Compensational strategies for a merchant after stroke with anosognosia for alexia without agraphia.

We report the first case, to our knowledge, of successful return to work of a patient with alexia without agraphia. This case is also interesting as it is the first report of which we are aware of anosognosia for alexia without agraphia: the patient confabulated when asked to read English text, but immediately stated that he could not read Chinese text because he did not know that language. The selective nature of this confabulation would not be inconsistent with anosognosia being one of the brain's responses to absence of afferent information.

[Interhemispheric disconnection syndrome in glioma of the posterior part of the corpus callosum].

The syndrome of interhemispheric disconnexion has been systematically researched in 10 cases of medial and posterior callosal tumours. The cases have been documented with CT Scann and two of them were anatomically verified. Two had a complete disconnexion syndrome; another, whose lesion was situated on the forceps major, had no sign of disconnexion. In the others, the semiology was proportional to the extent of the tumour, sometimes minimal and in this case, only corresponding to an extinction of the left ear in the dichotic test, which therefore appeared in our series as the most constant sig

Does dyslexia develop from learning the alphabet in the wrong hemisphere? A cognitive neuroscience analysis.

A new perspective is described which views developmental dyslexia as the outcome of learning to write the alphabet in the nondominant (right) hemisphere. The letter-level and whole-word subtypes of dyslexia are seen as differing responses adopted to cope with this predicament. Striking similarities between dyslexics and callosotomy patients in the allocation of covert attention to lateralized stimuli provide direction for integrating a diversity of dyslexic research within this framework. This synthesis, together with information from pure alexia, brain activation, and reading research, lends

Meaning for reading pseudowords: errors reveal semantic influences on pseudoword reading after stroke.

Impaired reading, i.e., alexia, is common after left hemisphere stroke. The most common deficit in alexia is a difficulty reading aloud pronounceable novel words, also called pseudowords. While semantic and phonological processes both subserve reading real words, pseudoword reading deficits in alexia are typically ascribed to phonological deficits alone. Some theories, however, suggest that pseudoword reading relies in part on lexical-semantic knowledge, such that semantic deficits could also contribute to poor pseudoword reading in alexia. Leveraging a large sample of left-hemisphere stroke s

Visual attention in posterior stroke and relations to alexia.

Impaired visual attention is common following strokes in the territory of the middle cerebral artery, particularly in the right hemisphere, while attentional effects of more posterior lesions are less clear. Commonly, such deficits are investigated in relation to specific syndromes like visual agnosia or pure alexia. The aim of this study was to characterize visual processing speed and apprehension span following posterior cerebral artery (PCA) stroke. In addition, the relationship between these attentional parameters and single word reading is investigated, as previous studies have suggested

Reading faces: investigating the use of a novel face-based orthography in acquired alexia.

Skilled visual word recognition is thought to rely upon a particular region within the left fusiform gyrus, the visual word form area (VWFA). We investigated whether an individual (AA1) with pure alexia resulting from acquired damage to the VWFA territory could learn an alphabetic "FaceFont" orthography, in which faces rather than typical letter-like units are used to represent phonemes. FaceFont was designed to distinguish between perceptual versus phonological influences on the VWFA. AA1 was unable to learn more than five face-phoneme mappings, performing well below that of controls. AA1 suc

Alexia without agraphia in multiple sclerosis: case report with magnetic resonance imaging localization.

The syndrome of alexia without agraphia occurs rarely in multiple sclerosis (MS). We report a patient with right homonymous hemianopsia and alexia without agraphia as his initial manifestations of relapsing-remitting MS. Magnetic resonance imaging (MRI) demonstrated a hyperintense lesion in the left occipital subcortical white matter (WM) and an enhancing lesion in the splenium of the corpus callosum. The clinical presentation and MRI findings were consistent with disconnection of the functional right occipital visual cortex from structures responsible for language comprehension in the left he

Semantic capacities of the right hemisphere as seen in two cases of pure word blindness.

Two patients with pure alexia were studied with tachistoscopically presented stimuli to examine factors influencing their ability to distinguish words from nonwords and to derive semantic information at exposures too brief for explicit letter identification. Both patients had profound right hemianopia and computerized tomography (CT) evidence of splenial destruction. Both patients were successful in making word/nonword decisions for high-frequency, but not low-frequency, words. They could judge semantic class membership reliably for such common categories as animals and vegetables, but not for

Reading with the right hemisphere: evidence from transcranial magnetic stimulation.

Although many patients with dominant hemisphere damage retain some reading ability, the anatomic substrate of the preserved reading remains controversial. We tested the hypothesis that the right hemisphere mediates the reading of some patients with acquired dyslexia by using Transcranial Magnetic Stimulation (TMS) to assess the effect of transient cortical stimulation on reading. A patient with (partially recovered) pure alexia was asked to read aloud briefly presented words, half of which were shown in association with TMS of the right or left hemisphere. Consistent with the right hemisphere

The pathophysiology of letter-by-letter reading.

Pure alexia is a frequent and incapacitating consequence of left occipitotemporal lesions. It is thought to result from the disruption or the disconnection of the visual word form area (VWFA), a region reproducibly located within the left occipito-temporal sulcus, and encoding the abstract identity of strings of visual letters. Alexic patients often retain effective single letter recognition abilities, and develop an effortful letter-by-letter reading strategy which is the basis of most rehabilitation techniques. We study a patient who developed letter-by-letter reading following the surgical

Dissociation of reading strategies: letter-by-letter reading in the native language and normal reading in the learned language. A case study.

In letter-by-letter reading, which is typically observed in Dejerine's (1892) "pure alexia," oral reading seems to be mediated by the naming of the constituent letters of the printed sequence: reading time rises abnormally as a function of the number of letters of the target item. We describe a patient with fluent aphasia who showed the unusual pattern of letter-by-letter reading together with surface dyslexia in her native language (French) and apparently normal reading in the second, learned language (English). Thus, letter-by-letter reading should be considered as a description of a symptom

Imaging of Retrochiasmal and Higher Cortical Visual Disorders.

Retrochiasmal visual pathways include optic tracts, lateral geniculate nuclei, optic radiations, and striate cortex (V1). Homonymous hemianopsia and field defect variants with relatively normal visual acuity suggest that the lesions involve retrochiasmal pathways. From V1, visual input is projected to higher visual association areas that are responsible for perception of objects, faces, colors, and orientation. Visual association areas are classified into ventral and dorsal pathways. Damage to the ventral stream results in visual object agnosia, prosopagnosia, and achromatopsia. Balint syndrom

A case of pure agraphia due to left thalamic hemorrhage.

Pure agraphia is an isolated writing disturbance without intellectual impairment, aphasia, alexia, or apraxia. The authors report a case of pure agraphia following left thalamic hemorrhage. The patient showed impairment in the writing of both kanji and kana. The most frequent error in the writing of kanji was substitution (46%) followed by no reaction (23%) and partial omission or addition of characters (21%). The copying of kanji by hand was normal. Even when the patient was unable to write a kanji character, he understood the meaning and correctly recited the other readings. He complained th

A Patient With Word Blindness in the Seventeenth Century.

Johann Jakob Wepfer (1620-1695), city physician in Schaffhausen, Switzerland, published two books on "apoplexy." He proposed new ideas about the events in the brain during such attacks, based on Harvey's theory of the circulation of the blood. Wepfer postulated extravasation of whole blood or serum in the brain, in opposition to the Galenic notion of blocked ventricles. His case histories are remarkably precise and untainted by interpretation. This allows the recognition of a patient with word blindness, who was also unable to read words written by himself. Unlike patients with pure "alexia wi

Alexia without agraphia: from infarctions to malignancies.

Alexia without agraphia is a neurological syndrome characterised by an acquired inability to read with a preserved ability to write. It is caused by the combined effect of two lesions: in the splenium of the corpus callosum and in the occipital lobe of the dominant hemisphere. Splenial lesions disconnect the language areas in the temporal and parietal lobes of the dominant hemisphere from the visual areas in the occipital cortex of the contralateral side, while lesions in the dominant occipital lobe cause homonymous hemianopia. We describe two patients with lesions affecting the splenium and d

Brain MRI Lesions in Alexia Without Agraphia: A Case-Control Study.

BACKGROUND: Alexia without agraphia (AWA) is an acquired reading disturbance associated with left posterior cerebral artery (PCA) infarction. Based on autopsy and neuroimaging, there are two explanations for its pathogenesis: a visual cortex-language cortex disconnection and a visual word-form agnosia. Our goal was to discover if more refined brain imaging in a case-control study would provide further imaging support for either of these hypotheses. METHODS: A neuroradiologist masked to the presence of AWA reviewed diffusion-weighted and fluid-attenuated inversion recovery brain MRIs in patien
