AIMS: In wearable-based automated cardiac arrest detection technology, photoplethysmography (PPG) is the most commonly used sensor to detect the absence of pulsations. To minimize false-positive cardiac arrest alerts, accelerometry signals are often used for the detection of ongoing movement. We conducted the DETECT-2 study to develop an accelerometer-based machine learning model for the detection of cardiac arrest-related collapse, which is often a first manifestation of cardiac arrest.
METHODS AND RESULTS: Healthy volunteers simulated cardiac arrest-related collapses through sudden and soft

Background Nephrotic syndrome (NS) typically involves proteinuria, reduced levels of albumin in the blood, and swelling. It may also lead to complications, including an increased risk of thrombosis and a decline in kidney function. While pharmacological treatments are primary, physical activity (PA) may play a role in improving outcomes. However, limited data exist on PA levels in patients with NS, particularly regarding the validity of self-reported PA assessments. The COVID-19 pandemic also introduced significant lifestyle changes, potentially impacting PA and kidney function. This study aim

Physical activity levels among childhood cancer survivors are typically quantified as a total amount using time spent in various intensities. Yet, most analyses do not consider the transitory nature of children's behaviors and a more detailed approach could provide complimentary information. We aimed to explore various behavior profiles of survivors' daily and hourly physical activity patterns. We measured 8-18-year-old survivors' activity levels over 7 days using wrist accelerometry and cluster analysis. Of the 37 participant datasets, survivors engaged in mean (SD) 36.3 (19.0) min/day of MVP

BACKGROUND: People with type 1 diabetes (T1D) have varying sensitivities to insulin and also varying responses to meals and exercise. We introduce a new adaptive run-to-run model predictive control (MPC) algorithm that can be used to help people with T1D better manage their glucose levels using an artificial pancreas (AP). The algorithm adapts to individuals' different insulin sensitivities, glycemic response to meals, and adjustment during exercise as a continuous input during free-living conditions.
METHODS: A new insulin sensitivity adaptation (ISA) algorithm is presented that updates each

BACKGROUND/OBJECTIVES: Whether variation in sleep and physical activity explain marked ethnic and socioeconomic disparities in childhood obesity is unclear. As time spent in one behaviour influences time spent in other behaviours across the 24-hour day, compositional analyses are essential. The aims of this study were to determine how ethnicity and socioeconomic status influence compositional time use in children, and whether differences in compositional time use explain variation in body mass index (BMI) z-score and obesity prevalence across ethnic groups.
METHODS: In all, 690 children (58%

BACKGROUND: Post-stroke hemiparesis commonly reduces mobility, limiting daily activities and participation. Objective monitoring, such as step counting, quantifies real-world ambulatory activity, while patient-reported outcome measures (PROMs) capture perceived functional performance. The Lower Extremity Motor Activity Log (LE-MAL), translated into Portuguese, assesses self-reported use of the more affected lower limb.
OBJECTIVE: To examine the criterion-related validity of the LE-MAL by evaluating its association with step counts obtained from triaxial accelerometry activity monitors.

BACKGROUND: Walking is important for maintaining physical and mental well-being in aged residential care (ARC). Walking behaviors are not well characterized in ARC due to inconsistencies in assessment methods and metrics as well as limited research regarding the impact of care environment, cognition, or physical function on these behaviors. It is recommended that walking behaviors in ARC are assessed using validated digital methods that can capture low volumes of walking activity.
OBJECTIVE: This study aims to characterize and compare accelerometry-derived walking behaviors in ARC residents a

We examined the comparability of children's nocturnal sleep estimates using accelerometry data, processed with and without a sleep log. In a secondary analysis, we evaluated factors associated with disagreement between processing approaches. Children (n = 722, age 5-12 years) wore a wrist-based accelerometer for 14 days during Autumn 2020, Spring 2021, and/or Summer 2021. Outcomes included sleep period, duration, wake after sleep onset (WASO), and timing (onset, midpoint, waketime). Parents completed surveys including children's nightly bed/wake time. Data were processed with parent-reported b

AIM: Physical activity is associated with prognosis in older adults with chronic kidney disease (CKD). However, previous studies have relied mainly on self-report questionnaires, and objective evidence remains limited, particularly among patients undergoing peritoneal dialysis (PD). We assessed physical activity using a triaxial accelerometer across CKD stages and PD and examined its association with exercise self-efficacy.
METHODS: This cross-sectional study included 114 outpatients recruited between June 2023 and June 2024. Physical activity was measured with a triaxial accelerometer over f

PURPOSE: This study aims to test whether week-long wrist accelerometry combined with deep learning can (i) distinguish healthy individuals from people with knee osteoarthritis (KOA), (ii) separate prodromal KOA from established KOA, and (iii) identify individuals who will receive a KOA diagnosis within 5 years.
METHODS: We conducted a retrospective case-control study using the UK Biobank data set. After quality control, 102,120 participants with valid accelerometry were available; KOA was identified via ICD-10 M17.x codes (n = 7262). To reduce adiposity confounding, analyses were restricted t

Living alone is associated with adverse outcomes after acute coronary syndromes (ACS). One potential mediator of the relation between partner status and outcomes after ACS is physical activity. To evaluate the association of partner status with physical activity after ACS, data from 107 participants enrolled in the Prescription Use, Lifestyle, and Stress Evaluation (PULSE) study, a prospective observational study of post-ACS patients, were analyzed. Accelerometers were used to measure physical activity after hospital discharge. The primary outcome measure was a maximum 10 hours of daytime acti

BACKGROUND: Data are sparse regarding the value of physical activity (PA) surveillance among older adults-particularly among those with mobility limitations. The objective of this study was to examine longitudinal associations between objectively measured daily PA and the incidence of cardiovascular events among older adults in the LIFE (Lifestyle Interventions and Independence for Elders) study.
METHODS AND RESULTS: Cardiovascular events were adjudicated based on medical records review, and cardiovascular risk factors were controlled for in the analysis. Home-based activity data were collect

BACKGROUND: The walking efficiency of individuals who have experienced a stroke and have mild hemiparesis may be reduced even in the absence of visually apparent gait abnormalities. We investigated the association between several gait parameters assessed by a triaxial accelerometer and walking efficiency in this population.
METHODS: Patients with first-ever stroke and mild hemiparesis who could ambulate independently without assistive devices (n = 36) were included. Gait assessments were conducted when the patients were able to walk continuously for ≥14 m and for 3 min. We used the root mean

BACKGROUND: Skeletal muscle energetics decline with age, and physical activity (PA) has been shown to offset these declines in older adults. Yet, many studies reporting these effects were based on self-reported PA or structured exercise interventions. Therefore, we examined the associations of accelerometry-measured and self-reported PA and sedentary behavior (SB) with skeletal muscle energetics and explored the extent to which PA and sedentary behavior would attenuate the associations of age with muscle energetics.
METHODS: As part of the Study of Muscle, Mobility and Aging, enrolled older a

Physical activity (PA) during childhood and adolescence is important for the accrual of maximal peak bone mass. The precise dose that benefits bone remains unclear as methods commonly used to analyze PA data are unsuitable for measuring bone-relevant PA. Using improved accelerometry methods, this study identified the amount and intensity of PA most strongly associated with bone outcomes in 11-12-year-olds. Participants (n = 770; 382 boys) underwent tibial peripheral quantitative computed tomography to assess trabecular and cortical density, endosteal and periosteal circumference and polar stre

BACKGROUND: Energy requirement (ER) in young adults is commonly estimated from predicted resting metabolic rate (RMR) and assumed physical activity level (PAL). These assumptions may not fully reflect free-living total energy expenditure (TEE) in healthy young adults from Southwest China with low-activity daily routines.
OBJECTIVE: This study compared doubly labeled water (DLW)-measured TEE with factorial TEE based on measured RMR and with equation-estimated TEE, interpreted as estimated ER, derived from Chinese and FAO/WHO/UNU RMR prediction equations. Measured and predicted RMR, TEE partiti

Accelerometry enables passive, continuous, high-frequency monitoring under free-living conditions. For individuals with isolated REM sleep behavior disorder (iRBD), a potential prodromal phase of Parkinson's disease (PD) and dementia with Lewy bodies, accelerometry has been primarily applied to aid diagnosis and to assess phenoconversion risk. To extend this knowledge, we cross-sectionally combined clinical assessments focusing on non-motor symptoms with accelerometry-derived features of physical activity (PA), sleep, and circadian rhythm of N = 68 individuals with iRBD (age: 69.48 ± 6.01 year
