive Ageing Reversal (PLICAR): study protocol for a randomized controlled trial.

BACKGROUND: Age-related cognitive decline (ARCD) is of major societal concern in an ageing population, with the development of dietary supplements providing a promising avenue for amelioration of associated deficits. Despite initial interest in the use of phospholipids (PLs) for ARCD, in recent years there has been a hiatus in such research. Because of safety concerns regarding PLs derived from bovine cortex, and the equivocal efficacy of soybean-derived PLs, there is an important need for the development of new PL alternatives. Phospholipids derived from milk proteins represent one potential

International Consensus on the Evaluation and Management of Hypothalamic Hamartomas: Results From a Modified Delphi Survey.

BACKGROUND AND OBJECTIVES: Hypothalamic hamartomas (HH) are rare brain lesions associated with epilepsy and numerous comorbidities. Worldwide treatment is varied. There is a paucity of high-quality evidence to guide treatment. This study aimed to establish expert consensus on the evaluation and management of HH. METHODS: A modified Delphi survey was designed by the Medical Advisory Board of Hope for Hypothalamic Hamartomas and was conducted among 17 International League Against Epilepsy level II epilepsy surgery centers. The survey included 257 questions in round 1 and 81 refined questions in

Region-Specific Brain Imaging Correlates of Repetitive Head Impact Exposure in Nonconcussed High School Football Players.

Repetitive, nonconcussive head impacts in collision sports are associated with lasting neurophysiological effects. This study investigates region-specific changes in neocortical mean diffusivity (MD) and resting-state functional magnetic resonance imaging (fMRI) power spectral density (fMRI-PSD) that correlate with cumulative nonconcussive linear risk-weighted exposure (RWE-Lin) sustained over one high school football season. Pre- to postseason percent changes in MD (%ΔMD) and fMRI-PSD (%ΔfMRI-PSD) were calculated for football players without diagnosed concussions during the season of observat

Seizure outcomes after laser interstitial thermal therapy for pediatric extratemporal lobe epilepsy.

OBJECTIVE: Approximately one-third of patients with epilepsy develop drug-resistant epilepsy (DRE). Extratemporal lobe epilepsy (ETLE) represents 30%-40% of focal epilepsy cases. ETLE poses significant challenges in localization and treatment because these patients often have diffuse and complex epileptogenic networks. Laser interstitial thermal therapy (LITT) has emerged as a minimally invasive alternative for localizable DRE, yet data for its use in the pediatric ETLE population remain limited. This study aimed to evaluate the safety and efficacy of LITT in pediatric ETLE and identify predic

SEEG-Guided Radio-Frequency Thermocoagulation of the Epileptogenic Networks: Its Utility for Both Treatment and Validation for Localizing Epileptogenic Networks.

INTRODUCTION: Stereoelectroencephalography-guided radio-frequency thermocoagulation (SEEG-RFTC) is a minimally invasive technique whereby radio-frequency thermocoagulation is performed using SEEG electrodes, following recording and stimulation. It helps to disconnect/disrupt or ablate the epileptogenic networks and provides both therapeutic and diagnostic abilities. METHODS: This study is a retrospective study (2016-2024). All underwent comprehensive epilepsy surgery workup (video EEG, MRI, ictal-SPECT, PET, and magnetoencephalography). SEEG was placed using robotic guidance. Recording of hab

Associations between testing and treatment pathways in lesional temporal or extratemporal epilepsy: A census survey of NAEC center directors.

OBJECTIVE: The evaluation to determine candidacy and treatment for epilepsy surgery in persons with drug-resistant epilepsy (DRE) is not uniform. Many non-invasive and invasive tests are available to ascertain an appropriate treatment strategy. This study examines expert response to clinical vignettes of magnetic resonance imaging (MRI)-positive lesional focal cortical dysplasia in both temporal and extratemporal epilepsy to identify associations in evaluations and treatment choice. METHODS: We analyzed annual report data and a supplemental epilepsy practice survey reported in 2020 from 206 a

Phasic, Event-Related Transcutaneous Auricular Vagus Nerve Stimulation Modifies Behavioral, Pupillary, and Low-Frequency Oscillatory Power Responses.

Transcutaneous auricular vagus nerve stimulation (taVNS) has been proposed to activate the locus ceruleus-noradrenaline (LC-NA) system. However, previous studies failed to find consistent modulatory effects of taVNS on LC-NA biomarkers. Previous studies suggest that phasic taVNS may be capable of modulating LC-NA biomarkers such as pupil dilation and alpha oscillations. However, it is unclear whether these effects extend beyond pure sensory vagal nerve responses. Critically, the potential of the pupillary light reflex as an additional taVNS biomarker has not been explored so far. Here, we appl

Functional hemispherectomy: can preoperative imaging predict outcome?

OBJECTIVE: Although hemispherectomy is an effective treatment for children with intractable hemispheric epilepsy syndromes, as many as 40% of patients eventually develop seizure recurrence. The causes of seizure recurrence in these patients are incompletely understood. The authors sought to evaluate the efficacy of hemispherectomy at their center and determine whether contralateral MRI abnormalities can predict seizure recurrence. METHODS: A retrospective review of consecutive hemispherectomies performed at Miami Children's Hospital between January 2000 and June 2014 was performed. Time-to-ev

Efficacy and mechanism of sub-sensory sacral (optimised) neuromodulation in adults with faecal incontinence: study protocol for a randomised controlled trial.

BACKGROUND: Faecal incontinence (FI) is a substantial health problem with a prevalence of approximately 8% in community-dwelling populations. Sacral neuromodulation (SNM) is considered the first-line surgical treatment option in adults with FI in whom conservative therapies have failed. The clinical efficacy of SNM has never been rigorously determined in a trial setting and the underlying mechanism of action remains unclear. METHODS/DESIGN: The design encompasses a multicentre, randomised, double-blind crossover trial and cohort follow-up study. Ninety participants will be randomised to one o

Long-term effects of cranial irradiation and intrathecal chemotherapy in treatment of childhood leukemia: a MEG study of power spectrum and correlated cognitive dysfunction.

BACKGROUND: Prophylaxis to prevent relapses in the central nervous system after childhood acute lymphoblastic leukemia (ALL) used to consist of both intrathecal chemotherapy (CT) and cranial irradiation (CRT). CRT was mostly abolished in the eighties because of its neurotoxicity, and replaced with more intensive intrathecal CT. In this study, a group of survivors treated with CRT before 1983 and another group treated without CRT thereafter are investigated 20-25 years later, giving a much stronger perspective on long-term quality of life than previous studies. The outcomes will help to better

Haptic contents of a movie dynamically engage the spectator's sensorimotor cortex.

Observation of another person's actions and feelings activates brain areas that support similar functions in the observer, thereby facilitating inferences about the other's mental and bodily states. In real life, events eliciting this kind of vicarious brain activations are intermingled with other complex, ever-changing stimuli in the environment. One practical approach to study the neural underpinnings of real-life vicarious perception is to image brain activity during movie viewing. Here the goal was to find out how observed haptic events in a silent movie would affect the spectator's sensor

Prepare for scare-Impact of threat predictability on affective visual processing in spider phobia.

The visual processing of emotional faces is influenced by individual's level of stress and anxiety. Valence unspecific affective processing is expected to be influenced by predictability of threat. Using a design of phasic fear (predictable threat), sustained anxiety (unpredictable threat) and safety (no threat), we investigated the magnetoencephalographic correlates and temporal dynamics of emotional face processing in a sample of phobic patients. Compared to non-anxious controls, phobic individuals revealed decreased parietal emotional attention processes during affective processing at mid-l

MEG recording from the human ventro-occipital cortex in response to isoluminant color stimulation.

In contrast to PET and fMRI studies, color-selective responses from the ventro-occipital area have rarely been reported in MEG studies. We tried to minimize the stimulation to all areas in the visual system except the color-processing ones by using a color space based on psychophysical and physiological knowledge in order to maximize the signal-to-noise ratio for MEG responses from the ventro-occipital area. MEG obtained from long intermittent reversals (2.0-3.5 s) of isoluminant chromatic gratings showed two major peaks at the latencies of approximately 100 and 150 ms. The estimated location

Oscillatory characteristics of face-evoked neuromagnetic responses.

To study the oscillatory activities during face processing, we recorded magnetoencephalographic responses in 8 healthy subjects to upright and inverted human faces, and obtained the time-frequency representation by using wavelet transform. Delta to beta activities were clearly increased at 140-210 ms after stimulus onset in the bilateral occipitotemporal (OT) areas (t(7)>5.5; p<0.001), with larger power for theta, alpha and beta over the right side. Notably, more increase alpha activity for inverted than upright face condition was observed in the right OT area. Our results suggest that 4-25 Hz

Multimodal functional imaging of prolonged neurological deficits in a patient suffering from familial hemiplegic migraine.

The case of a patient with familial hemiplegic migraine (FHM) suffering from prolonged right sided hemiparesis and aphasia that persisted for more than 10 days is reported. The symptoms were accompanied by slowing of the magnetoencephalogram over the left hemisphere, which normalized parallel to the clinical improvement. Positron emission tomography obtained on the 6th day revealed glucose-hypometabolism (hemispheric difference > or =10%) in left hemisphere's fronto-basal cortex, caudate nucleus, and thalamus. In contrast, magnetic resonance imaging including perfusion and diffusion weighted i
