Co-occurrence of psychotic disorders and borderline personality disorder: a systematic review and meta-analysis.

BACKGROUND: The co-occurrence of psychotic disorders and borderline personality disorder (BPD) complicates clinical management, with overlapping symptoms exacerbating morbidity and impairing therapeutic outcomes. This systematic review and meta-analysis aimed to estimate the prevalence of psychotic disorders and BPD co-occurrence, including with first-episode psychosis (FEP) and to describe associated sociodemographic and clinical characteristics. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, four databases were systematically s

Multiple sclerosis and affective disorders.

Affective disorders occurring in association with multiple sclerosis have been attributed both to the psychosocial impact of a chronic disabling illness and to the structural lesions of cerebral demyelination. A review of research evidence suggests that while there is a correlation between chronic depressive symptoms and both progressive disability and lack of social support, acute major depressive and manic episodes may be psychiatric manifestations of demyelinating lesions and may be the initial presenting symptoms of multiple sclerosis. Anti-inflammatory agents may be required in the manage

Motor activity patterns in acute schizophrenia and other psychotic disorders can be differentiated from bipolar mania and unipolar depression.

The purpose of this study was to compare 24-h motor activity patterns between and within three groups of acutely admitted inpatients with schizophrenia and psychotic disorders (n = 28), bipolar mania (n = 18) and motor-retarded unipolar depression (n = 25) and one group of non-hospitalized healthy individuals (n = 28). Motor activity was measured by wrist actigraphy, and analytical approaches using linear and non-linear variability and irregularity measures were undertaken. In between-group comparisons, the schizophrenia group showed more irregular activity patterns than depression cases and h

[Development of schizophrenia-spectrum psychoses caused by psychotraumatic events].

A study included 213 patients diagnosed with schizoaffective disorder (56.3%) or paranoid schizophrenia (43.7%). In all patients, psychotic symptoms developed under the influence of psychogenic factors. The high percentage of pathological personality disorders (50.2%), neurotic disorders (52.1%) and psychogenic depressive disturbances (53.5%) in the premorbid were found that indirectly indicates a liability to psychogenically caused endogenous psychoses. Three stages of development of schizophrenia-spectrum psychosis under the influence of psychogenic factors were singled out: psychogenic, end

Childhood and adolescent psychotic experiences and risk of mental disorder: a systematic review and meta-analysis.

BACKGROUND: Psychotic experiences (PEs) are common in childhood and adolescence and their association with mental disorders is well-established. We aim to conduct a quantitative synthesis the literature on the relationship between childhood and adolescent PEs and (i) any mental disorder; and (ii) specific categories of mental disorder, while stratifying by study design. METHOD: Three electronic databases (PUBMED, PsycINFO and EMBASE) were searched from inception to August 2017 for all the published literature on childhood and adolescent PEs and mental disorder (outcome) in non-help-seeking co

Attempted suicide in the armed forces: a retrospective study of Norwegian conscripts.

In the easily surveyable and rather homogeneous military population of the Norwegian Armed Forces, a group of 112 male suicide-attempting conscripts was studied retrospectively. Compared to a normal conscript population, suicide attempters more often had troublesome backgrounds, had experienced psychiatric problems and abuse, and had previously more often attempted suicide. Diagnostically adjustment disorders, alcohol and drug abuse, and personality disorders predominated, whereas major affective disorders and psychotic disorders were rare. Methods of self-destruction employed were mostly drug

Use of the Research Diagnostic Criteria and the Schedule for Affective Disorders and Schizophrenia to study affective disorders.

In a pilot study of 150 manic or depressive patients, the authors used the Research Diagnostic Criteria (RDC) and the Schedule for Affective Disorders and schizophrenia (SADS) to perform preliminary analysis of symptom pictures of the index episode of different diagnostic groups, joint diagnostic classification of the different subtypes of major depressive disorder, and differential outcome by diagnostic groups. The results suggest that schizophrenic symptoms in affective disorders do have diagnostic and prognostic significance, that the term "psychotic depression" should be limited to impaire

The predictive validity and outcome of ICD-10 and DSM-5 short-lived psychotic disorders: a review and meta-analysis.

The ICD-10 Classification of Mental and Behavioural Disorders introduced the category of 'acute and transient psychotic disorders' (ATPDs) encompassing polymorphic, schizophrenic and predominantly delusional subtypes, and the forthcoming ICD-11 revision has restricted it to polymorphic psychotic disorder, while the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) listed 'brief psychotic disorder' (BPD). To assess the predictive validity and outcome of ATPDs and BPD, relevant papers in English, French and German were searched in PubMed and Web of Science. Where possible meta-analys

Free-thyroxine index in psychotic and neurotic depression.

The mean free-thyroxine index value of a group of 25 psychotic depressive patients was found to be significantly lower than that of an age- and sex-matched group of 20 neurotic depressives (P less than 0.01). These findings support the view that psychotic and neurotic depression are differentiated not only with respect to their clinical expression, but also on account of their biological substrate. Comparison of free-thyroxine index values of each group of patients with a group of 240 euthyroid subjects hospitalized for various somatic illnesses revealed a lower mean free-thyroxine index value

Adolescent bipolar disorder: a nine-year experience.

The clinical records of 53 adolescent inpatients diagnosed with bipolar disorder were re-evaluated to examine their profiles and differential diagnoses. Seventy-two percent satisfied DSM-III-R Bipolar Disorder criteria and most presented with manic or mixed mood symptoms. Half were psychotic. Non-affective psychosis, unipolar depression, organic syndromes and disruptive behavioral disorders complicated the differential diagnoses, particularly among younger boys. The well-documented bipolars were characterized by affective symptoms prior to the first admission and shorter hospitalizations than

[Psychogenic psychoses. A review of the clinical picture, genesis, prognosis and therapy].

The concept of psychogenic psychosis has always been regarded with scepticism, since it seems to harbour a contradiction in itself: Psychogenic mental disturbances are amenable to a rational approach, that is to say, they are understandable, whereas psychoses are incomprehensible mental disease patterns in respect of their content and course. Nevertheless, psychogenic psychoses have been described fairly often ever since scientific psychiatry came into being; in fact, they are being diagnosed quite frequently in some countries. For the purpose of helping to clarify these theoretical and practi

Understanding the intersections between ethnicity, area-level deprivation, and inpatient-related features amongst patients with psychotic disorders: a mental health electronic records analysis.

PURPOSE: Ethnic and area-level deprivation disparities in psychiatric inpatient outcomes amongst patients with psychotic disorders are known. However, how these two variables intersect to influence features of inpatient care is unclear. We investigated this intersection. METHODS: Using de-identified electronic health data from inpatient services at a large south London mental healthcare provider, we identified a sample of 6767 working-age patients with non-affective psychotic disorders who were admitted between 2016 and 2019. Logistic and negative binomial regressions were used to examine the

Epidemiology of Untreated Psychoses in 3 Diverse Settings in the Global South: The International Research Program on Psychotic Disorders in Diverse Settings (INTREPID II).

IMPORTANCE: Less than 10% of research on psychotic disorders has been conducted in settings in the Global South, which refers broadly to the regions of Latin America, Asia, Africa, and Oceania. There is a lack of basic epidemiological data on the distribution of and risks for psychoses that can inform the development of services in many parts of the world. OBJECTIVE: To compare demographic and clinical profiles of cohorts of cases and rates of untreated psychoses (proxy for incidence) across and within 3 economically and socially diverse settings in the Global South. Two hypotheses were teste

OBJECTIVE: Schizophrenia and other non-affective psychoses (NAP) are associated with an increased prevalence of both type 2 diabetes (DM2) as well as a family history of DM2. We performed a systematic review and meta-analysis of the association between comorbid DM2 and a family history of DM2 in patients with NAP. METHOD: We searched major electronic databases from inception until August 2018 for studies of comorbid DM2 in patients with non-affective psychosis and family history of DM2 status. Metacognitive beliefs mediate the effect of emotional abuse on depressive and psychotic symptoms in severe mental disorders.

BACKGROUND: Early trauma is linked to higher symptom levels in bipolar and psychotic disorders, but the translating mechanisms are not well understood. This study examines whether the relationship between early emotional abuse and depressive symptoms is mediated by metacognitive beliefs about thoughts being uncontrollable/dangerous, and whether this pathway extends to influence positive symptoms. METHOD: Patients (N = 261) with psychotic or bipolar disorders were assessed for early trauma experiences, metacognitive beliefs, and current depression/anxiety and positive symptoms. Mediation path

[Carbamazepine in the treatment of psychiatric diseases: effects and side effects].

The use of carbamazepine in psychiatry has widened its scope in the last years. Its efficacy is proven best in manic syndromes, in the prophylaxis of manic-depressive illness and in the treatment of alcohol withdrawal symptoms. Beyond this, carbamazepine has been used successfully in depression, schizophrenia, organic psychoses and intermittent explosive disorder. The manifold therapeutic possibilities should not make forget the side effects. The most severe effects to be expected are impairment of the hematopoietic system and skin reactions. Vegetative and neurotoxic effects most commonly occ

Clinical characteristics and legal consequences of violent behavior: a case of bipolar disorder.

The main subject of criminal proceedings is that of criminal responsibility, from this point of view bipolar disorders sometimes seem to be a neglected subject in legal scholarship. Yet they may affect decision-making across the spectrum of the law, especially when manic and psychotic symptoms are implicated. This case studies a 37-year-old woman, diagnosed with bipolar affective in disorder, who attacked the neighbour of her ex-husband during a manic episode with psychotic symptoms. Two groups of those psychotic symptoms are especially remarkable: delusions and experiences of influences playi

The co-occurrence of manic and depressive dimensions in early psychosis: a latent transition analysis.

BACKGROUND: Frequently associated with early psychosis, depressive and manic dimensions may play an important role in its course and outcome. While manic and depressive symptoms can alternate and co-occur, most of the studies in early intervention investigated these symptoms independently. The aim of this study was therefore to explore the co-occurrence of manic and depressive dimensions, their evolution and impact on outcomes. METHODS: We prospectively studied first-episode psychosis patients (N = 313) within an early intervention program over 3 years. Based on latent transition analysis, we

Mapping established psychopathology scales onto the Hierarchical Taxonomy of Psychopathology (HiTOP).

The Hierarchical Taxonomy of Psychopathology (HiTOP) organizes phenotypes of mental disorder based on empirical covariation, offering a comprehensive organizational framework from narrow symptoms to broader patterns of psychopathology. We argue that established self-report measures of psychopathology from the pre-HiTOP era should be systematically integrated into HiTOP to foster cumulative research and further the understanding of psychopathology structure. Hence, in this study, we mapped 92 established psychopathology (sub)scales onto the current HiTOP working model using data from an extensi
