Retroperitoneal hemorrhage following spinal anesthesia: A case report.

Serious neurological problems following spinal anesthesia are rare, although they can occur. We report a case of a 31-year-old female patient, parturient, Gravida 2 Para 1, with a body mass index (BMI) of 25 kg/m2 who had a paramedian approach spinal anesthesia using 23-gauge Quincke spinal needle for an emergent cesarean section due to failure to progress. Four attempts were required to achieve successful spinal blockade. The procedure was successfully performed with no complications. On the first postoperative day, the patient started to complain of pressure headache and abdominal pain radia

Spinal Sonography for Ultrasound-Guided Lumbar Neuraxial Anesthesia.

Neuraxial anesthesia is one of the few remaining forms of regional anesthesia that relies on palpation and tactile feedback techniques to facilitate catheterization into the epidural space. Over two decades ago, spine ultrasonography was demonstrated to provide reliable guidance for locating the epidural space. Compared to the palpation technique, preprocedural ultrasonography has been shown to result in fewer needle punctures and fewer traumatic procedures, particularly in patients with abnormal or distorted spine anatomy (e.g., scoliosis, obesity). Despite its utility, the ultrasound-guided

Pseudomonas Meningitis Post Spinal Anesthesia: A Case Report.

Nosocomial meningitis following spinal anesthesia is a rare but potentially life-threatening complication that breaches the central nervous system's natural defense barriers. This report presents a case of Pseudomonas meningitis post spinal anesthesia, emphasizing the diagnostic, management, and preventive strategies for iatrogenic bacterial meningitis. A 53-year-old patient with sickle cell disease developed febrile confusion 10 days after spinal anesthesia for hemorrhoidal surgery, presenting with meningeal signs and positive infectious markers. Cerebrospinal fluid analysis revealed purulent

Preoperative anxiety about spinal surgery under general anesthesia.

PURPOSE: No study has investigated preoperative anxiety about spinal surgery under general anesthesia. The purposes of this study were (1) to determine how many patients have preoperative anxiety about spinal surgery and general anesthesia, (2) to evaluate the level of anxiety, (3) to identify patient factors potentially associated with the level of anxiety, and (4) to describe the characteristics of the anxiety that patients experience during the perioperative period. METHODS: This study was performed in 175 consecutive patients undergoing laminectomy for lumbar stenosis or discectomy for he

Persistent paralysis after spinal anesthesia for cesarean delivery.

Anterior spinal artery syndrome has rarely been reported as a cause of permanent neurologic complications after neuraxial anesthesia in obstetric patients. A parturient developed anterior spinal artery syndrome after spinal anesthesia for cesarean delivery. A healthy 32-year-old parturient presented at 41(2/7) weeks for primary elective caesarean delivery for breech presentation. Spinal anesthesia was easily performed with clear cerebrospinal fluid, and block height was T4 at 5 minutes. Intraoperative course was uneventful except for symptomatic bradycardia (37-40 beats per minute) and hypoten

Black esophagus (acute esophageal necrosis) after spinal anesthesia.

Acute esophagic necrosis or black esophagus is an uncommon clinical entity that owes its name to the endoscopic view of the necrotic esophageal mucosa. It is always related with a critical medical condition and usually has an ischemic etiology. We report the first case of acute esophageal necrosis after a spinal anesthetic for partial hip joint arthroplasty. We discuss the underlying pathophysiological mechanisms.

[Unusual complication after hip surgery under spinal anesthesia].

The development of an acute respiratory distress syndrome following hip surgery in elderly patients is suggestive of thromboembolism in most instances. However, we must keep in mind the possibility of rarer complications, which can remain undiagnosed because they are hidden by prominent abnormal behaviours, which can develop following any type of anaesthesia. We report the case of a patient who developed a confusion following an orthopaedic surgery under spinal anaesthesia; this confusion concealed a penetration syndrome resulting from accidental inhalation of a dental crown. Because this pati

Hemiballism-hemichorea from marked hypotension during spinal anesthesia.

Hemiballism and hemichorea following anesthesia-induced hypotension has rarely been described, but a recent case suggests an association. After experiencing marked hypotension during spinal anesthesia, a 70-year-old woman developed hemiballism and hemichorea. Involuntary ballistic movements with writhing, consisting of repetitive rotation and flexion-extension without apparent muscle weakness, affected her left limbs proximally. Low-amplitude, involuntary, choreiform movements involved the distal portions of these limbs. Magnetic resonance imaging demonstrated an area of high signal intensity

[Neurologic sequelae of spinal anesthesia: local anesthetic toxicity].

Recent reports of cauda equina syndrome following continuous spinal anesthesia have generated concern regarding the safety of not only this particular technique but also of the local anesthetic agent itself. This concern has been reinforced by data suggesting that similar injuries have occurred with repeated injection after a "failed spinal", and by reports of transient radicular irritation following single subarachnoid injection. This paper summarizes the above cases and reviews recent experiments performed to determine the mechanisms underlying local anesthetic toxicity and the factors that

Cranial nerve paresis following epidural and spinal anesthesia.

Three cases of cranial nerve paresis following epidural and spinal anesthesia are presented. One patient had combined oculomotor and abducens nerve paresis, while the other two patients had solitary abducens nerve pareses. The authors postulate that mechanical traction is the cause of cranial nerve paresis following epidural and spinal anesthesia.

Postoperative delirium after short-acting spinal anesthesia and general anesthesia after shared decision-making.

ObjectivePrevious studies have shown that the postoperative delirium rate does not differ between anesthetic techniques in randomized controlled trials. Subjective concerns such as anxiety and pain are often not adequately addressed in randomized controlled trials and reported to be associated with postoperative delirium. Shared decision-making is reported to have an impact on anxiety and pain. Therefore, the aim of this study was to evaluate the effect of shared decision-making while making a choice between spinal and general anesthesia on postoperative delirium incidence.MethodsThis prospect

Laparoscopic Totally Extraperitoneal Inguinal Hernia Repair Under Combined Spinal Epidural Anesthesia Versus General Anesthesia.

BACKGROUND: Inguinal hernia repair is one of the most frequently used elective surgical operations in the world. General anesthesia (GA) has risks and routine postoperative complications, such as nausea, vomiting, throat irritation, and postoperative pain. Regional anesthesia (RA) has many advantages over GA, such as faster recovery, less postoperative pain, nausea, and vomiting, and less hemodynamic compromise. We aimed to investigate the efficacy and safety of combined spinal and epidural anesthesia (CSEA) in preventing shoulder pain in patients undergoing totally extraperitoneal herniorrhap

Spinal anesthesia for pediatric urological surgery: Reducing the theoretic neurotoxic effects of general anesthesia.

BACKGROUND: Spinal anesthesia (SA) is an effective technique that has been used in children for years. With growing concern with regard to the risks of general anesthesia (GA), we developed a SA program to provide an alternative option. We present our initial experience with this program. OBJECTIVE: To implement a SA program at a large tertiary care pediatric center and assess the safety and efficacy of the technique as an alternative to GA for urologic surgery. STUDY DESIGN/METHODS: We prospectively collected data on all children undergoing SA at our institution. We recorded demographics, p

Spinal anesthesia using Taylor's approach helps avoid general anesthesia in short stature asthmatic patient.

The case history of a 35-year-old female patient with short stature is presented. She was posted for rectopexy in view of rectal prolapse. She was a known case of bronchial asthma. She had crowding of intervertebral spaces, which made administration of spinal anesthesia via the normal route very difficult. Taylor's approach for administration of the same was tried and proved successful, thus saving the patient from receiving general anesthesia in the presence of bronchial asthma, for a perineal surgery. The possible cause for the difficulty in administration of spinal anesthesia and the Taylor

Total spinal anesthesia during combined general-epidural anesthesia in a 7-year-old child.

Total spinal anesthesia (TSA) is a rare complication of lumbar epidural anesthesia through inadvertent spinal injection of local anesthetics following an undiagnosed dural breach or spinal placement of the catheter. TSA has rarely been reported in children. TSA occurred during epidural anesthesia in a 7-year-old child undergoing abdominal surgery. Recent previous lumbar punctures and intrathecal chemotherapy for Burkitt's lymphoma at the same level may have facilitated dural breach. Epidural anesthesia should not be attempted at the same intervertebral level as prior recent lumbar punctures.

[Differentiation of brain stem anesthesia from high spinal anesthesia using auditory brain stem response].

A 67-year old woman having intractable chronic postherpetic neuralgia at the neck to forearm for two years was treated with subarachnoid block. Initially, bolus of 1% lidocaine 8 ml, with methylprednisolone acetate 20 mg was injected intrathecally at 6th cervical intervertebral space. The auditory brain stem response (ABR) during high spinal block with intact consciousness was not depressed in its wave height. ABR recorded showed prolongation of latency of 3 and 5 waves and prolongation of 1-3 phase to phase interval without prolongation of 3-5 phase to phase interval. Intrathecal nerve block
