In this paper, the results of studies that have used the intracarotid sodium amytal (amobarbital) test to examine hemispheric lateralization of emotional expression has been reviewed. The important findings are that the emotional states are dissociated from the neurological effects (aphasia, hemiplegia), and occur later, when the electroencephalogram (EEG) is showing evidence of activation. The effects of left hemisphere barbiturization are less clear than those of the right hemisphere, and there is good concordance that the latter lead to states of alteration in mood or euphoria.

Data are given for the relative electron capture response of the 2-chloroethyl derivatives of ten barbituric acids and five common anticonvulsant drugs. Results indicate that these new derivatives significantly improve the detection limits of the barbituric acids but lead to no advantages for the anticonvulsant compounds. Structural features in the two classes of compounds are believed to explain the differences in the sensitivity of detection. Linear calibration plots exist for the concentration ranges 0.1-1.0 and 1.0-10.0 microgram/ml of amobarbital.

Left-handedness is a normal variant but also may result from early-life brain insults. Speech typically resides in the left cerebral hemisphere in normal subjects. In 170 subjects with past brain injuries, bilateral or right hemisphere speech lateralization, determined by the intracarotid amobarbital procedure, was more frequent in females (19 vs 5, p = 0.003) as was left-handedness (26 vs 10, p = 0.004). This could be attributed to greater plasticity or functional symmetry in females.

A study was undertaken to determine if humans excreted barbital N-glucoside as a urinary metabolite following oral administration of barbital. A liquid chromatography method using gradient elution was developed for detecting and quantifying barbital N-glucoside and barbital in urine. A 55 year old left handed man with left hemisphere subcortical encephalomalacia, seizures, language impairment, and right hemiparesis from a motor vehicle accident at age five was evaluated for epilepsy surgery. Adult male rats were subjected to intraperitoneal (i.p.) saline (controls) or barbiturates (20 mg/rat) as a single injection. Seventeen hours later the rats were sacrificed and plasma collected. The prevalence of atypical (right, bilateral) speech lateralization is unknown in normal populations. The authors investigated this by studying people with normal developmental histories but a later, specific adult neurologic event leading to intractable epilepsy.
