UNLABELLED: Anisocoria often raises concerns about potential underlying conditions such as intracranial hemorrhage, brain tumor, or Horner syndrome. However, iatrogenic exposures may also lead to unilateral mydriasis. A six-month-old infant was hospitalized due to acute bronchiolitis with a history of prematurity, bronchopulmonary dysplasia, and periventricular leukomalacia (PVL). He received non-invasive respiratory support, salbutamol, and ipratropium bromide by inhalation. Four days after admission, the infant exhibited an episode of sudden unilateral mydriasis with no other new signs or sy

UNLABELLED: This Invited Commentary discusses the following article: BACKGROUND:: To identify the etiologies of adult Horner syndrome (HS) in the MRI era using a targeted evaluation approach and to assess the value and yield of targeted imaging.
METHODS: A retrospective chart review was performed of 200 adult outpatients with HS, confirmed with cocaine eyedrop testing. Patients were divided into subgroups based on the presence or absence of symptoms and those who did or did not receive additional testing with hydroxyamphetamine drops. Imaging was obtained based on pharmacologic localization a

Waardenburg and congenital Horner syndromes are both recognized causes of congenital hypochromic iris heterochromia. Each has been linked to disruptions in the pathway of tyrosinase induction, thus leading to a deficiency in melanin production of the iris. These syndromes must be considered in the differential diagnosis of a patient presenting with heterochromia iridis. We present the case of a 20-month old boy afflicted with both congenital Horner syndrome and Waardenburg syndrome, type II. In contrast to the more common presentation of congenital Horner syndrome, the affected iris in this ca

Understanding the anatomy and physiology of the eye, the orbit, and the central connections is key to understanding neuro-ophthalmologic emergencies. Anisocoria is an important sign that requires a systematic approach to avoid misdiagnosis of serious conditions, including carotid dissection (miosis) and aneurysmal third nerve palsy (mydriasis). Ptosis may be a sign of either Horner syndrome or third nerve palsy. An explanation should be pursued for diplopia since the differential diagnosis ranges from the trivial to life-threatening causes.

A 4-week-old boy with left ptosis, anisocoria, and a mass on his left hand was diagnosed with Horner syndrome. The diagnosis precipitated a work-up for a possible malignant etiology. Magnetic resonance imaging demonstrated enlarged left cervical and axillary lymph nodes. A biopsy of the hand lesion confirmed embryonal rhabdomyosarcoma, but a biopsy of the axillary lymph node was negative. Mechanical pressure by noncancerous enlarged lymph nodes is hypothesized to cause the Horner syndrome.

A 34-year-old man was referred to our hospital for management of multiple trauma in consequence of a car accident. Initial examination showed mild intracranial hemorrhage proven by computed tomographic scan (CT) of the brain and multiple fractures of face and thigh. Surgical repair of the multiple fractures was undertaken with stable vital signs and neurological condition. Anesthesia was induced with fentanyl and thiamylal and rocuronium was used to facilitate oroendotracheal intubation. Sevoflurane in a mixture of oxygen and air and rocuronium were used for maintenance of anesthesia and surgi

A 38-year-old woman presented with impaired consciousness and anisocoria due to a subarachnoid hemorrhage and an intracerebral hemorrhage of the left temporal lobe. Examination revealed severe tortuosity in the left middle cerebral artery and three sequential bead-like aneurysms. She underwent a craniotomy in the acute phase to stop rebleeding at the rupture site and remove the intracerebral hematoma. During the chronic phase, endovascular treatment with superselective balloon test occlusion (ssBTO) was performed for the remaining aneurysms. Preoperative ssBTO was useful in evaluating collater

Multiple ocular associations with Duane syndrome have been reported as summarized by DeRespinis et al. The most frequently noted include nystagmus, anisocoria, ptosis, optic nerve colobomas, and epibulbar dermoids. Less encountered abnormalities are optic nerve hypoplasia, heterochromia, and congenital cataract. To our knowledge, no reports of associated congenital glaucoma exist in the literature. We report a 10-year-old patient with bilateral Duane syndrome and bilateral congenital glaucoma who we have monitored for 7 months after birth.

Evaluation of horses for eye problems and for suspected impairment of visual acuity often requires a thorough neuroophthalmic examination to be conducted. Along with a full ophthalmic evaluation, the results of such an examination should indicate the likely site of any lesion(s) accounting for blindness, miosis, mydriasis, anisocoria, Horner's syndrome, strabismus, facial paralysis and facial spasm, nystagmus and other signs of vestibular disease, and cerebellar disorders, that may be present. Such a thorough examination with guidance for correct interpretation of findings is given here.

A 10-year-old girl with Apert syndrome underwent a Le Fort III osteotomy with the positioning of internal and external distraction devices. The operation was straightforward with no intraoperative complications. Very soon after completion of surgery an anisocoria (unilateral dilation of a pupil) was noticed. This was followed by intracranial oedema which was fatal. The aetiology was dissection of the right internal carotid artery is reported. The complications of Le Fort osteotomies are discussed regarding patients with complex syndromal craniosynostosis and midface hypoplasia, such as Apert s

BACKGROUND: Pupillary examinations provide early subtle signs of worsening intracranial pathology. Objective pupillomtery assessment, although not yet the standard of care, is considered best practice. However, inconsistent findings from objective pupillometry studies have caused a lack of consensus among clinicians; as such, no clinical guidelines are available to guide clinical use of objective pupillometer devices. To add to the body of evidence, the purpose of this project was to explore the relationship between objective pupillometry metrics and midline shift (MLS). METHODS: A retrospecti

The records of 90 cases of oculosympathetic paresis (1982-1991), 39 women and 51 men aged between 3 months and 82 years, were evaluated. The mean baseline anisocoria was 0.92 mm but did not exceed 2.4 mm. The mean difference in the position of the upper eyelid was 2.3 mm. Enophthalmus of 1 mm or more was found in only 25% and exophthalmus of 1 mm or more, in 18%. Exophthalmus or enophthalmus of more than 2 mm was not encountered. The cocaine test (5% solution in most cases) was performed in 85 cases and could be quantified in 65 cases. The average dilation of the involved pupil was 0.52 mm, wh

BACKGROUND: Asymmetric mydriasis, or unequal pupil size, requires careful evaluation to exclude serious neurological conditions. Pharmacologic causes are well-known, but reports have mainly involved prescription antiperspirants for underarm use. This case is novel because it describes, to our knowledge, the first instance of bilateral mydriasis and anisocoria caused by a non-prescription facial antiperspirant. This highlights a new public health concern related to easily accessible consumer products. CASE PRESENTATION: A young female presented with acute, unequally enlarged pupils (right pupil

OBJECTIVES: The autonomic neural circuitry of the pupil makes it a unique target for identifying impaired autonomic regulation following spinal cord injury (SCI). This systematic review characterizes the evidence on pupillary changes after SCI.
METHODS: A systematic search was conducted in MEDLINE, Embase, and CINAHL. Articles were independently assessed by 2 reviewers and included studies reporting on pupillary changes (e.g., miosis, mydriasis, anisocoria) following traumatic or nontraumatic SCI at any level. Data on study design, patient demographics, injury characteristics, and pupillary c

BACKGROUND: South Dakota is one of eight states that do not require any vision screening for children. This study describes the results of the first children's vision screening program in the state.
METHODS: Children ages 6 months to 12 years were screened using the SPOT photoscreener by lay volunteers as part of the Northern Plains Eye Foundation's Western South Dakota Children's Vision Screening Initiative (CVSI). Referral criteria were based on the recommendations of the manufacturer. Data was stratified by age group, sex, and percentage of children referred for hyperopia, myopia, astigmat

A 33-year-old white man developed premature presbyopia and anisocoria as initial manifestations of acute pandysautonomia. Nine months later signs and symptoms of generalized, severe autonomic dysfunction developed, and six years later only paralysis of pupillary reactions, presbyopia, and orthostatic hypotension were unresolved. Pharmacologic testing of the pupils demonstrated no mydriasis to cocaine 4% or hydroxyamphetamine 1% and hypersensitivity to epinephrine 0.1%, methacholine 2.5%, and pilocarpine 0.0625%, suggesting the presence of sympathetic and parasympathetic, postganglionic blockag

A 30-year-old male sought medical consultation due to sudden anisocoria. Clinical examination confirmed Adie's tonic pupil. Subsequently, the presence of segmental anhidrosis with compensatory hyperhidrosis and areflexia established the diagnosis of Ross syndrome. In a 15-year follow-up, uncommon manifestations developed in the facial territory affected by compensatory hyperhidrosis: unilateral facial pallor and alopecia following a midline-defined pattern. This case emphasizes the importance of recognizing neurocutaneous manifestations in dermatology practice and suggests that Ross syndrome m

A 57-year-old female presented with headache, miosis, and ptosis diagnosed as Horner syndrome (HS). After delaying the recommended diagnostic imaging, she experienced transient, unilateral visual impairment in bright light. The patient was subsequently determined to have a spontaneous internal carotid artery dissection (ICAD) and secondary retinal ischemia with minimal cardiovascular risk factors and no history of preceding trauma. She wore dark glasses, received gabapentin for pain control, and was anticoagulated for a total of 4 months at which time the ICAD resolved despite a residual bleph
