BACKGROUND CONTEXT: Muscle tissue is known to remodel in response to changes to its mechanical environment. Alterations in passive mechanical properties of muscles can influence spine stiffness and stability.

With the help of thick, transparent finger cross sections, it was shown that fibers of the extensor aponeurosis insert distal to the avulsed fragment of the distal phalanx. The consequence for hand surgery is not to expose the fragment through a dorsal incision, but through a mid-lateral one. In this way, the distant parts of the aponeurosis distal to the fracture remain intact, and there is no break in the continuation of the dorsal extension plate.

Acquired ptosis has previously been felt to be due to a dehiscence or disinsertion of the levator aponeurosis. In the early years of levator surgery, the aponeurosis was often exposed by using blunt dissection. We have found that with a change in surgical technique, which eliminates blunt dissection, the incidence of aponeurotic defects decreases dramatically. Dehiscence and disinsertion may therefore be caused by traumatic surgical dissection of an attenuated aponeurosis.
