The ultrastructure and histochemistry of the human urethral mucosa was studied. By scanning electronmicroscopy (SEM) the cover cell of the urethra was found to be polygonal and with a surface topography characterized by numerous microvilli and micro-ridges. By transmission electronmicroscopy (TEM) the cover cells were shown to be interconnected with tight junctions but to lack the asymmetric luminal membrane and the fusiform vacuoles that characterize urothelium above the bladder neck. Insulin-like immunoreactivity can be localized in the cells of the mid-gut of Unio pictorum Lam. The cells containing the substance immunoreactive to mammalian anti-insulin serum, can be stained with paraldehyde-fuchsin or alcian blue and give positive PAS reaction. Intramuscular glucose administration produces degranulation of the cytoplasm in these cells: the quantity of cells stainable with paraldehyde-fuchsin significantly decreased 24 hours after glucose injection.

To understand better the response of patients with allergic rhinitis to nasal challenge with antigen, we studied the mechanism of priming, that is, the increased clinical response to daily sequential nasal challenges. Ten subjects with ragweed hay fever were challenged four times with increasing doses of ragweed pollen. Cutaneous calcinosis is reported in the lesions of a woman with long-standing discoid lupus erythematosus. Calcium deposits were found in the middle and deep parts of the dermis surrounded by homogeneous collagen staining positively with Alcian blue solution. In contrast to the relative frequency of this event in patients with scleroderma or dermatomyositis, calcinosis cutis occurs very rarely in lupus erythematosus. To the best of our knowledge, this is the first report of calcinosis cutis in discoid lupus erythematosus.

We report 4 cases of a naevocellular naevus variant containing abundant amounts of mucinous matrix. The mucinous matrix was Alcian blue (pH 2.5) and Halle's colloidal iron positive and mucicarmine and PAS negative. When the blocks of tumours were cut down, regions typical of common intradermal naevi were found in all 4 cases. The differential diagnosis with mucinous carcinoma of the skin and myxoid malignant melanoma is discussed.

Although aortic or cardiac complications are common in patients with Marfan syndrome, the presence of an intracranial aneurysm is comparatively rare. In this study, the authors report on their experience with resection of a ruptured fusiform aneurysm of the posterior cerebral artery in a 30-year-old woman with Marfan syndrome. Microscopic examination of the resected tissue showed many Alcian blue-staining deposits, consistent with the presence of mucopolysaccharide in the tunica media and focal fragmentation of the internal elastic lamina.

Histopathological study of filamentary keratitis was done by using replica technique in five patients. The filaments are produced by sliding of the epithelial cells around small areas of focal degeneration of the superfical epithelium. In comparison to the filaments in other conditions, the filaments in KCS are smaller and thicker and do not show the torsional segment. The filaments contain degenerated epithelial cells and alcian blue-PAS positive mucoid material. Making a corneal replica may result in healing of filamentary keratitis.
