Participant Demographic and Baseline Drinking Factors Can Predict Alcohol Use Disorder Pharmacotherapy Clinical Trial Completion and Drinking Outcomes.

BACKGROUND: Partially due to the complexity associated with conducting trials, there have been relatively few regulatory agency-approved medications for the treatment of alcohol use disorder (AUD). Heterogeneity of study samples, such as participant characteristics (including variation in alcohol consumption) as well as drinking efficacy endpoints, may lead to inconsistency in results and a potential increase in Type II errors. The aim of this study is to begin to fill in the knowledge gap of optimal clinical trial design by analyzing potential predictors of outcomes. METHODS: Five federally

Alcohol Drinking Patterns and Laboratory Indices of Health: Does Type of Alcohol Preferred Make a Difference?

Although excessive alcohol consumption is a highly prevalent public health problem the data on the associations between alcohol consumption and health outcomes in individuals preferring different types of alcoholic beverages has remained unclear. We examined the relationships between the amounts and patterns of drinking with the data on laboratory indices of liver function, lipid status and inflammation in a national population-based health survey (FINRISK). Data on health status, alcohol drinking, types of alcoholic beverages preferred, body weight, smoking, coffee consumption and physical ac

The successor representation in high-risk drinking and alcohol-related contexts.

The successor representation (SR) is a reinforcement learning strategy which has been suggested to underlie nuanced forms of habitual behavior, rendering it a candidate mechanism for mental health conditions thought to involve habitual control, such as addiction. However, paradigms used to assess habits in humans cannot detect SR use. Together with the fact that they are usually conducted in disorder-irrelevant contexts, this may explain why the evidence for a hypothesized shift from goal-directed to habitual behavior in addiction is inconsistent. Here, we tested whether individuals with high-

Drinking motives among patients with alcohol use disorder: a longitudinal study.

BACKGROUND: This current two-year clinical study among adult patients with alcohol use disorder (AUD) examined drinking motives and their cross-sectional and longitudinal associations with alcohol consumption, severity of AUD, and other alcohol-related problems. Enhancement, coping, conformity, and social motives have been found to impact alcohol use, yet longitudinal studies on drinking motives in clinical populations are sparse. METHODS: This observational study used data from a randomized controlled trial (RCT) conducted in Stockholm, Sweden, from 2017 to 2022, including 250 participants wi

Peripheral alcohol metabolism dictates ethanol consumption and drinking microstructure in mice.

BACKGROUND: Ethanol metabolism is intimately linked with the physiological and behavioral aspects of ethanol consumption. Ethanol is mainly oxidized by alcohol dehydrogenase (ADH) to acetaldehyde and further to acetate via aldehyde dehydrogenases (ALDHs). Understanding how ethanol and its metabolites work together to initiate and drive continued ethanol consumption is crucial for identifying interventions for alcohol use disorder (AUD). Therefore, the goal of our study was to determine how ADH1, which is mainly peripherally expressed and metabolizes >90% of ingested ethanol, modulates ethanol

The Drinking Dashboard for alcohol-induced blackout: A randomized pilot trial.

BACKGROUND: Alcohol-induced "blackouts," or memory loss for events that occur while drinking, are prevalent and problematic among young adults. They also increase motivation to change. This study developed and pilot-tested a theoretically informed digital health intervention ("Drinking Dashboard") for alcohol-induced blackouts. METHODS: Data were collected using qualitative (Study 1) and quantitative (Study 2) methods. Participants in both studies were young adults (ages 18-30 years) across the United States who reported alcohol-induced blackout(s) in the past month. Study 1 participants (N =

Traumatic stress-enhanced alcohol drinking: Sex differences and animal model perspectives.

PURPOSE OF REVIEW: Stress is associated with alcohol drinking, and epidemiological studies document the comorbidity of alcohol use disorder (AUD) and post-traumatic stress disorder (PTSD), with higher comorbid prevalence in females than in males. The aim of this paper is to highlight information related to sex differences in stress-enhanced alcohol drinking from clinical studies and from preclinical studies utilizing an animal model of traumatic stress. RECENT FINDINGS: Stress is associated with alcohol drinking and relapse in males and females, but there are sex differences in the alcohol-re

Mouse parasubthalamic Crh neurons drive alcohol drinking escalation and behavioral disinhibition.

Corticotropin-releasing factor (CRF, encoded by Crh) signaling is thought to play a critical role in the development of excessive alcohol drinking and the emotional and physical pain associated with alcohol withdrawal. Here, we investigated the parasubthalamic nucleus (PSTN) as a potential source of CRF relevant to the control of alcohol consumption, affect, and nociception in mice. We identified PSTN Crh neurons as a neuronal subpopulation that exerts a potent and unique influence on behavior by promoting not only alcohol but also saccharin drinking, while PSTN neurons are otherwise known to

Using machine learning to predict heavy drinking during outpatient alcohol treatment.

BACKGROUND: Accurate clinical prediction supports the effective treatment of alcohol use disorder (AUD) and other psychiatric disorders. Traditional statistical techniques have identified patient characteristics associated with treatment outcomes. However, less work has focused on systematically leveraging these associations to create optimal predictive models. The current study demonstrates how machine learning can be used to predict clinical outcomes in people completing outpatient AUD treatment. METHOD: We used data from the COMBINE multisite clinical trial (n = 1383) to develop and test p

Alcohol solution strength preference predicts compulsive-like drinking behavior in rats.

BACKGROUND: Understanding compulsive drinking behavior is key to improving outcomes in the treatment of addiction. In the present study, we investigated compulsive-like drinking in alcohol-addicted rats using the alcohol deprivation effect (ADE) model of relapse behavior, which involves repeated deprivation and reintroduction phases; the latter approximate relapse. METHODS: High-resolution longitudinal drinking and locomotor data were measured while rats (n = 30) underwent a four-bottle (water, 5%, 10%, 20% alcohol v/v) free-choice ADE paradigm. Alcohol bottles were adulterated with the bitte

Frequency of Hazardous and Binge Drinking Alcohol Among Hospitalized Cardiovascular Patients.

Excessive alcohol use is a risk factor for most cardiac diseases. The prevalence of unhealthy alcohol use among hospitalized cardiac patients is uncertain as is the frequency with which it is addressed. We performed a single center, patient-level anonymous survey among hospitalized cardiac patients eligible for cardiac rehabilitation. Hazardous drinking was defined as an Alcohol Use Disorders Identification Test (AUDIT) score of 8 or greater. Binge drinking was defined as 5+ drinks for men or 4+ for women on ≥1 occasion within the past 30 days. Unhealthy drinking was defined as either hazardou

Prevention, screening, and treatment for heavy drinking and alcohol use disorder.

Heavy drinking and alcohol use disorder are major public health problems. Practitioners not specialising in alcohol treatment are often unaware of the guidelines for preventing, identifying, and treating heavy drinking and alcohol use disorder. However, a consensus exists that clinically useful and valuable tools are available to address these issues. Here, we review existing information and developments from the past 5 years in these areas. We also include information on heavy drinking and alcohol use disorder among individuals with co-occurring psychiatric disorders, including drug use disor

Safer-drinking strategies used by chronically homeless individuals with alcohol dependence.

Chronically homeless individuals with alcohol dependence experience severe alcohol-related consequences. It is therefore important to identify factors that might be associated with reduced alcohol-related harm, such as the use of safer-drinking strategies. Whereas effectiveness of safer-drinking strategies has been well-documented among young adults, no studies have explored this topic among more severely affected populations, such as chronically homeless individuals with alcohol dependence. The aims of this study were thus to qualitatively and quantitatively document safer-drinking strategies

Fenofibrate--a lipid-lowering drug--reduces voluntary alcohol drinking in rats.

The administration of disulfiram raises blood acetaldehyde levels when ethanol is ingested, leading to an aversion to alcohol. This study was aimed at assessing the effect of fenofibrate on voluntary ethanol ingestion in rats. Fenofibrate reduces blood triglyceride levels by increasing fatty acid oxidation by liver peroxisomes, along with an increase in the activity of catalase, which can oxidize ethanol to acetaldehyde. UChB drinker rats were allowed to consume alcohol 10% v/v freely for 60 days, until consumption stabilized at around 7 g ethanol/kg/24 h. About 1-1.2 g ethanol/kg of this inta

Drinking trajectories from adolescence to the fifties among alcohol-dependent men.

OBJECTIVE: Although it has been recognized that the course of alcoholism may differ across individuals, little work has characterized drinking trajectories from drinking onset to midlife. METHOD: The current study examined trajectories of alcohol dependence from adolescence to the mid-50s in a sample of 420 men with a lifetime diagnosis of alcohol dependence. Men from the Vietnam Era Twin Registry were given the Lifetime Drinking History, which assesses the patterns of alcohol consumption and diagnostic symptoms for self-defined drinking phases. Phase data were converted into person-year data
