Vascular cognitive disorder: a new diagnostic category updating vascular cognitive impairment and vascular dementia.

Vascular cognitive impairment (VCI) was proposed as an umbrella term to include subjects affected with any degree of cognitive impairment resulting from cerebrovascular disease (CVD), ranging from mild cognitive impairment (MCI) to vascular dementia. VCI may or may not exclude the host of "focal" circumscribed impairments of specialized functions such as language (aphasia), intentional gesture (apraxia), or categorical recognition (agnosia), among others, that may result from a stroke. Therefore, there are no universally accepted diagnostic criteria for VCI. We conclude that this concept could

Impact of expressive intentions on upper-body kinematics in two expert pianists.

INTRODUCTION: Expression is a key aspect of music performance. Studies on pianists' gestures and expression have mainly documented the impact of their expressive intentions on proximal segments and head linear kinematics. It remains unclear how pianists' expressive intentions influence joint angular kinematics as well as exposure to risk factors of injury, such as poor overall posture and distal jerky movements, two kinematic factors linked to injury. The first objective of this exploratory case study was to analyze the influence of pianists' expressive intentions on proximal and distal joint

Challenging Case: 2 Year Old With "Autistic Behaviors".

"George" is a 2-year, 9-month-old boy who presented for Developmental Behavioral Pediatric evaluation with the concern of global developmental delay and autistic behaviors. He had been receiving early intervention support since he turned 2 years, receiving a parent coaching model with a developmental specialist.During the clinical interview, parents endorsed limited expressive language skills and felt that his receptive language outpaced his expressive language skills. They reported consistent response to name when called and integration of eye contact and other nonverbal gestures (e.g., point

A Scoping Review on Conversational Memory and Characteristics of Conversations in Alzheimer's Disease.

PURPOSE: Typical late-onset Alzheimer's disease (AD) compromises episodic memory, the ability to encode new events and recollect past events. Much of the research on episodic memory in AD has relied on lab-based memory tasks (e.g., word lists, short stories). It is unclear how well these tasks characterize the impact of episodic memory impairments across different domains of everyday life, including conversational memory. The goal of the review was to establish what is known about conversational memory in AD, that is, memory for the content of conversations one overhears or in which one partic

An Assessment of Risk Factors of Delayed Speech and Language in Children: A Cross-Sectional Study.

Introduction Communication is the exchange of information through speaking, writing, and other mediums. Speech is the expression of thoughts in spoken words. Language is the principal method that humans use for relaying information; consisting of words conveyed by speech, writing, or gestures. Language is the conceptual processing of communication. Problems in communication or oral motor function are called speech and language disorders. Developmental delay is diagnosed when a child does not attain normal developmental milestones at the expected age. Speech and/or language disorders are amongs

Humour in clinical-educational interactions between graduate student clinicians and people with aphasia.

BACKGROUND: During clinical interactions, clinicians and people with aphasia (PWA) use humour and laughter for a range of purposes, most of which contribute to friendly interactions in which the participants appear to develop a positive regard for one another. Moreover, humour is a vital component of facework, or the processes interactants engage in to protect their own and one another's well-respected, public personas. AIMS: To examine the ways in which speech-language pathology graduate student clinicians enlist humour during one-on-one therapy sessions for PWA. METHODS & PROCEDURES: Three

Homeless in Dhaka: violence, sexual harassment, and drug-abuse.

Bangladesh has experienced one of the highest urban population growth rates (around 7% per year) over the past three decades. Dhaka, the capital city, attracts approximately 320,000 migrants from rural areas every year. The city is unable to provide shelter, food, education, healthcare, and employment for its rapidly-expanding population. An estimated 3.4 million people live in the overcrowded slums of Dhaka, and many more live in public spaces lacking the most basic shelter. While a small but growing body of research describes the lives of people who live in urban informal settlements or slum

Social communication skills profile in infants with sex chromosome trisomy at 12 months of age.

Sex Chromosome Trisomy (SCT) conditions are genetic disorders that affect approximately 1:500 children. Although there is considerable variability in phenotype, individuals with SCT have an elevated likelihood, compared to the general population, of developing social deficits and autism spectrum disorder (ASD). Studies report that 10-35 % of individuals with SCT meet criteria for ASD, depending on differences in ascertainment methods and diagnostic criteria. More specifically, ASD prevalence has been estimated at approximately 18 % (range 10-27 %) for XXY, 15 % (10.8-20 %) for XXX, and 30 % (1

Moving towards culturally competent health systems for migrants? Applying systems thinking in a qualitative study in Malaysia and Thailand.

BACKGROUND: Cultural competency describes interventions that aim to improve accessibility and effectiveness of health services for people from ethnic minority backgrounds. Interventions include interpreter services, migrant peer educators and health worker training to provide culturally competent care. Very few studies have focussed on cultural competency for migrant service use in Low- and Middle-Income Countries (LMIC). Migrants and refugees in Thailand and Malaysia report difficulties in accessing health systems and discrimination by service providers. In this paper we describe stakeholder

Developmental Coordination Disorder, An Umbrella Term for Motor Impairments in Children: Nature and Co-Morbid Disorders.

BACKGROUND: Developmental Coordination Disorder (DCD) defines a heterogeneous class of children exhibiting marked impairment in motor coordination as a general group of deficits in fine and gross motricity (subtype mixed group) common to all research studies, and with a variety of other motor disorders that have been little investigated. No consensus about symptoms and etiology has been established. METHODS: Data from 58 children aged 6 to 13 years with DCD were collected on DSM-IV criteria, similar to DSM-5 criteria. They had no other medical condition and inclusion criteria were strict (bor

Symptom-related damage patterns in mesial temporal lobe epilepsy with hippocampal sclerosis.

OBJECTIVE: This study was undertaken to elucidate the neuroanatomical and network substrates underlying diverse seizure symptoms in mesial temporal lobe epilepsy with hippocampal sclerosis (mTLE-HS) by applying multivariate support vector regression (SVR) to individualized structural symptom mapping and atrophy network mapping (ANM). METHODS: We retrospectively enrolled 216 patients with mTLE-HS and 460 healthy controls. Six non-mutually exclusive seizure symptoms were assessed. We performed left-right flipping of all images of right-sided mTLE-HS patients along the midline before processing

Chromosome 22q13 terminal deletion size is associated with relevant clinical features in a sample of 63 Italian patients with Phelan-McDermid syndrome.

BACKGROUND: Phelan-McDermid syndrome (PMS) is caused in the majority of cases by the loss or mutation of one allele of the SHANK3 gene, located in human chr 22q13.33. PMS displays large interindividual differences in clinical severity and longitudinal trajectory. Other genes located in this chromosomal region are known to contribute to the clinical phenotype (CELSR1, TCF20) in patients with larger deletions. The aim of this study is to identify clinically-relevant phenotypic features significantly influenced by the size of chromosome 22q terminal deletion and to identify new potential candidat

Medial temporal lobe epilepsy associated with hippocampal sclerosis is a distinctive syndrome.

Epileptic syndromes are distinctive disorders with specific features, which when taken together, permit a specific diagnosis. There is actually a debate on that medial temporal lobe epilepsy with hippocampal sclerosis is an epileptic syndrome. To address this issue, we searched for discriminative semiological features between temporal lobe epilepsy patients with hippocampal sclerosis (TLE-HS patients or group 1), TLE patients with medial structural lesion other than hippocampal sclerosis or in MRI-negative cases with medial onset on further investigations (group 2) and lateral TLE patients (LT

Dissociation of mirroring and mentalising systems in autism.

The role of mirror neuron systems and mentalising systems in causing poor social and communication skills in individuals with autistic spectrum conditions is hotly debated. We studied 18 adults with autistic spectrum conditions in comparison to 19 age and IQ matched typical individuals. Behavioural assessments revealed difficulties in mental state attribution and action comprehension in the autism sample. We examined brain responses when observing rational and irrational hand actions, because these actions engage mirror and mentalising components of the social brain respectively. Both typical

Augmentative and alternative communication (AAC) in pediatric cochlear implant recipients with complex needs: A scoping review.

INTRODUCTION: Augmentative and alternative communication (AAC) encompasses all forms of unaided and aided modes of communication, but typically excludes codified language such as spoken words or American Sign Language (ASL). In pediatric patients with a documented additional disability (population of interest), deficits in communication may pose a barrier to language development. While forms of AAC are frequently mentioned in the literature, recent innovations have permitted the use of high-tech AAC in the rehabilitation process. Our objective was to review the implementation of AAC in pediatr

Can explainable AI scaffolding reduce cognitive load and enhance embodied expression? A quasi-experimental study of multilingual vocal learning in Bel Canto education.

Exploring how artificial intelligence technology can effectively support music education has become a critical issue in the digital era. This study addresses the high cognitive load and embodied expression difficulties faced by bel canto students when learning multilingual vocal works, designing and validating the educational effectiveness of an explainable AI scaffolding learning system. The study employed a 2 × 3 mixed-factor quasi-experimental design, with 64 college students from the Conservatory of Music allocated to the XAI scaffolding group and the traditional AI-assisted group through

Cognitive decline heralds onset of symptomatic inherited prion disease.

The clinical effectiveness of any disease-modifying treatment for prion disease, as for other neurodegenerative disorders, will depend on early treatment before damage to neural tissue is irrevocable. Thus, there is a need to identify markers that predict disease onset in healthy at-risk individuals. Whilst imaging and neurophysiological biomarkers have shown limited use in this regard, we recently reported progressive neurophysiological changes in individuals with the inherited prion disease mutation P102L. We have also previously demonstrated a signature pattern of fronto-parietal dysfunctio

Structural correlates of impaired working memory in hippocampal sclerosis.

PURPOSE: Temporal lobe epilepsy (TLE) has been considered to impair long-term memory, whilst not affecting working memory, but recent evidence suggests that working memory is compromised. Functional MRI (fMRI) studies demonstrate that working memory involves a bilateral frontoparietal network the activation of which is disrupted in hippocampal sclerosis (HS). A specific role of the hippocampus to deactivate during working memory has been proposed with this mechanism faulty in patients with HS. Structural correlates of disrupted working memory in HS have not been explored. METHODS: We studied
