Idiopathic hypertrophic cranial pachymeningitis causing progressive polyneuropathies in a child.

We describe the clinical manifestations, radiological features and response to therapy of a 10-year-old child with idiopathic hypertrophic cranial pachymeningitis, a rare condition with potentially severe disabling neurological consequences. The child presented with progressive cranial polyneuropathies secondary to dural compression of the neural and vascular structures within the cavernous sinus. Corticosteroids and methotrexate proved effective in inducing near complete and sustained remission of the condition. To our knowledge, this is the first case reported of idiopathic hypertrophy crani

[Simulating the outcome of ptosis and squint surgery with digital photomontage].

BACKGROUND: When being informed preoperatively about the risks and prospects of ptosis and squint surgery patients may wish to see pictures of the possible outcome. In most cases pre- and postoperative prints of patients with similar problems will be sufficient. But, to come to a decision, some patients need portrait prints of their own face modified to show the changes that could be produced by the surgery proposed. Such photo prints can be rather easily produced with modern computer and software equipment. MATERIALS AND METHODS: Portrait photos have been shot with a digital camera and have

Abducens-oculomotor synkinesis associated with internuclear ophthalmoplegia and acquired abducens nerve palsy.

We report a case of acquired abducens-oculomotor synkinesis, internuclear ophthalmoplegia, and abducens nerve palsy following brainstem hemorrhage. Weakening surgery on the medial rectus muscle did not eliminate the synergistic movement. The mechanism responsible for the abducens-oculomotor synkinesis is discussed.

[Complicated oculomotor disorders evaluated by the medical expert].

The association of sensory and motor defects may lead the medical expert to a false estimation of a patient's impairment, especially (1) if he simply adds to the sensory impairment of one eye the motor impairment of the other, without thinking of the eyes as a paired organ; (2) if he underestimates a cumulation of sensory and motor defects in the case of partial adaptation, and (3) if he judges from dyskinetic behavioral characteristics that the patient is blind. Several clinical cases illustrating these problems and allowing the following recommendations are described: (1) Prior to simply sum

[Ophthalmologic findings in multiple handicapped patients].

PURPOSE: To report the visual findings in patients with multiple handicaps. METHODS: Two hundred and seventy-four patients cared for at the Pediatric and Strabismus Ophthalmology Department of the "Fundação Altino Ventura" - Brazilian National Health System, were examined from June to September 2004. Age varied from 0.1 to 20 years with a median of 5. RESULTS: The majority of the patients (61.3%) presented good visual acuity; however low visual acuity (< 20/80) was observed in 38.7% of the patients. Heterotropias were observed in 66 patients (24.0%); astigmatism (53.2%) and hyperopia (29.0%)

Sherrington innervational surgery in the treatment of chronic sixth nerve paresis.

INTRODUCTION AND PURPOSE: To describe a new operation to treat unilateral chronic sixth nerve paresis based on Sherrington's innervational law. A recession of the medial rectus (MR) in the good eye, yoke to the paretic lateral rectus (LR), will have the reciprocal innervational effect of relaxing the contracture of the contralateral MR and by doing so will enhance the effect of a weakening procedure performed on this muscle. The goal of this study was to eliminate diplopia in primary position by improving the function of the paretic LR and reducing the contracture of its antagonist MR. METHOD

A clinical study of the craniofacial features in Apert syndrome.

A clinical study of the craniofacial features in Apert syndrome is based on our experience with 136 cases. Characteristics included hyperacrobrachycephaly, steep wide forehead, flat occiput, common craniofacial asymmetry, ocular hypertelorism and proptosis, downslanting palpebral fissures, divergent upgaze and esotropic downgaze, a tendency towards large ears, and marked depression of the nasal bridge. The nose is short and wide with a bulbous tip, and the anterior facial height is reduced. Common features during infancy included horizontal grooves above the supraorbital ridges that disappear

Unmasking metastatic lung cancer through orbital apex syndrome.

A female patient in her sixth decade of life presented with a 7-day history of progressive diminution of vision and restricted ocular motility associated with fever, headache and vomiting. On general examination, she was febrile (temperature 100.4°F) and had enlarged submandibular lymph nodes, along with multiple nodules on the scalp and upper back. At baseline, ocular examination revealed visual acuity of 20/30 in the right eye and hand movements close to the face in the left eye. A relative afferent pupillary defect was noted in the left eye. The Hirschberg test revealed left esotropia (15°°

One of the First Cases with PIK3CA-related Overgrowth Spectrum (PROS) in Saudi Arabia: A Case Report and Literature Review.

PIK3CA-related overgrowth spectrum (PROS) is an umbrella that includes a broad range of rare disorders, ranging from isolated digit enlargement to extensive overgrowth of the limbs, abdomen, or brain. One of these disorders is megalencephaly capillary malformation polymicrogyria syndrome (MCAP), which is characterized by cutaneous capillary malformations, megalencephaly, cortical brain malformations, abnormalities of somatic growth with body and brain asymmetry, developmental delay, and characteristic facial dysmorphism. The diagnosis of PROS syndrome is based on the clinical features of a pat

[Recruitment of Suitable Families to Identify Causative Genes in Hereditary Strabismus].

PURPOSE: The cause and origin of primary strabismus are not well understood. It is thought that there is multifactorial genetic inheritance. Only linkage analysis has been applied to study the genetic causes and correlations. The objective of this study was the compilation and statistical evaluation of a useful cohort of families for linkage analysis. METHODS: The archives of the Department of Ophthalmology of the Justus-Liebig-University Gießen were used as a database for this study. Medical records of visits between January 2001 and July 2008 were analysed. Evaluation was based on the medic

De novo 2p16.1 microdeletion with metastatic esophageal adenocarcinoma.

Microdeletions involving chromosome 2p15-16.1 are a rare genetic abnormality and have been reported in 18 separate patients, mainly children, since 2007. This microdeletion syndrome is characterised by a heterogeneous expression of intellectual impairment, dysmorphic facies, musculoskeletal abnormalities and potential neurodevelopmental anomalies. We report the first case of natural progression in an adult patient who died at a young age of metastatic esophageal adenocarcinoma. Important learning points include the variable phenotypic expression of this microdeletion syndrome and the fact that

Inadvertent intraocular injection of botulinum toxin A.

This report describes an inadvertent perforation of the globe while injecting botulinum toxin A into the medial rectus muscle without electromyography guidance. Botulinum toxin A does not appear to have toxic effects on the human retina. Close observation and necessary measures to treat any retinal breaks and control increased intraocular pressure should be undertaken.

Practical aspects and efficacy of intraoperative adjustment in concomitant horizontal strabismus surgery.

PURPOSE: To analyze the practical aspects and advantages of one-stage adjustable surgery under topical anesthesia in concomitant horizontal strabismus. METHODS: A retrospective review of 363 patients was completed to assess (1) the frequency of need to perform adjustment, (2) the amount and pattern of adjustment, and (3) the final alignment at least 6 months after surgery. Intraoperative adjustment was performed strictly toward the aim of orthophoria or slight overcorrection (heterophoria < 6 prism diopters [PD]) while avoiding diplopia. RESULTS: Of the 363 patients, 261 (72%) required intra

Divergence insufficiency revisited: natural history of idiopathic cases and neurologic associations.

OBJECTIVES: To determine the natural history of primary divergence insufficiency and to identify clinical features that distinguish patients with this neurologically isolated form of divergence insufficiency from those harboring neurologic disorders. METHODS: Retrospective survey of patients with divergence insufficiency. Patients were categorized into 2 groups, primary (ie, neurologically isolated based on clinical criteria) and secondary (ie, associated with a neurologic or systemic disorder). Long-term follow-up and clinical features of the 2 groups were compared. RESULTS: Of the 20 patie

Activity of near-response cells during disconjugate saccades in strabismic monkeys.

Infantile strabismus is a common disorder characterized by a chronic misalignment of the eyes, impairment of binocular vision, and oculomotor abnormalities. Nonhuman primates with strabismus, induced in infancy, show a pattern of abnormalities similar to those of strabismic children. This allows strabismic nonhuman primates to serve as an ideal animal model to examine neural mechanisms associated with aberrant oculomotor behavior. Here, we test the hypothesis that impairment of disparity vergence and horizontal saccade disconjugacy in exotropia and esotropia are associated with disrupted tunin
