Assessment of Some Genetic thrombophilias in Nigerian patients with Venous Thromboembolism.

Venous thromboembolism (VTE) is a leading cause of mortality globally, resulting from genetic risk factors and/or acquired risk factors like oral contraceptives, smoking, diabetes, immobilization, cancer, trauma, fracture, and surgical procedures. This study aims to assess the involvement and prevalence of some genetic thrombophilias in Nigerian VTE patients. A total of 107 participants were recruited, comprising 67 individuals with VTE from three tertiary hospitals in Southwest Nigeria and 50 apparently healthy controls. Parameters included the absolute platelet count, prothrombin time (PT),

Livedo reticularis associated with hereditary protein C deficiency and recurrent thromboembolism.

We report the occurrence of livedo reticularis in a patient with symptomatic hereditary type 1 protein C deficiency. Antithrombin III deficiency and the antiphospholipid syndrome may also be associated with livedo reticularis, and we suggest that a thrombophilia screen may be a useful investigation in a patient with otherwise unexplained livedo, particularly if there is a personal or family history of thromboembolism.

Absence of congenital prethrombotic disorders in children with Legg-Perthes disease.

Resistance to activated protein C (RPCA) and other congenital prethrombotic disorders have been recently reported to be strongly associated with Legg-Perthes disease. RPCA and deficiencies of protein C, protein S, and antithrombin III were sought in 22 children with Legg-Perthes disease. Detection of the factor V Leiden mutation was found in children with RPCA. Twenty-two healthy children paired by age and sex served as controls. The prevalence of congenital prethrombotic disorders was not found to differ significantly among patients with Legg-Perthes disease and among control subjects. Only o

[Thrombolytic, antiplatelet and anticoagulant therapy for thrombotic disorders].

In the antithrombotic therapy, it's important to evaluate the role of thrombus in the present clinical status of thrombotic vascular diseases such as acute arterial occlusion, arteriosclerosis obliterans or deep vein thrombosis. For this purpose, coagulofibrinolytic immunochemical molecular markers such as thrombin-antithrombin III complex and D dimer may be useful. These markers are also useful for the evaluation of therapeutic effect. For the extremely severe ischemic limb, survey and treatment of transient atrial fibrillation and possible embolic source in heart, aorta or arteries are indis

SERPINC1 mutations and thrombotic events in inherited antithrombin deficiency: a study on the han population of East China.

BACKGROUND: Inherited antithrombin deficiency (ATD), a rare autosomal dominant disorder due to SERPINC1 gene mutations, is the most severe inherited thrombophilia. Limited literature exists that focuses on ATD and its mutations in the Chinese population. This study aimed to characterize SERPINC1 gene mutations in a Chinese cohort and to explore their relationship with thrombophilia. METHODS: Coagulation screening results and clinical data were meticulously collected from 23 unrelated probands with ATD and their family members. Genomic DNA was extracted and subjected to PCR amplification and di

Hereditary Antithrombin Deficiency Presenting with Cerebral Venous Thrombosis in Three Members of a Family.

Hereditary antithrombin (AT) deficiency is a rare thrombophilia associated with cerebral vein thrombosis (CVT). We report a case study of hereditary AT deficiency causing CVT in three members of a family. A 29-year-old female presented with features of CVT. Her mother and a sister had CVT in the past and investigation for hereditary thrombophilia revealed low blood AT activity in all of them. The index patient (proband) was positive for the SERPINC1 gene mutation confirming the diagnosis of hereditary AT deficiency. She recovered well with anticoagulation and was advised to continue it lifelon

Antithrombin III metabolism in two patients with a nephrotic syndrome caused by minimal chain nephritis and primary amyloidosis.

The metabolism and urinary excretion of 125I antithrombin III (AT III) was investigated in 2 patients with a nephrotic syndrome caused by minimal chain nephritis and primary amyloidosis, and acquired deficiency of AT III. Increased AT III catabolism was observed in both patients, even after correction for urinary protein loss. Increased AT III catabolism was due to increased influx from the extra- to the intravascular compartment in 1 patient, and to an increased fractional catabolic rate in the other patient who developed later a pulmonary embolism. Analysis of urine samples revealed biologic

Gene editing of human iPSCs rescues thrombophilia in hereditary antithrombin deficiency in mice.

Hereditary antithrombin deficiency is caused by SERPINC1 gene mutations and predisposes to recurrent venous thromboembolism that can be life-threatening. Therefore, lifelong anticoagulation is required, which has side effects and may not be effective. In this study, peripheral blood mononuclear cells from a patient with severe antithrombin deficiency were reprogrammed into induced pluripotent stem cells (iPSCs). The mutation was corrected using CRISPR-Cas9 and Cre/LoxP genome editing. iPSCs were differentiated into hepatocytes, which were injected into the spleen of antithrombin knockout mice

The surgical implications of primary hypercoagulable states.

Primary hypercoagulable states are hereditary disorders that result in arterial and venous thromboses. The purpose of this report is to present three patients with hypercoagulable states, and offer current guidelines for diagnosis and treatment. Primary hypercoagulable disorders such as antithrombin III, protein C and protein S deficiencies, fibrinolytic disorders such as decreased plasminogen levels and plasminogen activator deficiency, and antiphospholipid syndromes such as anticariolipin antibody and lupus anticoagulants will be reviewed. We will emphasize clinical characteristics that shou

How we treat severe inherited antithrombin deficiency: lessons from cases homozygous for the Budapest 3 variant.

BACKGROUND: Antithrombin deficiency represents one of the most severe inherited thrombophilias. Albeit a rare disorder, available knowledge suggests that antithrombin deficiency is underestimated due to the limitations of current diagnostic algorithms. The high clinical variability of this patient population may be another cause of underdiagnosis. Heterozygous type I (quantitative) variants are normally associated with a severe thrombophilic phenotype, while heterozygous type II (qualitative) variants are heterogeneous, including heparin-binding site defects, which are mild/moderate and the mo

Role of antithrombin concentrate in treatment of hereditary antithrombin deficiency. An update.

Antithrombin (AT) functions as a potent natural anticoagulant and serine protease inhibitor that inactivates many enzymes in the coagulation cascade. Antithrombin also possesses antiinflammatory properties, many of which are mediated by its actions as an anticoagulant. Hereditary AT deficiency is a rare, underrecognised medical condition that is associated with inadequate endogenous anticoagulation thought to result from impaired inhibition of serine protease coagulation factors. Inherited as an autosomal dominant trait, congenital AT deficiency typically reduces functional AT levels to 40-60%

[Antithrombin III substitution in patients in shock].

In a trial involving 61 patients suffering from shock and DIC, the relations between the concentration of heparin, the effect of heparin on coagulation and the activity of AT III were studied. The effect of heparin decreased to one-half when the activity of AT III was 75 per cent of the average normal level. At 50 per cent AT III, the effect of heparin was negligible. When AT III was substituted the disappearance rate of the AT III activity from circulation was considerably increased in the case of acute DIC, while the concentration of AT III when measured immunologically, did not show an incr

A survey of inherited thrombotic syndromes in Italy. ad hoc Study Group.

A collaborative survey was conducted among Italian thrombosis centers to gather information about the number and clinical features of patients with inherited thrombotic syndromes. The survey, based on 74 unrelated kindreds, revealed that antithrombin III, protein C and protein S defects are the most frequent genetic disorders. Venous thromboembolism was more frequent than arterial thrombosis, which was seen in only a minority of cases, most frequently with dysfibrinogenemia. About half of the patients developed venous thrombosis with a similar incidence in antithrombin III, protein S and prote

Hereditary thrombophilia as a cause of Budd-Chiari syndrome: a study from Western India.

The inherited deficiencies of protein C, protein S, antithrombin III, factor V Leiden mutation, prothrombin gene polymorphism, and antiphospholipids were studied in 53 Budd-Chiari syndrome (BCS) and 33 portal vein thrombosis (PVT) cases and compared with 223 age- and sex-matched controls. Protein C deficiency was detected in 7 (13.2%), protein S in 3 (5.7%), and antithrombin III in 2 (3.8%) of the BCS cases. Factor V Leiden was the most common risk factor, i.e., 14 of 53 (26.4%) in BCS cases followed by protein C, as compared with PVT cases, i.e., 2 of 33 (6.06%) and controls, i.e., 5 of 223 (

Simple method for preparation of cryoprecipitate (CP) and cryodepleted plasma (CDP).

Cryoprecipitates (CP) and cryodepleted plasma (CDP) were prepared from fresh frozen plasma (FFP). Plasma was easily and cleanly frozen at -50 degrees C using a methanol-bath Ultracryostat, which has been available commercially for the past few years. From a random sample (n = 6), factors VIII:C, IX:C, V:C, fibrinogen, antithrombin III and fibronectin were determined. Concerning the total contents and the in-vitro-recovery of factor VIII:C (x:104 IU/53.5%) and fibrinogen x:175 mg/36.9%), the preparation method was as efficient as other equally common methods. Apart from the well-known applicati
