A Tool Integrated into the Electronic Health Record to Guide Proper Decision-Making Regarding Peri-Endoscopic Anticoagulant Management: A Retrospective Cohort Study.

Background-Anticoagulants, such as vitamin-K antagonists (VKA) and direct oral anticoagulants (DOAC), are widely used among patients who undergo endoscopic procedures. To balance between bleeding and thromboembolic risks, careful decisions must be made about whether and for how long anticoagulants have to be stopped peri-endoscopically and if bridging is necessary. We created a tool in the electronic health records system (EHR) HIX (Microsoft) for invasive procedures to aid this decision-making. By selecting the anticoagulant indication or thrombo-embolic risk and the bleeding risk of the proc

A Novel Anticoagulation Strategy for High-Volume Plasma Exchange in Pediatric Patients With Acute Liver Failure.

BACKGROUND: Pediatric acute liver failure represents a significant health care concern with significant morbidity and mortality. High-volume plasma exchange has been shown to serve as an effective blood purification technique that has allowed patients with ALF to survive to liver transplantation, or in some cases even spontaneous recovery. This study describes a novel anticoagulation protocol using a combination of citrate and heparin for high-volume plasma exchange in pediatric acute liver failure patients and demonstrates the safety of high-volume plasma exchange in pediatric acute liver fai

[Rivaroxaban, dabigatran and apixaban: new anticoagulants in operative urology].

The periprocedural management of patients receiving long-term oral anticoagulant therapy is a common but complex clinical problem. It is well established that maintaining oral anticoagulation is associated with an increased risk of bleeding in the periprocedural period while discontinuing anticoagulant therapy postoperatively leads to an elevated risk for thromboembolic events, especially in high risk patients. Nowadays there is growing evidence to maintain antiplatelet therapy with acetylsalicylic acid (ASS, Aspirin®) perioperatively in a setting of secondary prophylaxis. Beyond that the incr

Anesthesiologists Guide to the 2019 AHA/ACC/HRS Focused Update for the Management of Patients With Atrial Fibrillation.

Perioperative physicians should be well versed in atrial fibrillation (AF) management because it is the most common sustained arrythmia in the United States. In this narrative review of the 2019 American Heart Association/American College of Cardiologists/Heart Rhythm Society Focused Update on Atrial Fibrillation, the authors detail the emergence of new evidence from completed studies that may affect the management of patients with AF presenting for surgery. Updates regarding non-vitamin K oral anticoagulants (NOACs) comprise the bulk of the update with newer evidence emerging regarding their

Therapeutic anticoagulation and vena caval filter placement-a false sense of security? A case report and literature review.

A 66-year-old male with a past medical history of colon cancer came for elective filter placement in the inferior vena cava and the superior vena cava following repeated venous thromboses. The patient was undergoing chronic anticoagulation and was bridged to surgery with intravenous heparin. Two days postoperative, just prior to the date of discharge, the patient was found tachycardic and, despite all precautions, was diagnosed with an acute pulmonary embolism.

Extensive Iliofemoral Deep Vein Thrombosis Secondary to Uterine Leiomyoma-Induced Iliac Vein Compression: A Case Report.

Deep vein thrombosis (DVT) is an acute vascular condition commonly associated with immobility, hypercoagulability, and endothelial injury. We report the case of a 47-year-old woman presenting with progressive left lower extremity swelling and pain, ultimately diagnosed with extensive iliofemoral DVT secondary to extrinsic compression of the left common iliac vein by a markedly enlarged fibroid uterus. The patient was managed with therapeutic anticoagulation, blood transfusion, and intravenous iron therapy, followed by definitive hysterectomy approximately two months later, with guideline-direc

Anticoagulation Management During a Spinal Cord Stimulator Trial on a Patient With ST-Elevation Myocardial Infarction.

BACKGROUND: Spinal cord stimulators (SCSs) are indicated for the treatment of many pain syndromes. They are often trialed prior to placement of a permanent implant. Patients on anticoagulation therapy are instructed to follow the American Society of Regional Anesthesia and Pain Medicine guidelines during this process. CASE REPORT: Here we describe a unique case of a patient who had an SCS trial, followed by an ST-elevation myocardial infarction (STEMI) on day 4 postimplantation. After coronary revascularization, the patient started dual antiplatelet therapy, which posed significant for epidur

Dynamic transcatheter mitral valve repair: a new concept to treat functional mitral regurgitation using an adjustable spacer.

We report the first-in-man implantation of the Mitra-Spacer. The device was implanted transapically. FMR was reduced to moderate. At two months, while in NYHA Class II, LVEF had improved, but FMR increased and 2 mL was added, reducing FMR to mild. Despite anticoagulation, thrombi developed around the device and the valve was replaced at eight months. The Mitra-Spacer successfully bridged this patient to surgery after LVEF had recovered.

Safety of Laparoscopic Hernia Surgery in Patients With Preoperative Antiplatelet Continuation Therapy.

Introduction The optimal perioperative antithrombotic management of patients receiving antithrombotic therapy (ATT) remains controversial. In this study, we investigated the safety and feasibility of laparoscopic hernia surgery in patients taking ATT, especially those with a preoperative continuation of single antiplatelet therapy (APT). Methods Three hundred ninety-six (396) patients who underwent laparoscopic hernia surgery between April 2014 and March 2023 in our institution were retrospectively reviewed. The patients were divided into two groups: patients who continued single aspirin monot

Management of NOACs in patients undergoing water vapour thermal therapy (REZUM) for benign prostatic obstruction: a comprehensive review.

INTRODUCTION: Minimally Invasive Surgical Techniques (MIST) such as water vapor thermal therapy (WVTT, or Rezum) are increasingly adopted for treating benign prostatic obstruction (BPO), particularly in frail or elderly patients. These procedures offer a safer, less invasive alternative to conventional surgery while preserving sexual function. Frail patients often require long-term anticoagulation with non-vitamin K oral anticoagulants (NOACs), which poses challenges in balancing bleeding and thrombotic risks during urological procedures. OBJECTIVE: To evaluate the bleeding risk profile of in

Concomitant anti-platelet therapy in warfarin-treated patients undergoing cardiac rhythm device implantation: A secondary analysis of the BRUISE CONTROL trial.

BACKGROUND: Anti-platelet therapy is commonly used in patients receiving oral anticoagulation and may increase bleeding risk among patients undergoing cardiac implantable electronic device (CIED) surgery. We sought to determine the proportion of anticoagulated patients who are concomitantly receiving anti-platelet therapy, the associated risk of clinically significant hematoma (CSH), and the proportion of patients in whom anti-platelet usage is guideline-indicated. METHODS: A secondary analysis of the Bridge or Continue Coumadin for Device Surgery Randomized Controlled Trial (BRUISE CONTROL).

Extracorporeal Membrane Oxygenation for Amniotic Fluid Embolism-Induced Cardiac Arrest in the First Trimester of Pregnancy: A Case Report.

UNLABELLED: Amniotic fluid embolism is a rare obstetric emergency that can be accompanied by profound hypoxemia, coagulopathy, hemorrhage, and cardiogenic shock. Extracorporeal membrane oxygenation may provide a rescue strategy in amniotic fluid embolism with cardiopulmonary collapse. Approaches to anticoagulation must be balanced against the risk of hemorrhage with concomitant coagulopathy. Although extracorporeal membrane oxygenation has been described for cardiopulmonary collapse in the setting of amniotic fluid embolism, its initiation as a bridge to hemostasis and cardiopulmonary recovery

Evaluation of enoxaparin for bridging of warfarin in outpatients with left ventricular assist devices (LVADs).

BACKGROUND: Patients with left ventricular assist devices (LVADs) require systemic anticoagulation. The use of enoxaparin for bridging to warfarin remains understudied in this population. METHODS: This single-center retrospective study was performed to characterize enoxaparin use and associated thrombotic and bleeding outcomes in adult outpatients with LVADs from January 2018 to July 2021. RESULTS: Fifty-four enoxaparin bridging events were evaluated in 49 patients. Most patients with HeartMate II (HM2) and HeartWare (HVAD) devices received enoxaparin dosed 1 mg/kg every 12 h. Gastrointestinal endoscopy in patients on anticoagulant therapy and antiplatelet agents.

Periprocedural management of antithrombotics for gastrointestinal endoscopy is a common clinical issue, given the widespread use of these drugs for primary and secondary cardiovascular prevention. For diagnostic procedures, with or without biopsy, no adjustments in antithrombotics are usually needed. For operative procedures, balancing the risk of periprocedural hemorrhage with the continuation of antithrombotics against the chance of recurrent thromboembolic events with their discontinuation may be challenging. Oral anticoagulants need to be temporarily withheld, and consideration must be giv

Percutaneous Microwave Ablation of Hepatocellular Carcinoma As Bridge-to-Transplant Therapy in a High-Risk Patient With Alpha-1 Antitrypsin Deficiency and Factor V Leiden Mutation: A Case Report.

The use of percutaneous microwave ablation has been established as a treatment for early-stage hepatocellular carcinoma (HCC). HCC can often arise in the setting of cirrhosis and often requires locoregional therapy to ablate the area as a bridge for the ultimate treatment of liver transplantation. The challenge is its use in patients with decompensated cirrhosis and hypercoagulable states, as these high-risk populations can lead to other major complications, requiring anticoagulation, which remains challenging. In this case report, a 60-year-old woman with a history of alpha-1 antitrypsin defi

Balancing Bleeding and Clot: Type 2 Heparin-Induced Thrombocytopenia With Extensive Thromboses in a Patient With Subdural Hematoma.

Heparin-induced thrombocytopenia (HIT) type 2 is a rare, immune-mediated complication of heparin therapy characterized by thrombocytopenia and paradoxical thrombosis. It results from platelet-activating antibodies against platelet factor 4 (PF4)/heparin complexes and can precipitate life-threatening thromboembolic events. Management becomes especially challenging in patients with concurrent bleeding, such as intracranial hemorrhage, where anticoagulation carries significant risk. An 87-year-old man with essential thrombocythemia and atrial fibrillation on rivaroxaban presented with a right sub

Perioperative management of antithrombotic drugs in the real world: Study of the impact of not following the consensus document.

INTRODUCTION AND OBJECTIVES: In order to standardise the periprocedural management of antithrombotic drugs, a national and multidisciplinary consensus document was drawn up in 2018. This study analyses the follow-up of this consensus and its outcome in the perioperative setting. METHODS: Sub-analysis of the REQXAA study (observational, prospective, multicentre with 30-day follow-up) in patients scheduled for surgery. Antithrombotic management was considered appropriate according to the consensus document. Logistic regression analysis was used to study the effect of appropriate management and

Utility of a dedicated pediatric cardiac anticoagulation program: the Boston Children's Hospital experience.

Congenital heart disease is the leading cause of stroke in children. Warfarin therapy can be difficult to manage safely in this population because of its narrow therapeutic index, multiple drug and dietary interactions, small patient size, high-risk cardiac indications, and lack of data to support anticoagulation recommendations. We sought to describe our institution's effort to develop a dedicated cardiac anticoagulation service to address the special needs of this population and to review the literature. In 2009, in response to Joint Commission National Patient Safety Goals for Anticoagulati
