Older Adults' Knowledge of Anxiety Disorders.

OBJECTIVES: The underuse of mental health services for anxiety disorders is particularly pronounced among older adults and could, in part, be explained by a lack of knowledge on this subject in this population. The purpose of this study was to document senior's knowledge of anxiety. METHOD: Young adults (n = 64) and older adults (n = 78) completed a sociodemographic questionnaire (including clinical variables), the Beck Anxiety Inventory and a questionnaire measuring their knowledge of the various dimensions of anxiety disorders, including symptoms, risk factors, and treatments. RESULTS: Res

[Current nosographic classifications of anxiety disorders].

CURRENT NOSOGRAPHIC CLASSIFICATIONS TOF ANXIETY DISORDERS. Anxiety disorders are psychiatric pathologies covering a wide range of clinical entities that are characterized by a pathological anxiety leading to severe emotional distress and marked impairment in daily functioning. They primarily differentiate from each other according to the general content of concerns, and originally included the panic disorder with or without agoraphobia, specific phobias, social anxiety disorder, generalized anxiety disorder, obsessive-compulsive disorder and post-traumatic stress disorder. However, the 5th edi

[Virtual reality therapy in anxiety disorders].

During the last decade a number of studies have been conducted in order to examine if virtual reality exposure therapy can be an alternative form of therapy for the treatment of mental disorders and particularly for the treatment of anxiety disorders. Imaginal exposure therapy, which is one of the components of Cognitive Behavioral Therapy, cannot be easily applied to all patients and in cases like those virtual reality can be used as an alternative or a supportive psychotherapeutic technique. Most studies using virtual reality have focused on anxiety disorders, mainly in specific phobias, but

Neurostructural abnormalities in pediatric anxiety disorders.

Functional neuroimaging studies have consistently demonstrated abnormalities in fear and threat processing systems in youth with anxiety disorders; however, the structural neuroanatomy of these systems in children and adolescents remains largely unknown. Using voxel-based morphometry (VBM), gray matter volumes were compared between 38 medication-free patients with anxiety disorders (generalized anxiety disorder; social phobia; separation anxiety disorder, mean age: 14.4±3 years) and 27 comparison subjects (mean age: 14.8±4 years). Compared to healthy subjects, youth with anxiety disorders had

Exercise for anxiety disorders: systematic review.

BACKGROUND: Anxiety disorders are commonly treated with antidepressants and psychological treatments. Some patients may prefer alternative approaches such as exercise. OBJECTIVE: To investigate the treatment effects of exercise compared with other treatments for anxiety disorders. DATA SOURCES: Randomised controlled trials (RCTs) of exercise interventions for anxiety disorders were identified by searching six online databases (July 2011). A number of journals were also hand searched. MAIN RESULTS: Eight RCTs were included. For panic disorder: exercise appears to reduce anxiety symptoms but

Diffusion tensor imaging in anxiety disorders.

Diffusion tensor imaging (DTI) can be used to examine the structural integrity of regional white matter and to map white matter tracts. DTI studies have been performed in several psychiatric disorders, especially in those for which a developmental or a neuropsychiatric component was postulated. Thus far, the use of DTI has been very limited in panic disorder, social anxiety disorder, and generalized anxiety disorder, and somewhat more extensive in post-traumatic stress disorder and obsessive-compulsive disorder. In most anxiety disorders, the results of DTI studies are in line with other struc

Mitral valve prolapse and anxiety disorders.

We investigated whether there is an association between anxiety disorders and mitral valve prolapse. We compared mitral valve prolapse prevalence in individuals with panic disorder (n = 41), social anxiety disorder (n = 89) and in healthy controls (n = 102) in an attempt to overcome the biases of previous studies. Our results show no associations between panic disorder or social anxiety disorder and mitral valve prolapse, regardless of the diagnostic criteria employed, and that the relationship between these conditions seems not to be clinically relevant.

Anxiety disorders in children and adolescents.

Anxiety disorders are among the most common and functionally impairing mental health disorders to occur in childhood and adolescence. Primary care providers can expect to treat youth who have anxiety disorders frequently, and this article aims to provide the tools necessary to evaluate and manage patients who present with anxiety symptoms during childhood or adolescence. This article discusses the epidemiology of anxiety disorders, including the increased risk of future anxiety disorders and other mental health problems that are associated with having an anxiety disorder in childhood and adole

Cognitive coping and childhood anxiety disorders.

To investigate differences in cognitive coping strategies between anxiety-disordered and non-anxious 9-11-year-old children. Additionally, differences in cognitive coping between specific anxiety disorders were examined. A clinical sample of 131 anxiety-disordered children and a general population sample of 452 non-anxious children were gathered. All children filled out the child version of the Cognitive Emotion Regulation Questionnaire (CERQ-k). Structured clinical interviews were used to assess childhood anxiety disorders. Results showed that anxiety-disordered children experience significan

Screening for anxiety disorders in children.

Anxiety disorders are highly prevalent and have negative consequences on individual and societal level. This study examined the usefulness of screening for anxiety disorders in primary school children. More specifically, the value of the screening method to discriminate between and to predict anxiety disorders was studied. Children and their parents were selected if the children had self-reported scores on the screening questionnaire Screen for Child Anxiety Related Emotional Disorders-71 (SCARED-71) within the top-15% (High-anxious) or from two points below to two points above the median (Med

Comorbidity of pain and anxiety disorders.

The comorbidity of anxiety disorders with pain has received little research attention even though recent studies show that these disorders are as likely to co-occur with chronic pain conditions as depressive disorder. Comorbid anxiety has implications for the impact and outcome of pain conditions. Even though it may be intuitively plausible to suppose that the anxiety occurs in the context of a preexisting pain condition, there is evidence for a reverse causal link and shared risk factors, including distal events occurring in childhood.

[Psychotherapy and pharmacotherapy of anxiety disorders].

Anxiety disorders are highly prevalent in the general population, their course of illness is often chronic with many relapses, and is associated with a pronounced psychiatric comorbidity and psychosocial disability. Therefore, anxiety disorders are a major challenge to any mental health delivery system. From a perspective of treatment both differential psychopharmacological and psychotherapeutic interventions can be outlined which are well based in empirical terms. In psychiatric care biological and psychological approaches are often combined although the empirical basement of combined treatme

Toddler anxiety disorders: a pilot study.

OBJECTIVE: This research examined the validity of criteria for diagnosing social phobia (SOC) and generalized anxiety disorder (GAD), where the DSM-IV criteria were modified to better identify toddlers who could have these disorders. METHOD: Diagnoses were made with a semistructured clinical interview that included child observations. Parents and caregivers completed child behavior, temperament, and socioemotional functioning questionnaires to test convergent and discriminant validity. RESULTS: Of 72 children, 18 months to 5 years old, 19 met modified SOC criteria (8 met DSM-IV criteria SOC

[Usefulness of antidepressants in anxiety disorders].

This article deals with four anxiety disorders particularly frequent in the population as well as in general practice: panic disorder, specific phobias, social phobia and generalized anxiety. After a brief description of their clinical characteristics and prevalence, it provides the general principles of treatment and mainly the specific role of antidepressants. Finally, for each disorder, a treatment algorithm based on available guidelines is proposed. Schematically, antidepressants and particularly selective serotonin reuptake inhibitors exhibit a usefulness assessed as important in panic di

Anxiety disorders: clinical presentation and epidemiology.

This chapter gives an overview of the clinical presentation of anxiety disorders and reviews basic epidemiological knowledge on them. The presented knowledge is largely related to the classification of anxiety disorders as presented by the Diagnostic and Statistical Manual of Mental Disorders since itsthird revision (DSM-III). Without going into detail into the history of the classification of anxiety disorders and into the history and development of the several editions of the Diagnostic Manual of Mental Disorders (DSM) of the American Psychiatric Association (APA) it should just briefly be m

[Anxiety disorders in children and adolescents].

Anxiety disorders are among the most common psychiatric disorders in children and adolescents with estimated prevalence rates of between five and ten percent. Comorbidity rates are high--in particular for further anxiety and depressive disorders. The etiology is considered to be multifactorial, with genetic, biological and temperamental factors as well as stressful life events, parental role models and parenting all playing a part. Therapeutic programs combine psychological counseling--including the family--psychotherapy and psychopharmacotherapy, as required. As psychotherapy, cognitive-behav

Predictors of response in anxiety disorders.

Anxiety disorder variables such as duration, severity of illness, and comorbidity with other anxiety or mood disorders appear to identify individuals who are at the greatest risk of treatment nonresponse. Conversely, in accord with clinical experience, shorter periods of illness, less severe illness, being treatment naive, and the absence of comorbidity tend to identify patients who are likely to respond robustly to medication management. Symptom clusters in OCD and PTSD are promising as a means of stratifying those more likely to respond to standard pharmacologic treatment. The presence of ho

Treatment of anxiety disorders to remission.

Treating anxiety disorders to remission should be the goal of all practitioners. A remitted patient should be well, both in symptoms and function, and be indistinguishable from a never-ill counterpart. The definition of remission in patients with anxiety disorders should also be clear, practical, and easy to use. It is useful to measure response in an objective way, such as with standardized instruments appropriate for the disorder, and to develop remission criteria specific to each disorder. This article proposes remission criteria, using standardized measures, for 5 common anxiety disorders:

Comorbidity of mood and anxiety disorders.

This article reviews data on the prevalence of panic, social phobia, generalized anxiety, and posttraumatic stress disorder, and research documenting the comorbidity of these disorders with major depression (MDD). These anxiety disorders are frequently comorbid with MDD, and 50-60% of individuals with MDD report a lifetime history of one or more of these anxiety disorders. The anxiety disorders are also highly correlated with one another, and approximately one-quarter to one-half of individuals with each of the anxiety disorders report a lifetime history of an alcohol or substance use disorder
