Allergic Contact Cell-Mediated Hypersensitivity in Psoriasis: A Narrative Minireview.

The dysfunctionality of the protective skin barrier in psoriasis allows easier cutaneous penetration of various contact haptens; thus, such patients can develop allergic contact hypersensitivity as a comorbidity. Both skin conditions involve T-cell-mediated mechanisms. Dermatologists and allergists should consider assessing allergic contact cell-mediated hypersensitivity in selected psoriasis patients, especially those with palmoplantar psoriasis and who are refractory to topical treatments, and in patients with psoriasis, with or without arthritis, treated with biologics that present skin les

Evaluation of house dust mite allergy in real life: patients' characteristics and satisfaction with treatment.

BACKGROUND: HDMs are a ubiquitous allergen source, with a very well defined biology, but their role in clinical settings and in everyday clinical practice is not well characterized. Aim of this cross-sectional, questionnaire-based study was to assess the clinical characteristics of HDM-related respiratory allergy in a large population of Italian patients. METHODS: A structured questionnaire was sent to allergists randomly chosen among those of the Italian Federation of Immunology, Allergy and Clinical Immunology (IFIACI). They were asked to fill it with the clinical data of 10-12 consecutive

Patterns of skin prick test positivity in allergic patients: usefulness of a nationwide SPT chart review.

BACKGROUND: A previous survey on allergens used by Mexican allergists in their skin prick test (SPT) panel showed wide variation. Humidity varies in different zones of Mexico. This might lead to differences in natural exposure and allergic sensitisation throughout the country. We aim to describe the SPT sensitivity patterns in the different climatic zones in Mexico and to show the usefulness of a structured SPT chart-review including multiple clinics in obtaining these allergen sensitisation patterns. METHODS: A retrospective, structured chart-review of SPT results was undertaken in allergy c

The health effects of non-industrial indoor air pollution.

BACKGROUND: There is growing public awareness regarding the risk associated with poor indoor air quality in the home and workplace. Because Americans spend approximately 22 hours every day indoors, susceptible individuals are at much greater risk of adverse health effects from chronic low levels of exposure to indoor air pollutants over time. Along with particulate matter, gases such as ozone, nitrogen dioxide, carbon monoxide, and sulfur dioxide; microbial and chemical volatile organic compounds; passive smoke; and outdoor ambient air are the most common types of air pollutants encountered in

Dietary protein enterocolitis.

Dietary protein enterocolitis generally presents in the 1st year of life with diarrhea, emesis, and irritability. When there is a delay in diagnosis, persistent exposure to the offending dietary antigen leads to increasing enteric inflammation manifesting as bloody diarrhea, anemia, dehydration, and failure to sustain normal patterns of weight gain and growth. The extent of enteric inflammation may be limited to mild proctitis, pancolitis, or true enterocolitis with esophagitis, gastritis, enteropathy, and colitis. The offending antigen is usually cow's milk protein or soy protein. A significa

Penicillin allergy delabeling practices and barriers across SHEA research network US institutions: a cross-sectional survey.

We surveyed SHEA Research Network institutions in the U.S. to characterize penicillin allergy delabeling practices. Although most institutions reported active delabeling programs, we found substantial variability in these initiatives. Reported barriers included minimal electronic health record (EHR) integration and time constraints. Expanding non-allergist-led programs and EHR integration are critical to optimizing and advancing delabeling.

Not quite asthma: differential diagnosis of dyspnea, cough, and wheezing.

Every allergist will eventually encounter a patient who has a persistent cough, dyspnea, and wheezing, but few atopic references, and a limited response to conventional treatment. Is this asthma or something different? The conditions that are misleading can be fairly ordinary or they can be esoteric. This article reviews the characteristics of several of the more common, less common, and uncommon masqueraders of asthma, along with the differentiating features that can help physicians recognize them.

Mast Cell Activation Syndrome and Mimickers.

Mast cell activation syndrome (MCAS) is a challenging specialized condition that allergists-immunologists treat in the outpatient setting. Despite the wide prevalence of the disease, clinicians may feel overwhelmed to provide consistent compassionate care for these patients for a multitude of reasons. Patients uniformly describe feeling unheard during a clinical encounter and thereby often end up seeing multiple physicians and specialists which further complicates their medical care. As such, it is important to characterize patients with MCAS accurately, which includes the assessment of coexis

COVID-19 pandemic: Practical considerations on the organization of an allergy clinic-An EAACI/ARIA Position Paper.

BACKGROUND: The coronavirus disease 2019 (COVID-19) has evolved into a pandemic infectious disease transmitted by the severe acute respiratory syndrome coronavirus (SARS-CoV-2). Allergists and other healthcare providers (HCPs) in the field of allergies and associated airway diseases are on the front line, taking care of patients potentially infected with SARS-CoV-2. Hence, strategies and practices to minimize risks of infection for both HCPs and treated patients have to be developed and followed by allergy clinics. METHOD: The scientific information on COVID-19 was analysed by a literature se

COVID-19 and allergy: How to take care of allergic patients during a pandemic?

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), also known as COVID-19, is a new strain of coronavirus that has not been previously identified in humans. SARS-CoV-2 is recognized as a highly contagious respiratory virus with severe morbidity and mortality, especially in vulnerable populations. Being a novel disease, everyone is susceptible, there are no vaccine and no treatment. To contain the spread of the disease, health authorities throughout the world have restricted the social interactions of individuals in various degrees. Allergists, like other physicians, are faced with t

Updating Allergy and/or Hypersensitivity Diagnostic Procedures in the WHO ICD-11 Revision.

The classification of allergy and/or hypersensitivity conditions for the World Health Organization (WHO) International Classification of Diseases (ICD)-11 provides the appropriate corresponding codes for allergic diseases, assuming that the final diagnosis is correct. This classification should be linked to in vitro and in vivo diagnostic procedures. Considering the impact for our specialty, we decided to review the codification of these procedures into the ICD aiming to have a baseline and to suggest changes and/or submit new proposals. For that, we prepared a list of the relevant allergy and

Advances in the treatment of peanut allergy: a case report.

BACKGROUND: Peanut allergies affect 1.5% of children. The majority of reactions to peanuts are mild, but peanut allergy is also the most common cause of fatal anaphylactic reactions to food. CASE REPORT: The purpose of this case report was to describe a 1-year old boy who developed difficulty breathing after eating a peanut food product. The boy was taken immediately by his mother to an Emergency Department, exhibiting severe respiratory distress. After speaking to the child's mother, the emergency physician (EP) realized that the wheezing was due to a peanut food allergy. The child's respira

The Importance of Considering Olfactory Dysfunction During the COVID-19 Pandemic and in Clinical Practice.

The emergence of a worldwide pandemic due to coronavirus disease 2019 (COVID-19) and frequent reports of smell loss in COVID-19-infected patients have brought new attention to this very important sense. Data are emerging that smell impairment is a prominent symptom in COVID-19 and that this coronavirus behaves differently in causing olfactory dysfunction compared with other respiratory viruses. Anosmia and hyposmia, the complete and partial loss of smell, respectively, can result from many causes, most commonly from viral infections, sinonasal disease, and head trauma. Olfactory dysfunction ne
