BACKGROUND: Little is known about the effects of blood rheology on the occurrence of acute chest syndrome and painful vaso-occlusive crises in children with sickle cell anemia and hemoglobin SC disease.
DESIGN AND METHODS: To address this issue, steady-state hemorheological profiles (blood viscosity, red blood cell deformability, aggregation properties) and hematologic parameters were assessed in 44 children with sickle cell anemia and 49 children with hemoglobin SC disease (8-16 years old) followed since birth. Clinical charts were retrospectively reviewed to determine prior acute chest synd

INTRODUCTION: Decreased exhaled nitric oxide levels (FE(NO)) have been described in patients with sickle cell disease (SCD) and a history of acute chest syndrome (ACS) when compared with non-ACS controls. Oral arginine supplementation has been shown to increase FE(NO) in healthy participants, but its effect in SCD patients is not known.
OBJECTIVE: To determine the effect of oral arginine intake on FENO in sickle cell patients with and without history of ACS, and in healthy controls.
HYPOTHESIS: No differences in the FE(NO) increase were seen in SCD patients with a history of ACS (ACS+) compa

The Acute Chest Syndrome is a frequent complication of sickle cell disease characterised by chest pain, fever and new infiltrate on chest X ray image. Early diagnosis and treatment are needed for Acute Chest Syndrome, which the aetiology remains unknown on more than half of cases. We report 3 cases of Acute Chest Syndrome with different aetiology due to a fat embolism in the first case, a pleuro-pneumonia infection in the second case and to tuberculosis in the third case. This report illustrates the importance of the bronchoscopy and the brochoalveolar lavage in the determination of the aetiol

BACKGROUND: To decrease the morbidity burden of cardiovascular disease and to avoid the development of potentially preventable complications, early assessment and treatment of acute coronary syndrome (ACS) are important. The aim of this study has therefore been to explore the possible association between patients' estimated intensity of chest pain when first seen by the ambulance crew in suspected ACS, and the subsequent outcome before and after arrival in hospital.
METHODS: Data was collected both prospectively and retrospectively. The inclusion criteria were chest pain raising suspicion of

Acute chest syndrome (ACS) is one of the leading causes of morbidity and mortality in children with sickle cell disease (SCD) and is commonly precipitated by respiratory infections. Among viral triggers, influenza A is a recognized cause of severe ACS and may contribute to rapid clinical deterioration. We report a case of a nine-year-old girl with homozygous sickle cell disease (HbSS), with irregular follow-up and poor adherence to hydroxyurea therapy, who presented with severe influenza A-associated ACS. Her course was complicated by plastic bronchitis, posterior reversible encephalopathy syn

BACKGROUND: The presence of bilateral lung infiltrates on chest imaging is a mandatory clinical criterion for the diagnosis of acute respiratory distress syndrome (ARDS). Although lung infiltrates are usually identified on chest radiograph in clinical practice, chest computed tomography (CT) outperforms chest radiograph for the diagnosis of lung infiltrates. We hypothesized that the performance of chest radiograph for the diagnosis of ARDS, as compared to chest CT as reference, is limited.
METHODS: We conducted a retrospective, single-center, observational study over a 7-year period, includin

Background and Objectives: Acute chest syndrome (ACS) is the leading cause of sickle cell disease (SCD)-related mortality (~25% of deaths). A subset of patients develops severe respiratory compromise requiring intensive care unit (ICU) admission or mechanical ventilation (MV), a high-risk group with widely variable reported mortality. This review aimed to determine the mortality rates, ICU/hospital length of stay, and reported complications among adult SCD patients with ACS admitted to the ICU or placed on MV. Materials and Methods: This PRISMA 2020-compliant systematic review was prospectivel

Acute chest syndrome (ACS) is a leading cause of morbimortality in sickle cell disease (SCD). In this prospective observational study, we investigated sputum interleukin-6 (IL-6) level as an ACS severity marker during 30 ACS episodes in 26 SCD children. Sputum IL-6 levels measured within the first 72 h of hospitalisation for ACS were significantly higher in patients with oxygen requirement ≥2 L/min, ventilation (invasive and/or non-invasive) length ≥5 days, bilateral and/or extensive opacities on chest X-ray or erythrocytapheresis requirement. Sputum IL-6 could serve as an ACS severity marker

Data on acute chest syndrome (ACS) in adult sickle cell disease patients are scarce. In this study, we describe 105 consecutive ACS episodes in 81 adult patients during a 32-month period and compare the characteristics as a function of the time to onset after hospital admission for a vaso-occlusive crisis (VOC), that is early-onset episodes (time to onset ≤24 h, 42%) versus secondary episodes (>24 h, 58%; median [interquartile range] time to onset: 2 [2-3] days). The median age was 27 [22-34] years, 89% of the patients had an S/S or S/β0 -thalassaemia genotype; 81% of the patients had a histor

STUDY OBJECTIVE: Acute chest syndrome is a leading cause of mortality in patients with sickle cell disease (SCD). Because early detection of acute chest syndrome is directly tied to prognosis, young patients with SCD undergo countless chest radiography screenings throughout their lifetime for commonly occurring acute chest syndrome risk factors such as fever, chest pain, or cough. Chest radiography is not an ideal screening method because it is associated with radiation exposure, which accumulates with repeated imaging. Point-of-care lung ultrasonography is a nonradiating imaging modality that

IMPORTANCE: Acute chest syndrome (ACS) is a common, serious complication of sickle cell disease (SCD) and a leading cause of hospitalization and death in both children and adults with SCD. Little is known about the effectiveness of guideline-recommended antibiotic regimens for the care of children hospitalized with ACS.
OBJECTIVES: To use a large, national database to describe patterns of antibiotic use for children with SCD hospitalized for ACS and to determine whether receipt of guideline-adherent antibiotics was associated with lower readmission rates.

UNLABELLED: The investigators of this study sought to examine whether abnormal physiological parameters are associated with increased risk for acute coronary syndrome (ACS) in patients presenting to the emergency department (ED) with chest pain.
METHODS: We used prospectively collected data on adult patients presenting with suspected ACS in 2 EDs in Australia and New Zealand. Trained research nurses collected physiological data including temperature, respiratory rate, heart rate, and systolic blood pressure (SBP) on presentation to the ED. The primary endpoint was ACS within 30 days of presen

Managing patients with chest pain in the Emergency Department (ED) remains a challenge where advanced imaging technology may play a role. Detecting acute coronary syndrome in the emergency department with cardiac magnetic resonance imaging (MRI) was studied in 161 consecutive chest pain patients in a prospective observational study. Patients with ST elevation myocardial infarction were excluded from study participation. All patients underwent MRI study when hemodynamically stable, free of ongoing chest pain, and within 12 hours of symptoms onset. The MRI study takes an average of 38 +/- 12 min

Sickle-cell trait has been characterized as a benign condition. However, life-threatening complications sometimes develop. Acute chest syndrome (ACS) is usually described in homozygous sickle cell disease, but it rarely develops in individuals with sickle-cell trait. Two cases of ACS in patients with sickle-cell trait are reported here. Factors such as advanced age at the time of presentation and absent past medical and family history can be misleading. Although ACS in sickle-cell trait has thus far only been reported in persons of Afro-American origin, persons of Mediterranean origin can, on

Extracorporeal membrane oxygenation (ECMO) is widely used in the treatment of respiratory and cardiovascular failure in neonatal patients. The authors present a case of a child with hemoglobin SS disease who was treated with ECMO after acute chest syndrome and acute respiratory distress syndrome developed. They also present data from the Extracorporeal Life Support Organization on this use of ECMO from other centers. To date, there have been 15 pediatric patients with acute chest syndrome treated with ECMO. Survival rate has been 26%. In selected patients with severe disease, ECMO can provide

AIM: Rapid evaluation of patients with acute coronary syndrome (ACS) attending the emergency service under emergency room conditions and using appropriate risk scoring would improve treatment success. Calcium levels accumulate in the tissue in people with coronary artery disease and this has been found to correlate with osteopontin levels in some studies. It is predicted that osteopontin level could be used as a biomarker to detect coronary artery calcification. In this study, we aimed to evaluate the use of osteopontin levels in the differential diagnosis of ACS in conjunction with cardiac tr

BACKGROUND: Complement activation has been associated with atherosclerosis, atherosclerotic plaque destabilization and increased risk of cardiovascular events. Complement component 7 (CC7) binds to the C5bC6 complex which is part of the terminal complement complex (TCC/C5b-9). High-sensitivity C-reactive protein (hsCRP) is a sensitive marker of systemic inflammation and may reflect the increased inflammatory state associated with cardiovascular disease.
AIM: To evaluate the associations between CC7 and total- and cardiac mortality in patients hospitalized with chest-pain of suspected coronary

Blastomyces is a fungus found in the soil of regions of North America including the Mississippi and Ohio River Valleys. It can be inhaled into the lungs and cause pneumonia and disseminated disease. Although blastomycosis is not widely reported in the sickle cell literature, sickle cell patients may be at increased risk of complications from blastomycosis pneumonia due to their immune compromise and risk of developing acute chest syndrome. We describe the case of a 13-year-old female with homozygous sickle cell disease who presented with pneumonia and acute chest syndrome and was found to have
