Prevalence of amyloid-β pathology in distinct variants of primary progressive aphasia.

OBJECTIVE: To estimate the prevalence of amyloid positivity, defined by positron emission tomography (PET)/cerebrospinal fluid (CSF) biomarkers and/or neuropathological examination, in primary progressive aphasia (PPA) variants. METHODS: We conducted a meta-analysis with individual participant data from 1,251 patients diagnosed with PPA (including logopenic [lvPPA, n = 443], nonfluent [nfvPPA, n = 333], semantic [svPPA, n = 401], and mixed/unclassifiable [n = 74] variants of PPA) from 36 centers, with a measure of amyloid-β pathology (CSF [n = 600], PET [n = 366], and/or autopsy [n = 378]) av

Quantitative classification of primary progressive aphasia at early and mild impairment stages.

The characteristics of early and mild disease in primary progressive aphasia are poorly understood. This report is based on 25 patients with aphasia quotients >85%, 13 of whom were within 2 years of symptom onset. Word-finding and spelling deficits were the most frequent initial signs. Diagnostic imaging was frequently negative and initial consultations seldom reached a correct diagnosis. Functionality was preserved, so that the patients fit current criteria for single-domain mild cognitive impairment. One goal was to determine whether recently published classification guidelines could be impl

Creutzfeldt-Jakob disease revealed by a logopenic variant of primary progressive aphasia.

Logopenic aphasia, mainly characterized by word anomia, sentence and phrase comprehension difficulties secondary to phonological loop deficits but relatively preserved single word comprehension and no agrammatism, is one of the 3 main variants of primary progressive aphasia (PPA). We describe the first case of PPA that fulfilled clinical criteria of logopenic aphasia but showed abnormal DWI hyperintensities that were predominant on the left hemisphere and compatible with Creutzfeldt-Jakob disease (CJD). After abnormally long isolated language deficits, the patient rapidly worsened and died. Au

Progression of logopenic variant primary progressive aphasia to apraxia and semantic memory deficits.

BACKGROUND: Due to the nature of neurodegenerative disorders, patients with primary progressive aphasia develop cognitive impairment other than aphasia as the disorder progresses. The progression of logopenic variant primary progressive aphasia (lvPPA), however, has not been well described. In particular, praxic disorders and semantic memory deficits have rarely been reported. CASE PRESENTATIONS: We report three patients in the initial stage of lvPPA who subsequently developed apraxia in the middle stage and developed clinically evident semantic memory deficits in the advanced stages. Pick Complex: an integrative approach to frontotemporal dementia: primary progressive aphasia, corticobasal degeneration, and progressive supranuclear palsy.

BACKGROUND: Frontotemporal dementia (FTD) is a new label for clinical Pick's disease (PiD) because the eponymic term is increasingly restricted to the pathologic finding of Pick bodies. This restriction created the impression that PiD is rare and that is it difficult to diagnose. FTD is also a term most often used for behavioral and personality alterations. Primary progressive aphasia (PPA) and corticobasal degeneration (CBD), formerly the extrapyramidal variety of PiD, are also part of the syndrome. Recently, chromosome 17 localization and tau mutations were discovered in familial forms of th

Neurofilament as a blood marker for diagnosis and monitoring of primary progressive aphasias.

OBJECTIVE: To assess the utility of serum neurofilament for diagnosis and monitoring of primary progressive aphasia (PPA) variants. METHODS: We investigated neurofilament light chain (NF-L) levels in blood of 99 patients with PPA (40 with nonfluent variant PPA [nfvPPA], 38 with semantic variant PPA [svPPA], 21 with logopenic variant PPA [lvPPA]) and compared diagnostic performance with that reached by CSF NF-L, phosphorylated neurofilament heavy chain (pNF-H), β-amyloid (Aβ1-42), tau, and phosphorylated tau. The longitudinal change of blood NF-L levels was measured and analyzed for correlatio

The Wernicke conundrum and the anatomy of language comprehension in primary progressive aphasia.

Wernicke's aphasia is characterized by severe word and sentence comprehension impairments. The location of the underlying lesion site, known as Wernicke's area, remains controversial. Questions related to this controversy were addressed in 72 patients with primary progressive aphasia who collectively displayed a wide spectrum of cortical atrophy sites and language impairment patterns. Clinico-anatomical correlations were explored at the individual and group levels. These analyses showed that neuronal loss in temporoparietal areas, traditionally included within Wernicke's area, leave single wor

Memory and orientation in the logopenic and nonfluent subtypes of primary progressive aphasia.

Memory and orientation were investigated as predictors of underlying Alzheimer's disease (AD) pathology in patients with logopenic (lv) and non-fluent (na) variants of primary progressive aphasia (PPA). Memory and orientation scores from Addenbrooke's Cognitive Examination were compared between 26 lv-PPA, 29 na-PPA, 59 AD, and 90 controls using analysis of variance. Forty-five patients underwent Pittsburgh compound B (PiB) positron emission tomography scans. Patients with lv-PPA performed poorer on memory and orientation than na-PPA and did not differ from the AD group. Post-hoc analysis on th

[What are important goals in the treatment of people with primary progressive aphasia?].

BACKGROUND: Primary progressive aphasia (PPA) is a neurodegenerative disease that leads to progressive impairment of language and communication, with serious consequences for the daily lives of affected patients. OBJECTIVE: As part of an international research collaboration (https://www.comet-initiative.org/Studies/Details/1871), important endpoints for investigating the success of (speech and language) therapy for people with PPA were developed (Core Outcome Set for PPA, COS-PPA). Here we report the results from Germany, discuss them in the context of the overall project, and compare them wi

A Case of Logopenic Variant of Primary Progressive Aphasia with Parkinsonism and Anosmia.

A 69-year-old right-handed woman developed difficulty naming objects and word-finding. The clinical features of language disorder and predominant atrophy on MRI and predominant hypoperfusion on 123I-IMP SPECT in the left temporo-parietal junction area were consistent with the diagnostic criteria for the logopenic variant of primary progressive aphasia (lvPPA). Neurological examination showed slight right-side rigidity and resting tremor (UPDRS-III: 4). 123I-FP-CIT SPECT showed presynaptic dopamine transporter reduction in the posterior putamina with left-side predominance. The odor-stick ident

Parkinsonism is associated with altered primary motor cortex plasticity in frontotemporal dementia-primary progressive aphasia variant.

In frontotemporal dementia (FTD), the behavioral variant (bv-FTD) and nonfluent variant of primary progressive aphasia (nfv-PPA) reflect a prominent neurodegenerative involvement of the frontal lobe networks, which may include the premotor and motor areas and thus cause heterogeneous clinical symptoms including parkinsonism. With the technique of transcranial magnetic stimulation, we investigated long-term potentiation- and long-term depression-like plasticity in the primary motor cortex of bv-FTD and nfv-PPA patients, with and without parkinsonism, by using the theta-burst stimulation (TBS) p

Primary Tauopathy With Logopenic/Semantic Mixed Progressive Aphasia and Frontotemporal Dementia-like Behavior.

Here we present the cases of two individuals with language and behavioral symptoms indicative of the early clinical manifestations of both Alzheimer disease (AD) and frontotemporal lobar degeneration (FTLD). Phonological language symptoms similar to those evident in logopenic variant primary progressive aphasia suggested AD pathology, while semantic impairment and behavioral changes (ie, abnormal eating behavior and disinhibition) suggested a diagnosis of FTLD. Multimodal neuroimaging studies revealed underlying neuropathology indicative of primary tauopathy with presumable 3/4-repeat isoform,

Brain Perfusion, Atrophy, and Dopaminergic Changes in Amyloid Negative Logopenic Primary Progressive Aphasia.

Although most cases of logopenic variant primary progressive aphasia (lvPPA) are caused by Alzheimer's disease (AD), Lewy body disease (LBD) has also been reported. We assessed brain perfusion, atrophy, dopamine transporter (DAT) uptake, and language function among patients with lvPPA based on beta-amyloid. Thirty-three patients with lvPPA and 28 healthy controls (HCs) underwent MRI, 18F-florbetaben PET, and early- and late-phase DAT PET. All patients completed a language test. General linear models were applied to investigate the association of brain imaging with the aphasia quotient (AQ) and

Cortical and behavioral correlates of alexia in primary progressive aphasia and Alzheimer's disease.

The underlying causes of reading impairment in neurodegenerative disease are not well understood. The current study seeks to determine the causes of surface alexia and phonological alexia in primary progressive aphasia (PPA) and typical (amnestic) Alzheimer's disease (AD). Participants included 24 with the logopenic variant (lvPPA), 17 with the nonfluent/agrammatic variant (nfvPPA), 12 with the semantic variant (svPPA), 19 with unclassifiable PPA (uPPA), and 16 with AD. Measures of Surface Alexia and Phonological Alexia were computed by subtracting control-condition word reading accuracy from

Picture Description and Functional Communication Rating Correlates in Variants of Primary Progressive Aphasia.

BACKGROUND: Generalization of speech-language pathology treatment is an important goal in clinical practice and research. Functional communication rating scales are often used to investigate potential treatment effects in daily life. Connected speech samples more closely reflect meaningful changes in communication as a result of treatment than test/re-test outcomes. Little attention has been directed to this relationship in aphasia due to neurodegenerative etiologies. AIMS: We investigated whether correlations between ratings on the Georgetown University Center for Aphasia Research and Rehabi

Applicability and Correlates of a Symptom-Led Staging System for Primary Progressive Aphasia.

BACKGROUND: This study aimed at assessing the applicability of a symptom-led staging system for primary progressive aphasia (PPA) based on retrospective medical records, as well as at exploring their demographic and clinical correlates. METHODS: 75 PPA patients (10 semantic, 28 non-fluent, 22 logopenic, and 16 mixed variants) were retrospectively staged according to the PPA Progression Planning Aid (PPA-Squared) system, which stages the disease by accounting for clinical features along three axes: (1) Communication; (2) Non-Verbal Thinking and Personality; (3) Personal Care and Well-Being. Effects of Cerebellar Transcranial Direct Current Stimulation in Bilingual Logopenic Primary Progressive Aphasia.

BACKGROUND: Primary progressive aphasia (PPA) is a language-based dementia, causing progressive decline of language functions. Transcranial direct current stimulation (tDCS) can augment effects of speech-and language therapy (SLT). However, this has not been investigated in bilingual patients with PPA. OBJECTIVE: We evaluated the case of Mr. G., a French (native language, L1)/Dutch (second language, L2)-speaking 59-year-old male, with logopenic PPA, associated with Alzheimer's disease pathology. We aimed to characterize his patterns of language decline and evaluate the effects of tDCS applied
