Distribution, diversity, and pathogenicity of Oomycete root pathogens affecting Fraser fir (Abies fraseri) Christmas tree production in the Southern Appalachian Mountains.

Fraser fir (Abies fraseri) is the most economically important Christmas tree species in the Southern Appalachian Mountains, especially in North Carolina. However, its production is increasingly threatened by Phytophthora root rot (PRR), caused primarily by Phytophthora cinnamomi and several other Phytophthora species, resulting in significant losses for growers annually. This study surveyed symptomatic Fraser firs across the region and recovered 151 Phytophthora isolates. Six species were identified including: P. cinnamomi (88.7%), P. cryptogea (6%), P. europaea (2%), P. cactorum (1.3%), P. ci

Seroprevalence of Strongyloides stercoralis Among Pediatric Solid Organ Transplant Candidates at a Tertiary Pediatric Medical Center in the United States.

BACKGROUND: SOT recipients with prior Strongyloides stercoralis infection risk hyperinfection post-transplant. Risk-based screening may miss at-risk candidates as endemicity expands beyond classic regions. METHODS: Retrospective study of SOT candidates < 26 years at Cincinnati Children's Hospital (2018-2024). Universal serology-based screening was implemented in 2023. Seroprevalence was estimated with exact 95% CIs; seropositive and seronegative patients were compared on demographic and exposure characteristics. RESULTS: 178 patients were screened. Seroprevalence was 3.4% (6/178; 95% CI 1.2%

Current Status of La Crosse Virus in North America and Potential for Future Spread.

La Crosse virus (LACV) is an arthropod-borne RNA virus with substantial potential for future spread in North America. La Crosse virus is responsible for La Crosse encephalitis, a leading cause of arboviral encephalitis in children in the United States. Primarily transmitted by Aedes triseriatus (Eastern treehole) mosquitos and amplified by small mammal hosts, LACV has caused infections throughout the upper Midwest and, more recently, the mid-Atlantic and southeastern United States. Notably, in recent years, infections have also been identified increasingly in the Appalachian region. Anthropoge

Global prevalence of strongyloidiasis: critical review with epidemiologic insights into the prevention of disseminated disease.

Opportunistic disseminated strongyloidiasis is an important cause of morbidity and mortality in immunocompromised patients. However, the worldwide prevalence of Strongyloides stercoralis is difficult to determine because adequate data are lacking. In this paper more than 100 epidemiologic studies reporting on the prevalence of S. stercoralis among various populations on five continents are critically reviewed. Analysis of this information indicates that the following groups of people may be at risk: residents of an emigrants from any developing country and southern, eastern, and central Europe

Fumarate Hydratase-Deficient Uterine Leiomyomas in Appalachian Women: A Case Series Highlighting Cancer Disparities in Underserved White Populations.

Uterine leiomyomas are highly prevalent, yet specific variants, such as fumarate hydratase (FH)-deficient leiomyomas, are rare and clinically significant due to their association with hereditary leiomyomatosis and renal cell carcinoma syndrome (HLRCC). Diagnosis of this subtype relies on identifying unique histopathological features and confirming loss of FH expression or positive S-(2-succino)-cysteine (2SC) staining. This diagnostic challenge is compounded in underserved regions like Appalachia, where health inequities and limited access to specialist care can hinder the management of heredi

Assessing the relationship between multimorbidity, psychosocial variables, and clinical outcomes among older adults living in rural Appalachia.

The objective of this study was to assess the relationship between multimorbidity, psychosocial variables, and clinical outcomes among older adults with type 2 diabetes (T2D) in rural Appalachia. Point-of-care clinical data (e.g. A1c, systolic blood pressure, diastolic blood pressure) and self-reported survey data, that includes psychosocial factors (e.g. distress, depressive symptoms, social support, perceived stress), chronic conditions, and sociodemographic variables were obtained from older adults living in rural Appalachia. Clinical data and psychosocial variables were treated as outcome

The CATCH My Breath Vaping Prevention Curriculum: An Evaluation of Impacts in Central Appalachian Middle and High Schools, 2019-2023.

INTRODUCTION/OBJECTIVES: Since 2015, the rise in e-cigarette use among youth has concerned public health authorities. After peaking in 2019, usage rates have declined but remain high. In 2023, 10% of high school and 4.6% of middle school students reported current use. This report describes the implementation and evaluation of a school-based e-cigarette prevention program, CATCH My Breath (CMB), in 8 central Appalachian counties. These counties have some of the highest rates of tobacco use, poor health, and premature death in the United States. METHODS: A total of 6217 students across 25 middl

Ensuring contact: calling rural Appalachian older adults during the COVID-19 epidemic.

INTRODUCTION: Older adults, especially those aged 85 years or older, remain at significantly higher risk for COVID-19. This group, along with those with pre-existing heart and lung disease and diabetes, have accounted for 80% of hospitalizations and an even higher percentage of COVID-19 related deaths in the USA. West Virginia, the only state in the USA located completely within Appalachia, has a higher percentage of elderly than all but two states in the nation. Rural seniors are hesitant to use hospital emergency departments and attend routine care visits for fear of exposure to the virus. R

Variable infection of stream salamanders in the southern Appalachians by the trematode Metagonimoides oregonensis (family: Heterophyidae).

Many factors contribute to parasites varying in host specificity and distribution among potential hosts. Metagonimoides oregonensis is a digenetic trematode that uses stream-dwelling plethodontid salamanders as second intermediate hosts in the Eastern US. We completed a field survey to identify which stream salamander species, at a regional level, are most likely to be important for transmission to raccoon definitive hosts. We surveyed six plethodontid species (N = 289 salamanders) from 23 Appalachian headwater sites in North Carolina: Desmognathus quadramaculatus (n = 69), Eurycea wilderae (n

Health effects of acid aerosols on North American children: air pollution exposures.

Air pollution measurements were conducted over a 1-year period in 24 North American communities participating in a respiratory health study. Ozone, particle strong acidity, sulfate, and mass (PM10 and PM2.1) were measured in all communities. In 20 of the communities, sulfur dioxide, ammonia, nitric acid, nitrous acid, and particulate nitrate were measured. The sampler was located centrally in the community whenever possible and samples were collected every other day. Concentrations of particle strong acidity, mass, sulfate, and ozone were highly correlated both in the region of the country def

"Built on Respect and Good Honest Communication:" A Study of Partnerships Between Mental Health Providers and Community Corrections.

The prevailing approach to managing persons with criminal histories involves community supervision professionals like probation and parole officers partnering with other mental health providers to address clients' needs. The relationships between individual professionals are seldom researched, though, and the current study aims to address this deficit in the empirical literature. This study utilized interviews about professionals' perceptions of their work experiences, analyzed open-endedly to identify major themes. Mental health providers' themes included appreciation and process of collabora

Risk factors for heroin use following release from jail or prison in adults in a Central Appalachian state between 2012-2017.

BACKGROUND: Corrections-involved adults with a history of opioid use disorder are at elevated risk of opioid overdose following release from correctional settings. Increased opioid prescribing restrictions and monitoring during a time when heroin is becoming cheaper and ubiquitous means that adults who misused prescription opioids prior to incarceration may be reentering communities at greater risk for heroin exposure and use. OBJECTIVES: Determine risk factors of post-release heroin use among a sample of adults who participated in corrections-based drug treatment in Kentucky released between

Evidence of Leptospiral Presence in the Cumberland Gap Region.

BACKGROUND: Leptospirosis is a widespread zoonotic disease that causes reproductive losses and/or hepatorenal failure in a number of animal species. Wild reservoirs of the disease, such as rodents, harbor the causative bacterium, Leptospira spp., in their kidneys and contaminate the environment by excreting infected urine. In this study, we tested small wild mammals, environmental water, and livestock in the Cumberland Gap region of southeastern Appalachia for the presence of pathogenic Leptospira or leptospiral antibodies. METHODS/RESULTS: Small wild mammals (n = 101) and environmental water

Peer mentor versus teacher delivery of a physical activity program on the effects of BMI and daily activity: protocol of a school-based group randomized controlled trial in Appalachia.

BACKGROUND: Rural Appalachian populations have poorer health and fewer positive health-related behaviors compared to other United States populations. Appalachians are the most sedentary U.S. population and teens are particularly sedentary. Obesity prevention through improving physical activity is a top priority in Rural Healthy People 2020. Obesity prevalence among Appalachian teens exceeds the national rates of 13.9% and has consistently been greater than 26%. Organized sports has not been effective at improving daily physical activity or health outcomes for Appalachian teens. The purpose of

Psychological health and academic success in rural Appalachian adolescents exposed to physical and sexual interpersonal violence.

Interpersonal violence (IPV) in adolescence is a serious public health concern and may have lasting behavioral effects and implications for adult relationships. Adolescents from 2 rural Appalachian high schools in 2011/2012 were surveyed using the Centers for Disease Control and Prevention Youth Risk Behavioral Surveillance (YRBS) survey, which assessed (a) physical IPV within a dating relationship, (b) sexual IPV (defined as forced sex/rape), and (c) those who experienced both. We present baseline rates of each form of IPV for these rural male and female adolescents and assessed the strength

The early-onset Alzheimer's disease MRI signature: a replication and extension analysis in early-stage AD.

Early-onset Alzheimer's disease (EOAD) is less investigated than the more common late-onset Alzheimer's disease (LOAD) despite its more aggressive course. A cortical signature of EOAD was recently proposed and may facilitate EOAD investigation. Here, we aimed to validate this proposed MRI biomarker of EOAD neurodegeneration in an Appalachian clinical cohort. We also compared differences in EOAD signature atrophy in participants with biomarker-positive EOAD, LOAD, early-onset non-AD pathologies, and cognitively normal individuals. Cortical thinning was reliably detected in eight of nine signatu

Evidence of canine intestinal parasites and associated fecal bacteria among urban and rural dog parks in central Appalachia U.S.

Public dog parks provide opportunities for exercise and socialization, benefiting both dogs and their owners. However, dog parks can serve as reservoirs for gastrointestinal (GI) parasitic helminths, posing risks to canine and public health. This study investigated the presence of three common GI parasites, hookworm (Ancylostoma spp.), roundworm (Toxocara canis), and whipworm (Trichuris vulpis), in abandoned fecal samples from nine dog parks in Central Appalachia, comparing rural and urban environments. We also examined the relationship between parasite presence and fecal bacterial community c

Family Caregivers' Management of Behavioral Expressions of Dementia.

BACKGROUND AND OBJECTIVES: Behavioral expressions of dementia are often stressful for family caregivers to manage as they strive to ensure their relative's needs are met. Guided by Lazarus and Folkman's Transactional Model of Stress and Coping, we identified specific behaviors that disrupt daily routines and challenge the achievement of caregiving goals, and the approaches and strategies caregivers employ to address them. RESEARCH DESIGN AND METHODS: We conducted semistructured interviews with 30 family caregivers in rural Appalachia caring for a relative living with dementia. Analysis involv

Associations among sexual attraction status, school belonging, and alcohol and marijuana use in rural high school students.

Analysis of data collected from 1725 9th graders from 25 rural high schools revealed that students reporting same-sex attraction or uncertainty about their attraction status also reported significantly lower GPAs, lower school belonging, and higher marijuana and alcohol use. Regression analyses confirmed that beyond the effects of GPA and biological sex, sexual minority attraction status was negatively associated with school belonging. Further, while higher school belonging significantly decreased the odds of alcohol and marijuana use, sexual attraction status did not significantly contribute
