Accuracy of ultrasound in diagnosing ankle injuries in emergency care.

BACKGROUND: Ankle injuries are one of the most common presentations in the ED. Although fractures can be ruled out using the Ottawa Ankle Rules, the specificity is low, which means many patients may still receive unnecessary radiographs. Even once fractures are ruled out, assessment of ankle stability is recommended to rule out ruptures, but the anterior drawer test has only moderate sensitivity and low specificity and should be performed only after swelling has receded. Ultrasound could be a reliable, cheap and radiation free alternative to diagnose fractures and ligamentous injuries. The pur

Acute ankle injuries: association between sprain severity and ancillary findings.

UNLABELLED: Miranda et al. reported a correlation between the significance of injuries to osseous, chondral, tendon, and ligamentous tissues in participants with low-grade versus high-grade acute ankle sprains. They demonstrated that participants with high-grade ankle sprains presented with shorter calcaneonavicular distances and increased rates of structural abnormalities compared to those with low-grade sprains. Special attention should be paid to acute ankle sprains in emergency settings to avoid failure in detecting severe injuries that could lead to chronic pain, impairment, or instabilit

Bosworth Ankle Injuries - Treatment Challenges in Late and Neglected Presentations.

INTRODUCTION: Bosworth ankle injuries (BAIs) are rare talocrural dislocations with bi or trimalleolar fractures. Attempted closed reduction fails due to incarcerated fibular fracture behind the distal tibia. The diagnostic delay and failed attempts to restore anatomical reduction result in significant ankle swelling, compromised skin, and imminent compartment syndrome. In neglected cases with unreduced dislocation over months, capsular and tendo-Achilles contracture, additional disuse osteoporosis, adds to the complexity of surgical management. In chronic and neglected cases, literatures have

Acute foot and ankle injuries and time return to sport.

Foot and ankle sports injuries encompass a wide spectrum of conditions from simple contusions or sprains that resolve within days to more severe injuries that change the trajectory of an athlete's sporting career. If missed, severe injuries could lead to prolonged absence from the sport and therefore a catastrophic impact on future performance. In this article, we discuss the presentation of the commonest foot and ankle sports injuries and share recent evidence to support an accurate diagnosis and best management practice.

Weightbearing Cone-Beam Computed Tomography of Acute Ankle Syndesmosis Injuries.

Syndesmotic injuries are common, but only a subset of these injuries are unstable. A noninvasive tool for identifying instability would aid in the selection of patients for surgery. Weightbearing computed tomography (CT) data have been reported for healthy patients, but there are limited data on unstable syndesmoses. We evaluated the syndesmotic area of arthroscopically proven unstable ankles after acute injury. This is a prospective comparative study of consecutive patients recruited to a weightbearing CT database. Thirty-nine patients were included for analysis with arthroscopically proven u

Ankle injuries in the Netherlands: Trends over 10-25 years.

Ankle injuries are a common health problem; data on ankle injury rates and time trends in the population at large are scarce. Our aim was to investigate the incidence of and time trends in population-based and emergency department-treated ankle injuries related to sports activities and other activities related to daily living. Data were obtained from one national survey on accidents and injuries (2000-2010) and one based on emergency department data (1986-2010). Linear regression was used to determine linear trends in ankle injuries per 1000 person-years. The number of ankle injuries related t

Simplified diagnostic algorithm for Lauge-Hansen classification of ankle injuries.

Ankle injuries occur in a predictable sequence, allowing a logical understanding of their classification once the injury mechanism is recognized. The Lauge-Hansen classification system was developed on the basis of the mechanism of trauma and is useful for guiding treatment. Three radiographic views of the ankle (anteroposterior, mortise, and lateral) are necessary to classify an injury with the Lauge-Hansen system. Two additional criteria are also necessary: the position of the foot at the time of injury and the direction of the deforming force. Because understanding the mechanism of trauma i

Unique complications of foot and ankle injuries secondary to warfare.

This article discusses the common complications associated with lower extremity trauma and amputations secondary to combat injuries. The complications include retained fragments, soft tissue adhesions, poor wound healing, painful bursae, neuroma formation, heterotopic ossification, and depleted uranium. Although there is some literature on these topics, most is based on noncombat injuries, indicating a need for further research into the management of these devastating injuries.

Foot and ankle injuries during the Athens 2004 Olympic Games.

BACKGROUND: Major, rare and complex incidents can occur at any mass-gathering sporting event and team medical staff should be appropriately prepared for these. One such event, the Athens Olympic Games in 2004, presented a significant sporting and medical challenge. This study concerns an epidemiological analysis of foot and ankle injuries during the Games. METHODS: An observational, epidemiological survey was used to analyse injuries in all sport tournaments (men's and women's) over the period of the Games. RESULTS: A total of 624 injuries (525 soft tissue injuries and 99 bony injuries) were

The epidemiology of knee and ankle injuries on Macquarie Island.

Macquarie Island is a small, rugged sub-Antarctic island with a scientific research station and a considerable reputation for knee and ankle injuries amongst the Australian Antarctic Division expeditioner population. In order to examine the accuracy of this reputation, a 10-year retrospective analysis of all knee and ankle injuries recorded by the Macquarie Island Medical Officer in the medical logs was undertaken. Knee and ankle injuries comprised 13% of the 2,678 recorded medical consultations. The majority of initial injuries occurred in the field and almost a third occurred during work rel

Limited dorsiflexion predisposes to injuries of the ankle in children.

Injuries to the ankle are common in children. We investigated whether decreased dorsiflexion predisposes to such fractures and sprains. Passive dorsiflexion in children with ankle injuries was compared with that in a control group of patients with a normal ankle. The uninjured side was examined to determine flexibility in those patients with ankle injuries. In 82, the mean dorsiflexion was 5.7 degrees with the knee extended and 11.2 degrees with the knee flexed. In 85 controls, the mean dorsiflexion was 12.8 degrees with the knee extended and 21.5 degrees with the knee flexed (p < 0.001, Stude

Sprained ankle syndrome: prevalence and analysis of 639 acute injuries.

The ankle sprain is often thought of as an injury involving only the lateral ankle ligaments. Frequently other structures are also injured. However, the literature describes only some of the associated injuries. The authors feel that a thorough analysis of each structure injured with the inversion and eversion ankle sprain along with the incidence would be invaluable in making an accurate diagnosis and providing appropriate treatment. The authors conducted a prospective study using a standardized evaluation during the initial examination of patients reporting with an ankle sprain. Over a 33-mo
