Arachnoid cyst of the posterior fossa.

Arachnoid cysts of the posterior fossa are uncommon. Our case of a 49-year-old man presented with a 2 month history of headaches, nausea, and vertigo associated with walking instability. An MRI revealed a median well-circumscribed cystic lesion of the posterior fossa, with similar signal characteristics to CSF, and without connection to the fourth ventricle. This aspect suggested either arachnoid or hydatid cysts. Direct open surgery was performed allowing complete removal of the cyst wall, with a good outcome.

Extradural arachnoid cysts in children.

BACKGROUND: Extradural arachnoid cysts in the spine are uncommon causes of spinal cord compression in the pediatric population that are thought to arise from congenital defects in the dura mater. Most reports describe such cysts as communicating with the intrathecal subarachnoid space through a small defect in the dura. Excision of the cyst with obliteration of the communicating dural defect is the mainstay of treatment in symptomatic patients. Solitary extradural arachnoid cysts have been reported in several studies, but multiple extradural arachnoid cysts are very rarely reported in children

Prenatal diagnosis of arachnoid cysts.

Arachnoid cysts are a rare central nervous system malformation, representing only 1% of all intracranial masses in newborns. Primary (congenital) arachnoid cysts are benign accumulation of clear fluid between the dura and the brain substance throughout the cerebrospinal axis in relation to the arachnoid membrane and do not communicate with the subarachnoid space. Secondary (acquired) arachnoid cysts result from hemorrhage, trauma, and infection and usually communicate with the subarachnoid space. The common locations of arachnoid cysts are the surface of the brain at the level of main brain fi

Recurrent intrasellar arachnoid cyst.

A 48-year-old man presented with complaints of decreased visual acuity persisting for 6 weeks in December 1997. Neurological examination demonstrated defects in the superior bitemporal visual field and bilateral optic atrophy. Computed tomography and magnetic resonance (MR) imaging showed an intrasellar cystic lesion. The cyst wall was excised via a transsphenoidal approach. The diagnosis was intrasellar arachnoid cyst. The sellar floor was reconstructed after packing fat in the sellar turcica. The visual complaint improved, but he was lost to follow up. Four years later, he was re-admitted co

Posterior fossa arachnoid cysts.

Arachnoid cysts of the posterior fossa are rare lesions that are considered to be mostly congenital in origin. In this article, we retrospectively review 12 patients who underwent surgical treatment for their symptomatic posterior fossa arachnoid cysts. The most common presenting symptoms were gait disturbances and headache. The diagnosis was established on computed tomography or magnetic resonance imaging. Surgery consisted of cyst wall excision with fenestration in nine cases and shunting procedures in three cases. In all cases except one who-died, the postsurgical follow-up neuroradiologica

[Arachnoid cyst of the sphenoid sinus].

BACKGROUND: Arachnoid cysts of the paranasal sinus are rare. They have not been described yet in the sphenoid sinus. PATIENT AND METHOD: Microscopic-endoscopic endonasal surgery of the sphenoid sinus was performed on a 34-year-old female with a history of chronic headaches and a suspected mucocele of the sphenoid sinus in computed tomography (CT) and magnetic resonance imaging (MRI) studies. An extended arachnoid cyst was found in the enlarged sphenoid sinus, which was obliterated with collagen, fibrin glue, and abdominal fat. RESULTS: There were no complications after the operation, and 12

Arachnoid cyst of the lateral ventricle.

BACKGROUND: Arachnoid cysts rarely occur within the lateral ventricles, with only 10 reported cases in the literature. They may arise from the arachnoid layer that is present in the choroidal fissure. CASE DESCRIPTION: This 25-year-old man with headache and a left jacksonian seizure had an intraventricular arachnoid cyst of the right occipital horn and trigone found on radiologic examination by computed tomography (CT) and magnetic resonance imaging (MRI). He was successfully treated by evacuation of the cyst and removal of its wall through a right parietooccipital craniotomy. Histologic stud

Neuroendoscopic approach to arachnoid cysts.

A prospective study of seven consecutive patients with congenital arachnoid cysts treated endoscopically is reported. The ages of the patients at the time of diagnosis ranged from 6 to 47 years with three patients under 15 years. Two cysts were located in the posterior cranial fossa, four in the middle cranial fossa, and one in the suprasellar-prepontine area. The patients' symptoms included headache, seizures, vomiting, nausea, dizziness, balance problems, and precocious puberty. The authors performed cystocisternostomies and ventriculocystostomies via burr holes with the aid of a universal n

Arachnoid cyst in the quadrigeminal cistern.

Two cases of arachnoid cysts in the quadrigeminal cistern are presented, and one occurred in an adult. Signs and symptoms of increased intracranial pressure were noted, and obstructive hydrocephalus was shown by computed tomography scans in both cases. Arachnoid cysts in the quadrigeminal cistern have rarely been reported, especially in adults. The definition, classification, and etiology of arachnoid cyst are still confused in the literature, and various terms have also been used to indicate this particular location of cyst. In this study, these confusing problems are reviewed, and the useful

Postoperative lumbar arachnoidal diverticula.

Five patients with arachnoidal diverticula after lumbar operations are reported. Myelography was performed in all cases, spinal computed tomography scanning in three; three patients underwent surgical treatment with good clinical results. Arachnoidal diverticula are a rare complication of operations on the lumbar spine. Spinal computed tomography scanning is very useful, especially in the investigation of large diverticula and in the radiologic visualization of the arachnoidal stalk. Surgical treatment, which is required in symptomatic cases, results in remission of pain and prevents neurologi

[Arachnoid cysts of the cerebellopontile angle].

Ten cases of arachnoid cysts of the ponto-cerebellar angle are presented. In most cases, local arachnoiditis is disclosed during surgery, directing the discussion toward acquired pathology, and perhaps toward local infection. The clinical symptoms are dominated by the cochleo-vestibular deficit, but the involvement of the V and the VII cranial nerves is inconstant and discreet. At the present time, C.T. scan allows a rapid diagnosis but the prognosis must be reserved, in account to the possibility of a local post operatory arachnoiditis .

[A case of intrasellar arachnoid cyst].

Intrasellar arachnoid cyst is very rare. We report a case of intrasellar arachnoid cyst and review 15 cases which were previously reported. A 58-year-old male was admitted for evaluation of visual disturbance on April 24, 1981. Neurological examination revealed bilateral decreased visual acuity, optic atrophy and bitemporal hemianopsia. X-ray film of the skull showed a balloon-shaped sella turcica with thinning of the cortex of the sella floor. Axial and coronal CT scan showed a cystic lesion with water-density occupying the intrasellar and suprasellar region. After administration of contrast

A Comprehensive Review of Arachnoid Cysts.

Arachnoid cysts are cerebrospinal fluid (CSF)-filled sacs that develop within the arachnoid membrane surrounding the brain or spinal cord, often remaining asymptomatic but occasionally causing neurological symptoms due to mass effect or CSF flow obstruction. The optimal management of these cysts remains debated, balancing conservative observation with surgical intervention. This review analyzes the epidemiology, pathophysiology, clinical presentation, diagnostic strategies, treatment options, and future management directions for arachnoid cysts. With a prevalence of approximately 1-2% and a ma

Arachnoid Cyst: An Asymptomatic Exuberance.

A 66-year-old man presented to the emergency department with sudden onset of dysarthria, left central facial palsy, and left hemihypesthesia involving the tongue. The patient was hemodynamically stable (blood pressure of 153/84 mmHg and heart rate of 80 bpm) and normoglycemic, without a history of trauma or toxic exposure. Assuming an acute stroke, the patient immediately underwent a brain CT scan that revealed a large left-sided fronto-parieto-temporal arachnoid cyst, with approximately 9.5 x 5.1 cm of maximum diameters (anteroposterior and transversal), compressing the brain parenchyma and t

Extradural Arachnoid Cyst Excision.

Arachnoid cysts are an uncommon postoperative complication and can result in back pain, radiculopathy, and/or cauda equina syndrome. For symptomatic postoperative arachnoid cysts, surgical management is the accepted treatment. While excision of arachnoid cysts is the preferred surgical method, cysts with adhesions to nerve roots and/or dura can be treated with cyst fenestration to reduce morbidity of excision. Surgeons should be prepared for dural defects after cyst excision, which can require dural grafting and lumbar drains for cerebrospinal fluid diversion.

Temporal arachnoid cysts: are they congenital?

CASE REPORT: The authors report two cases of arachnoid cysts (ACs) neither detected during pregnancy nor shortly after birth when newborns underwent CT scan evaluation after birth head trauma. ACs were diagnosed at 10 months and 6 years, respectively. The first one becomes symptomatic, and the other one was incidentally found during a head trauma investigation. DISCUSSION: These cases give support for the postnatal pathogenesis for some of the assumed congenital ACs. We collected data from the literature that supports the acquired hypothesis for ACs.

Asymptomatic giant arachnoid cyst.

A 51-year-old male bricklayer without cerebral symptoms underwent whole-body FDG PET for suspicion of mediastinal sarcoidosis. PET/CT incidentally demonstrated a voluminous right frontal arachnoid cyst with normal glucose metabolism in the adjacent cortex. MRI demonstrated compression of the ipsilateral cerebral parenchyma and precentral, angulate, callosal, and superior frontal sulci. Functional bilateral finger-thumb paradigm MRI revealed right hemispheric reorganization of cortical motor activation with prominent activation between the inferior frontal gyrus and the postcentral gyrus.

[Arachnoid cysts and epilepsy in children].

BACKGROUND: At neurodevelopment age arachnoid cysts (ac) are commonly found by accident in neuroimaging examinations performed in the process of differential diagnostics of different diseases, including epilepsy. They are neurodevelopmental disorders which usually develop in the fetal life. Ac are usually supratentorial, less often subtentorial and sporadically interstellar. They are often the only structural lesions of the brain found in the child with epilepsy. There is still lack of univocal statement whether presence of ac determines type of seizures and focal paroxysmal changes in EEG rec
