Smoldering in the sanctuary: HIV-associated brain injury in the ART era.

Although combination antiretroviral therapy (ART) has dramatically reduced the incidence of severe HIV-associated neurological disease, the central nervous system (CNS) remains a viral sanctuary in which inflammation and brain injury persist despite systemic viral suppression. Here, we synthesize evidence that ongoing HIV-associated brain injury is sustained not primarily by unchecked viral replication but by persistent viral transcription from defective proviruses, immune-mediated synaptic dysfunction driven by bystander activation, and long-lived microglial reprogramming shaped by epigenetic

Measuring neurocognitive impairment in virally suppressed HIV: HIV-associated neurocognitive disorders, HIV-associated brain injury problems and solutions.

PURPOSE OF REVIEW: To discuss neurocognition measurement in view of current controversies in HIV-associated neurocognitive disorders (HAND). RECENT FINDINGS: The HAND criteria have been criticised for several reasons with some proposing alternative criteria and terminology, HIV-associated brain injury (HABI). The following concerns have been raised: the legacy effect from past ineffective therapy is not included, the adoption of an asymptomatic neurocognitive impairment (ANI) stage, including liberal neuropsychological test cut-offs overestimate neurocognitive impairment (NCI), and that NCI w

HIV-associated neurocognitive disorders (HAND): Optimal diagnosis, antiviral therapy, pharmacological treatment, management, and future scopes.

In the context of HIV infection, HIV-associated neurocognitive disorders (HAND) have become a serious concern. An extensive summary of the diagnosis, care, and mental health consequences related to HAND is given in this article. The diagnosis of HAND entails a multimodal approach that includes neuroimaging, cognition tests, and clinical examinations. Numerous screening instruments and standardized evaluations have been created to support the early identification and tracking of HAND. Appropriate actions and individualized treatment plans are made possible by prompt diagnosis. A multidisciplina

The clinical utility of three frailty measures in identifying HIV-associated neurocognitive disorders.

OBJECTIVE: Frailty measures vary widely and the optimal measure for predicting HIV-associated neurocognitive disorders (HAND) is unclear. DESIGN: A study was conducted to examine the clinical utility of three widely used frailty measures in identifying HIV-associated neurocognitive disorders. METHODS: The study involved 284 people with HIV (PWH) at least 50 years enrolled at UC San Diego's HIV Neurobehavioral Research Program. Frailty measurements included the Fried Phenotype, the Rockwood Frailty Index, and the Veterans Aging Cohort Study (VACS) Index. HAND was diagnosed according to Frasca

Is There Any Evidence of Premature, Accentuated and Accelerated Aging Effects on Neurocognition in People Living with HIV? A Systematic Review.

Despite evidence of premature, accentuated and accelerated aging for some age-related conditions such as cardiovascular diseases in people living with HIV (PLHIV), the evidence for these abnormal patterns of aging on neurocognition remains unclear. Further, no systematic review has been dedicated to this issue. Using PRISMA guidelines, we searched standard databases (PubMed, EMBASE, CINAHL and PsycINFO). Articles were included if they analyzed and reported the effect of age on neurocognition among PLHIV as one of their major findings, if they were conducted in the combination anti-retroviral t

Role of Macrophage Dopamine Receptors in Mediating Cytokine Production: Implications for Neuroinflammation in the Context of HIV-Associated Neurocognitive Disorders.

Despite the success of combination anti-retroviral therapy (cART), around 50% of HIV-infected individuals still display a variety of neuropathological and neurocognitive sequelae known as NeuroHIV. Current research suggests these effects are mediated by long-term changes in CNS function in response to chronic infection and inflammation, and not solely due to active viral replication. In the post-cART era, drug abuse is a major risk-factor for the development of NeuroHIV, and increases extracellular dopamine in the CNS. Our lab has previously shown that dopamine can increase HIV infection of pr

Neurologic Complications in Persons With HIV Infection in the Era of Antiretroviral Therapy.

Neurologic complications in persons with HIV infection are less severe in the era of potent antiretroviral therapy but remain highly prevalent. Prior to the use of antiretroviral therapy, opportunistic infections of the central nervous system (CNS) and CNS malignancy were common. Progressive multifocal leukoencephalopathy (PML), however, remains a diagnostic challenge in HIV-infected individuals, and no effective antiviral treatment for PML is currently available. Primary neurologic complications of acute HIV infection include aseptic meningitis and acute inflammatory demyelinating polyneuropa

Mechanisms of HIV Neuropathogenesis: Role of Cellular Communication Systems.

BACKGROUND: One of the major complications of Human Immunodeficiency Virus (HIV) infection is the development of HIV-Associated Neurocognitive Disorders (HANDs) in approximately 50-60% of HIV infected individuals. Despite undetectable viral loads in the periphery owing to anti-retroviral therapy, neuroinflammation and neurocognitive impairment are still prevalent in HIV infected individuals. Several studies indicate that the central nervous system (CNS) abnormalities observed in HIV infected individuals are not a direct effect of viral replication in the CNS, rather these neurological abnormal

A Screening Strategy for HIV-Associated Neurocognitive Disorders That Accurately Identifies Patients Requiring Neurological Review.

BACKGROUND: Human immunodeficiency virus (HIV)-associated neurocognitive disorders (HAND) are not routinely assessed due to the lack of an adequate screening strategy. We aimed to develop a clinically relevant screening procedure for symptomatic HAND, validated against a gold standard neuropsychological (NP) test battery. METHODS: Representative HIV-infected (HIV+) and demographically matched HIV-uninfected (HIV-) participants in an observational study completed a standard evaluation for mood, drug and/or alcohol use, and activities of daily living and a newly designed 20-minute computerized

Role of neurotrophic factor alterations in the neurodegenerative process in HIV associated neurocognitive disorders.

Migration of HIV infected cells into the CNS is associated with a spectrum of neurological disorders, ranging from milder forms of HIV-associated neurocognitive disorders (HAND) to HIV-associated dementia (HAD). These neuro-psychiatric syndromes are related to the neurodegenerative pathology triggered by the release of HIV proteins and cytokine/chemokines from monocytes/macrophages into the CNS -a condition known as HIV encephalitis (HIVE). As a result of more effective combined anti-retroviral therapy patients with HIV are living longer and thus the frequency of HAND has increased considerabl

Central nervous system antiretroviral efficacy in HIV infection: a qualitative and quantitative review and implications for future research.

BACKGROUND: There is conflicting information as to whether antiretroviral drugs with better central nervous system (CNS) penetration (neuroHAART) assist in improving neurocognitive function and suppressing cerebrospinal fluid (CSF) HIV RNA. The current review aims to better synthesise existing literature by using an innovative two-phase review approach (qualitative and quantitative) to overcome methodological differences between studies. METHODS: Sixteen studies, all observational, were identified using a standard citation search. They fulfilled the following inclusion criteria: conducted in

Olfactory and psychomotor symptoms in HIV and aging: potential precursors to cognitive loss.

In the United States, more than 100,000 adults 50 years old and over are diagnosed with the Human Immunodeficiency Virus (HIV). The number of older adults living with HIV is increasing dramatically due primarily to Highly Active Antiretroviral Therapy (HAART) which is transforming this disease into a chronic condition for many who are responding well to treatment. This population is also growing due to later-life infections and reflects the overall aging of the larger population in general. Yet, despite the novelty of such developing demographic trends, the negative consequences of aging with

Astrocyte specific viral strains in HIV dementia.

We molecularly characterized human immunodeficiency virus type 1 (HIV-1) present in pure populations of astrocytes, macrophages, and multinucleated giant cells isolated using laser capture microdissection from brain tissue of two patients who died with HIV-associated dementia. The V3 region of the HIV-1 envelope (env) gene was amplified from the pure-cell populations, and multiple clones were sequenced. In both patients, the V3 env sequences were distinct in astrocytes compared with neighboring macrophages or multinucleated giant cells and were characteristic of CCR5-using (R5) HIV-1. These re

Novel markers of oxidative stress in actively progressive HIV dementia.

Oxidative stress leads to the production of reactive oxygen species that can attack lipid membranes resulting in cellular dysfunction and death. Cellular redox state is closely linked to ceramide, sphingomyelin, and 4-hydroxynonenal (HNE) levels. We describe data showing increased levels of these oxidative stress markers in HIV encephalitis. In addition, actively progressing HIV dementia is associated with increases in HNE and ceramide, while inactive HIV dementia is associated with increases in sphingomyelin. These markers may be useful for distinguishing between different clinical phenotypes

AIDS and suicide.

An association between suicide and the acquired immune deficiency syndrome (AIDS) has been noted in several epidemiologic studies. Numerous case reports also support the trend of an increased suicide risk in persons infected with human immunodeficiency virus (HIV) and persons with AIDS. A variety of medical, neurologic, and psychiatric factors may contribute to the death of an HIV patient. I present the case of a 27-year-old man who committed suicide approximately 2 years after he received a diagnosis of AIDS. He had no previous psychiatric history. It is imperative that physicians caring for
