The "Step Diet" is a physician-designed activity management protocol for patients with foot pathology or prior reconstructive surgery who experience persistent pain from overuse. Rather than pursuing the mythologized 10,000 step daily goal-rooted in 1960s Japanese marketing, not medical evidence-patients track daily steps and record pain levels on a 1 to 10 scale over 4 weeks. A personalized safe step threshold is then identified, with two permitted "cheat days" weekly. This structured approach enables patients to budget around priorities, predict discomfort, and plan recovery periods. Clinica

The aim of this study was to completely disinfect the interior of an ambulance vehicle by hydrogen peroxide fumigation. This process was validated in three subsequent test runs to ensure that bacterial spores were inactivated. Thus, test carriers containing 1 × 106 spores of Geobacillus stearothermophilus as well as chemical indicators (strips showing a color change in the presence of hydrogen peroxide vapor) were placed in 20 different locations inside an ambulance vehicle. After testing different hydrogen peroxide fumigation protocols using a generator connected to the vehicle from the outsi

BACKGROUND: Despite increasing interest in the occurrence of workplace violence in ambulance services, few studies have explored the phenomenon from the perspective of the ambulance personnel. While previous studies have identified its incidence and risk factors, this study aimed to explore ambulance personnel's understanding of why workplace violence occurs.
METHODS: An inductive, qualitative study was conducted using digital focus group interviews. The inclusion criteria were as follows - having worked in ambulance services for a minimum of two years and having personally experienced workpl

The host immune response in sepsis involves both pro- and anti-inflammatory mechanisms, with monocytes playing a central role in the process. We have previously identified an in vitro response profile of endotoxin (LPS) tolerant primary human monocytes, consisting of eight cytokines/chemokines as well as a set of five transcription factors. In the current study, we evaluated differences in expression levels of these investigated molecular markers across different patient groups (patients with or without infection, and with or without sepsis), and their association with clinical outcomes (septi

BACKGROUND: Limb girdle muscular dystrophy type R9 (LGMDR9) results from biallelic variants in FKRP. There is limited data to predict loss of ambulation (LOA) among those with LGMDR9.
METHODS: Participants in an ongoing dystroglycanopathy natural history study (NCT00313677) with FKRP variants who had achieved ambulation and were more than 3 years old were included (n = 97). LOA was defined as self-reported full-time wheelchair use, weakness preventing completion of the 10-m walk-run test (10MWT) or 10MWT time > 30 s. Interval-censored time-to-event analysis was used to determine median age at

Pathogenic TUBB2A variants are known to cause a rare form of autosomal dominant tubulinopathy. Our aim is to expand the phenotypic spectrum of TUBB2A by documenting a milder phenotype caused by a previously unreported pathogenic variant. We report a newly diagnosed case of TUBB2A tubulinopathy with novel clinical findings of ataxia with preserved ambulation into adulthood, in addition to the already characteristic findings of global developmental delay in childhood, cortical malformations, and epilepsy. Informed consent was obtained for this publication. A 40 year-old man presented to the clin

Early postoperative (post-op) ambulation benefits burn survivors by expediting return to functional independence, preventing bedrest complications, and shortening hospital admissions. Despite published practice guidelines supporting early ambulation, significant variability in practice continues among burn centers, ranging between 0 and 14 days post-op. A 3-year retrospective review was completed of this 15 bed, adult verified burn center, to identify 149 patients who underwent split thickness skin graft (STSG) placement and/or Autologous Skin Cell Suspension (ASCS) application to their lower

BACKGROUND: The routine use of lights and sirens (L&S) by emergency medical services (EMS) may be associated with increased risk of ambulance crashes. Recently, the National Association of EMS Physicians and American College of Emergency Physicians, among others, released a joint statement recommending decreased use of L&S when appropriate.
OBJECTIVES: Given the recent shifting culture in recommended L&S use, our objective was to describe current EMS response times across the U.S., stratified by urbanicity and patient age.
METHODS: This was a cross-sectional evaluation of the 2023 National E

OBJECTIVE: The aim of this study was to evaluate the effect of surgical procedures on maintaining ambulation for Duchenne patients.
METHODS: This retrospective cohort study evaluated 35 patients for whom surgery was recommended at our institution from 2012 to 2020.
RESULTS: Twenty-seven patients were operated on before gait loss, and eight after. In this study, surgical treatment allowed recovery and prolongation of gait for 38.6 months, on average. The sooner the surgery was performed, the better the results were; logistic regression analysis showed that each day of delay after gait loss de

INTRODUCTION: Delirium is an acute and often fluctuating disturbance in attention and awareness and is one of the organ dysfunctions caused by acute illness. A previous study revealed that 8.3% of older patients in the emergency room (ER) had delirium, and dementia and older age were identified as independent risk factors for delirium in the ER. However, different ER structures (intermediate or acute care units) and disease clusters may have different prevalence rates and risk factors.
PURPOSE: This study aimed to identify the prevalence and risk factors for delirium on arrival at the ER.

BACKGROUND: The Modified 5-Factor Frailty Index (mFI-5) has been shown to predict complications following treatment in geriatric patients. However, few studies have compared the mFI-5 with other trauma scoring systems in cases involving multiple injuries. This study aimed to evaluate the Relationship Between mFI-5, Injury Severity Score (ISS), and Geriatric Trauma Outcome Score (GTOS) and their association with mortality in geriatric trauma patients.
METHODS: This retrospective cohort study included patients aged 65 and older who were admitted to the emergency trauma unit of a tertiary care h

Inability to ambulate is a common issue for which an Emergency Department (ED) may request inpatient admission to the hospital. Frequent contributing factors include advanced age, spinal stenosis, osteoarthritis, deconditioning, and polypharmacy. We present an interesting case of a patient admitted due to inability to ambulate who had a significant acute decline in function coincident with the introduction of gabapentin in the setting of heavy tetrahydrocannabinol (THC) use.

INTRODUCTION: There is significant public health interest towards providing medical care at mass-gathering events. Furthermore, mass gatherings have the potential to have a detrimental impact on the availability of already-limited municipal Emergency Medical Services (EMS) resources. This study presents a cross-sectional descriptive analysis to report broad trends regarding patients who were transported from National Collegiate Athletic Association (NCAA) Division 1 collegiate football games at a major public university in order to better inform emergency preparedness and resource planning for

BACKGROUND: Treatment of acute stroke, before a distinction can be made between ischemic and hemorrhagic types, is challenging. Whether very early blood-pressure control in the ambulance improves outcomes among patients with undifferentiated acute stroke is uncertain.
METHODS: We randomly assigned patients with suspected acute stroke that caused a motor deficit and with elevated systolic blood pressure (≥150 mm Hg), who were assessed in the ambulance within 2 hours after the onset of symptoms, to receive immediate treatment to lower the systolic blood pressure (target range, 130 to 140 mm Hg)

INTRODUCTION: Retrospectively investigated this relationship using data from Shimoda Fire Department.
METHODS: We investigated patients who were transported by Shimoda Fire Department from January 2019 to December 2021. The participants were divided into groups based on the existence of incontinence at the scene or not (Incontinence [+] and Incontinence [-]). We compared the variables mentioned above between these groups.
RESULTS: There were 499 cases with incontinence and 8241 cases without incontinence. There were no significant differences between the two groups with respect to weather an

OBJECTIVE: To investigate the association of multiple predictors (gait ability, depression, balance, strength of knee extensors on the paretic side, and mobility) with community ambulation (walking speed ≥0.8 m/s) of individuals after chronic stroke.
METHODS: A cross-sectional study was carried out with 60 individuals after chronic stroke with a mean age of 59 (±15.75) years. The walking speed (10-m walk test) was used to assess the outcome variable (walking speed ≥0.8 m/s) and the predictors were gait ability (ABILOCO), depression (GDS-15 Geriatric Depression Scale), balance (Berg Balance Sc

BACKGROUND: Quantifying associations between genetic mutations and loss of ambulation (LoA) among males diagnosed with childhood-onset dystrophinopathy is important for understanding variation in disease progression and may be useful in clinical trial design.
METHODS: Genetic and clinical data from the Muscular Dystrophy Surveillance, Tracking, and Research Network for 358 males born and diagnosed from 1982 to 2011 were analyzed. LoA was defined as the age at which independent ambulation ceased. Genetic mutations were defined by overall type (deletion/duplication/point mutation) and among del

In syndesmosis injury, whether the syndesmosis screw should be removed prior to weight-bearing remains controversial. The aim of this study was to compare the functional outcome between removed screw and retained groups and between recurrence of diastasis and no diastasis groups.Fifty-six patients who had undergone open reduction and internal fixation due to syndesmosis injury were retrospectively evaluated and divided into four groups: (A) removed syndesmotic screw before weight-bearing (postoperative 3 months, n = 28), (B)retained (n = 28), (C) recurrence of diastasis (n = 9), and (D) no dia

BACKGROUND: In out-of-hospital cardiac arrest (OHCA), geographic disparities in outcomes may reflect baseline variations in patients' characteristics but may also result from differences in the number of ambulances providing basic life support (BLS) and advanced life support (ALS). We aimed at assessing the association between allocated ambulance resources and outcomes in OHCA patients in a large urban community.
METHODS: From May 2011 to January 2016, we analyzed a prospectively collected Utstein database for all OHCA adults. Cases were geocoded according to 19 neighborhoods and the number o

UNLABELLED: Myelomeningocele (MMC)-commonly known as spina bifida-is a congenital birth defect that causes lifelong paralysis, incontinence, musculoskeletal deformities, and severe cognitive disabilities. The recent landmark Management of Myelomeningocele Study (MOMS) demonstrated for the first time in humans that in utero surgical repair of the MMC defect improves lower limb motor function, suggesting a capacity for improved neurologic outcomes in this disorder. However, functional recovery was incomplete, and 58% of the treated children were unable to walk independently at 30 months of age.
