Functional activity within brain tumors: a magnetic source imaging study.

OBJECTIVE: To determine whether low-grade gliomas contain functional cortical activity more often than high-grade gliomas within radiologically defined abnormal tissue. METHODS: Patients with intra-axial cerebral lesions located in the vicinity of eloquent brain cortex preoperatively underwent magnetic source imaging. A dual 37-channel biomagnetometer was used to perform the imaging. Evoked magnetic fields were analyzed using the single-equivalent dipole representation to ascertain the neuronal source. Stimuli included painless tactile somatosensory stimulation of fingers, toes, and lips and

Three-year reliability of MEG resting-state oscillatory power.

INTRODUCTION: Resting-state oscillatory activity has been extensively studied across a wide array of disorders. Establishing which spectrally- and spatially-specific oscillatory components exhibit test-retest reliability is essential to move the field forward. While studies have shown short-term reliability of MEG resting-state activity, no studies have examined test-retest reliability across an extended period of time to establish the stability of these signals, which is critical for reproducibility. METHODS: We examined 18 healthy adults age 23 - 61 who completed three visits across three y

Causal Inference in the Multisensory Brain.

When combining information across different senses, humans need to flexibly select cues of a common origin while avoiding distraction from irrelevant inputs. The brain could solve this challenge using a hierarchical principle by deriving rapidly a fused sensory estimate for computational expediency and, later and if required, filtering out irrelevant signals based on the inferred sensory cause(s). Analyzing time- and source-resolved human magnetoencephalographic data, we unveil a systematic spatiotemporal cascade of the relevant computations, starting with early segregated unisensory represent

Reduced occipital alpha power indexes enhanced excitability rather than improved visual perception.

Several studies have demonstrated that prestimulus occipital alpha-band activity substantially influences subjective perception and discrimination of near-threshold or masked visual stimuli. Here, we studied the role of prestimulus power fluctuations in two visual phenomena called double-flash illusion (DFI) and fusion effect (FE), both consisting of suprathreshold stimuli. In both phenomena, human subjects' perception varies on a trial-by-trial basis between perceiving one or two visual stimuli, despite constant stimulation. In the FE, two stimuli correspond to veridical perception. In the DF

Structural, functional and molecular imaging of the brain in primary focal dystonia--a review.

Primary focal dystonias form a group of neurological disorders characterized by involuntary, sustained muscle contractions causing twisting movements and abnormal postures. The estimated incidence is 12-25 per 100,000. The pathophysiology is largely unclear but genetic and environmental influences are suspected. Over the last decade neuroimaging techniques have been applied in patients with focal dystonia. Using structural, functional and molecular imaging techniques, abnormalities have been detected mainly in the sensorimotor cortex, basal ganglia and cerebellum. The shared anatomical localis

Fixation-sensitive myoclonus in Lafora disease.

The authors report a patient with Lafora disease, whose myoclonus was suppressed by passive eye closure. Neurophysiologic studies disclosed that fixation was the most important enhancer of myoclonus. Magnetoencephalographic studies of visual evoked fields revealed abnormal activation of the visual corticocortical pathway via the insular cortex not seen in controls. The authors hypothesize that abnormal activation of the insular cortex may be involved in triggering the mechanism of fixation-sensitive myoclonus.

Evidence of biochemical and biomagnetic interactions in Alzheimer's disease: an MEG and MR spectroscopy study.

OBJECTIVES: To quantify how adherence mismeasurement affects the estimated impact of adherence on inpatient costs among patients with serious mental illness (SMI). STUDY DESIGN: Proportion of days covered (PDC) is a common claims-based measure of medication adherence. Because PDC does not measure medication ingestion, however, it may inaccurately measure adherence. We derived a formula to correct the bias that occurs in adherence-utilization studies resulting from errors in claims-based measures of adherence. METHODS: We conducted a literature review to identify the correlation between gold-

Face recognition and cortical responses show similar sensitivity to noise spatial frequency.

To find cortical correlates of face recognition, we manipulated the recognizability of face images in a parametric manner by masking them with narrow-band spatial noise. Face recognition performance was best at the lowest and highest noise spatial frequencies (NSFs, 2 and 45 c/image, respectively), and degraded gradually towards central NSFs (11-16 c/image). The strength of the 130-180 ms neuromagnetic response (M170) in the temporo-occipital cortex paralleled the recognition performance, whereas the mid-occipital response at 70-120 ms acted in the opposite manner, being strongest for the cent

The silent period induced by transcranial magnetic stimulation in muscles supplied by cranial nerves: normal data and changes in patients.

The silent period induced by transcranial magnetic stimulation of the sensorimotor cortex (Magstim 200, figure of eight coil, loop diameter 7 cm) in active muscles supplied by cranial nerves (mentalis, sternocleidomastoid, and genioglossus) was studied in 14 control subjects and nine patients with localised lesions of the sensorimotor cortex. In the patients, measurements of the silent period were also made in the first dorsal interosseus and tibialis anterior muscles. In the controls, there was a silent period in contralateral as well as ipsilateral cranial muscle and the duration of the sile

Feasibility, Safety, and Preliminary Efficacy of Median Nerve Stimulation for Cognitive Dysfunction in Patients With Acute Traumatic Brain Injury: Study Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Currently, the treatment of cognitive dysfunction after acute traumatic brain injury (TBI) remains a challenge, and novel therapeutic methods are urgently needed. Median nerve stimulation (MNS) is a noninvasive neuromodulation technique that has recently shown positive effects in promoting recovery from coma after acute brain injury. It has been shown to improve cognition in healthy volunteers, and may be a potential therapeutic approach for cognitive dysfunction in patients with acute TBI. OBJECTIVE: The primary aim of this study is to evaluate the feasibility, safety, and prelim

Early visual alterations in individuals at-risk of Alzheimer's disease: a multidisciplinary approach.

BACKGROUND: The earliest pathological features of Alzheimer's disease (AD) appear decades before the clinical symptoms. The pathology affects the brain and the eye, leading to retinal structural changes and functional visual alterations. Healthy individuals at high risk of developing AD present alterations in these ophthalmological measures, as well as in resting-state electrophysiological activity. However, it is unknown whether the ophthalmological alterations are related to the visual-related electrophysiological activity. Elucidating this relationship is paramount to understand the mechani

Specific Oscillatory Power Changes and Their Efficacy for Determining Laterality in Mesial Temporal Lobe Epilepsy: A Magnetoencephalographic Study.

Appropriate determination of the epileptic focus and its laterality are important for the diagnosis of mesial temporal lobe epilepsy (MTLE). The aims of this study are to establish a specific oscillatory distribution and laterality of the oscillatory power in unilateral MTLE with frequency analysis of magnetoencephalography (MEG), and to confirm their potential to carry significant information for determining lateralization of the epileptic focus. Thirty-five patients with MTLE [left (LtMTLE), 16; right (RtMTLE), 19] and 102 healthy control volunteers (CTR) were enrolled. Cortical oscillatory

Magnetoencephalographic source imaging improves localization of the epileptogenic zone in multimodal imaging evaluation.

OBJECTIVE: To assess the added diagnostic value of magnetoencephalographic source imaging (MSI) beyond conventional magnetic resonance imaging (MRI) and fluorodeoxyglucose-positron emission tomography (FDG-PET) in localizing the epileptogenic zone, with stereo-electroencephalography (SEEG) and surgical resection serving as reference standards. METHODS: Among 101 patients prospectively enrolled with drug-resistant focal epilepsy in the EPIMAGE study to assess the diagnostic yield of non-invasive multimodal imaging (MSI, MRI, FDG-PET), those undergoing SEEG and/or surgery were analyzed to compa

Global resting-state functional connectivity of neural oscillations in tinnitus with and without hearing loss.

This study examined global resting-state functional connectivity of neural oscillations in individuals with chronic tinnitus and normal and impaired hearing. We tested the hypothesis that distinct neural oscillatory networks are engaged in tinnitus with and without hearing loss. In both tinnitus groups, with and without hearing loss, we identified multiple frequency band-dependent regions of increased and decreased global functional connectivity. We also found that the auditory domain of tinnitus severity, assayed by the Tinnitus Functional Index, was associated with global functional connecti

tACS-mediated modulation of the auditory steady-state response as seen with MEG.

BACKGROUND: Previous studies have shown that transcranial electrical stimulation can be successfully applied during simultaneous MEG measurements. In particular, using beamforming they have established that changes of stimulus induced as well as evoked activity can be inspected during transcranial alternating current stimulation (tACS). OBJECTIVE/HYPOTHESIS: We studied tACS-mediated changes of the auditory steady-state response (ASSR), hypothesizing that-due to the putatively inhibitory role of alpha oscillations-these evoked responses would be diminished. METHODS: We compared ASSRs in condi

Plasticity of premotor cortico-muscular coherence in severely impaired stroke patients with hand paralysis.

Motor recovery in severely impaired stroke patients is often very limited. To refine therapeutic interventions for regaining motor control in this patient group, the functionally relevant mechanisms of neuronal plasticity need to be detected. Cortico-muscular coherence (CMC) may provide physiological and topographic insights to achieve this goal. Synchronizing limb movements to motor-related brain activation is hypothesized to reestablish cortico-motor control indexed by CMC. In the present study, right-handed, chronic stroke patients with right-hemispheric lesions and left hand paralysis part

Disinhibition of the primary somatosensory cortex in patients with fibromyalgia.

Fibromyalgia (FM) is a chronic widespread pain condition linked to central sensitization. Altered excitability of sensorimotor cortex has been proposed as an underlying pathology of FM. This study aimed to investigate intracortical excitability of the primary somatosensory cortex (S1) and its potential role in clinical pain in patients with FM. Somatosensory evoked magnetic fields were recorded in 17 right-handed females with FM and 21 age-, sex-, and handedness-matched healthy control subjects. Paired-pulse median nerve stimulation was delivered to the left and right wrist. We assessed the pe

Unique discrepancy between cerebral blood flow and glucose metabolism in hemimegalencephaly.

Hemimegalencephaly (HME) presents as severe refractory seizures and requires early surgical treatment to prevent progression to catastrophic epilepsy. Single-photon emission computed tomography (SPECT) and positron emission tomography (PET) are useful imaging techniques for the presurgical evaluation of patients with intractable epilepsy. However, the results in HME are variable and no study has compared SPECT and PET performed at around the same time. We performed SPECT and PET for nine patients with HME, which was defined as a whole or part of affected hemisphere enlargement (three males, si

Neurophysiology of synesthesia.

Synesthesia is an experience in which stimulation in one sensory or cognitive stream leads to associated experiences in a second, unstimulated stream. Although synesthesia is often referred to as a "neurological condition," it is not listed in the DSM IV or the ICD classifications, as it generally does not interfere with normal daily functioning. However, its high prevalence rate (one in 23) means that synesthesia may be reported by patients who present with other psychiatric symptoms. In this review, I focus on recent research examining the neural basis of the two most intensively studied for

Magnetosensory evoked potentials: consistent nonlinear phenomena.

Electromagnetic fields (EMFs) having strengths typically found in the general environment can alter brain activity, but the reported effects have been inconsistent. We theorized that the problem arose from the use of linear methods for analyzing what were actually nonlinear phenomena, and therefore studied whether the nonlinear signal-processing technique known as recurrence quantification analysis (RQA) could be employed as the basis of a reliable method for demonstrating consistent changes in brain activity. Our primary purpose was to develop such a method for observing the occurrence of evo
