NonREM Disorders of Arousal and Related Parasomnias: an Updated Review.

Parasomnias are abnormal behaviors and/or experiences emanating from or associated with sleep typically manifesting as motor movements of varying semiology. We discuss mainly nonrapid eye movement sleep and related parasomnias in this article. Sleepwalking (SW), sleep terrors (ST), confusional arousals, and related disorders result from an incomplete dissociation of wakefulness from nonrapid eye movement (NREM) sleep. Conditions that provoke repeated cortical arousals, and/or promote sleep inertia, lead to NREM parasomnias by impairing normal arousal mechanisms. Changes in the cyclic alternati

Spontaneous and apnea arousals from sleep in preterm infants.

BACKGROUND: The significance of arousal in apnea termination in preterm infants is not known. METHODS: We investigated the appearance of arousals from sleep with polysomnography for 21 preterm infants at a median age of 36 gestational weeks. RESULTS: The polysomnographic appearance of sleep was fragmented by frequent arousals. The number of spontaneous arousals unrelated to apneas was 18 per hour in sleep; higher in rapid eye movement (REM) sleep than in non-REM sleep (p < 0.001). Eighty-two percent of arousals were regarded as spontaneous, and 18% were related to apneas. In turn, arousal fo

The Influence of Interactions with Dogs on Affect, Anxiety, and Arousal in Children.

Interactions with animals represent a promising way to reduce the burden of childhood mental illness on a large scale. However, the specific effects of child-animal interactions are not yet well-established. This study provides a carefully controlled demonstration that unstructured interactions with dogs can improve clinically relevant symptoms in children. Seventy-eight children (55.1% female, 44.9% male) ages 10 to 13 (M = 12.01, SD = 1.13) completed the Trier Social Stress Test for Children, followed by (a) interaction with a dog, (b) a tactile-stimulation control condition, or (c) a waitin

Thalamic strokes that severely impair arousal extend into the brainstem.

In this study, we evaluate the role of the thalamus in the neural circuitry of arousal. Level of consciousness within the first 12 hours of a thalamic stroke is assessed with lesion symptom mapping. Impaired arousal correlates with lesions in the paramedian posterior thalamus near the centromedian and parafascicular nuclei, posterior hypothalamus, and midbrain tegmentum. All patients with severely impaired arousal (coma, stupor) had lesion extension into the midbrain and/or pontine tegmentum, whereas purely thalamic lesions did not severely impair arousal. These results are consistent with gro

Neural Representation of Subjective Sexual Arousal in Men and Women.

INTRODUCTION: Studies investigating brain indices of sexual arousal have begun to elucidate the brain's role in processing subjective arousal; however, most research has focused on men, used discrete ratings of subjective arousal, and used stimuli too short to induce significant arousal in women. AIM: To examine brain regions modulated by changes in subjective sexual arousal (SSA) rating intensity in men and women. METHODS: Two groups (20 men, 20 women) viewed movie clips (erotic or humorous) while continuously evaluating changes in their SSA using a Likert-like scale (0 = not aroused, 10 =

Bidirectional interactions between the baroreceptor reflex and arousal: an update.

Studies involving genetic engineering on animal models and mathematical analysis of cardiovascular signals on humans are shedding new light on the interactions between the arterial baroreceptor reflex (baroreflex) and arousal. Baroreceptor stimulation, if very mild or performed under anaesthesia, may inhibit cortical arousal. However, substantial increases or decreases in baroreflex activation cause arousal in animal models and human subjects in physiological conditions. On the other hand, cardiovascular changes during autonomic arousals and between the states of wakefulness and sleep involve

A periclitoral mass as a cause of persistent genital arousal disorder.

INTRODUCTION: Persistent genital arousal disorder (PGAD) is an intrusive and unremitting disorder for which several possible etiologies and treatments have been suggested. AIM: To describe a woman who developed PGAD in association with a periclitoral mass, a potential physical cause of the disorder that has not been previously described in the medical literature. METHODS: A postmenopausal woman presented with 6 months of persistent, unrelenting genital arousal and clitoral pain that was unrelated to sexual stimuli. Careful examination revealed a tender, firm, mobile, left-sided mass that app

A psychophysiological investigation of sexual arousal in women with lifelong vaginismus.

INTRODUCTION: Relatively little is known about the subjective reactions of women with lifelong vaginismus to erotic stimuli and genital arousal has never been investigated. Reports of maladaptive cognitions and fears regarding sexuality and intercourse suggest that anxiety may interfere with sexual arousal in women with vaginismus. AIMS: To examine the genital and subjective responses to sexually explicit film stimuli of women with lifelong vaginismus compared with women with lifelong dyspareunia and women with no pain. METHODS: Forty-five women (15 vaginismus, 15 dyspareunia, and 15 no pain

Carotid plaque in Alzheimer caregivers and the role of sympathoadrenal arousal.

OBJECTIVES: To test the hypothesis that those who provide care for a spouse diagnosed with Alzheimer's disease would have increased prevalence of carotid artery plaque compared with noncaregiving controls and that prolonged sympathoadrenal arousal to acute stress would relate to this difference. Providing care for a spouse with Alzheimer's disease has been associated with an increased risk of coronary heart disease, potentially due to the impact of caregiving stress on the atherosclerotic disease process. METHODS: Participants were 111 spousal caregivers (74 ± 8 years of age; 69% women) to pa

The impact of rumination on aggressive thoughts, feelings, arousal, and behaviour.

Although rumination following a provocation can increase aggression, no research has examined the processes responsible for this phenomenon. With predictions derived from the General Aggression Model, three experiments explored the impact of two types of post-provocation rumination on the processes whereby rumination augments aggression. In Experiment 1, relative to distraction, self-focused rumination uniquely increased the accessibility of arousal cognition, whereas provocation-focused rumination uniquely amplified the accessibility of aggressive action cognition. In Experiment 2, provocatio

Genito-sensory analysis in women with arousal and orgasmic dysfunction.

INTRODUCTION: Diagnosis and treatment of female sexual dysfunction (FSD) are currently based on subjective female reports and physical examination. The GenitoSensory Analyzer (GSA) is a quantitative sensory testing tool designed to quantify vibratory and thermal sensation in the vagina and clitoris in a reproducible manner, and may therefore contribute to the diagnosis and management of FSD. AIM: To address the question of whether women with arousal and/or orgasmic sexual disorders have genital sensory abnormalities as measured by the GSA. MAIN OUTCOME MEASURES: Thresholds for warm, cold, an

Clitoral sexual arousal: an immunocytochemical and innervation study of the clitoris.

OBJECTIVE: To further define neural pathways and mechanisms responsible for the arousing properties of the epithelium of the clitoris as well as related neural pathways associated with sexual arousal in a murine model. MATERIALS AND METHODS: Adult female C57B1/6 mice were used for gross dissection, facilitated by resin injection of the vascular system, and silver staining, and immunostaining for S-100 and neuronal nitric oxide synthase (nNOS). We also comprehensively reviewed relevant published clinical and histological material from both human and non-human vertebrate studies. RESULTS: The

Emotional arousal and memory: a test of the poststimulus processing hypothesis.

Emotional arousal is believed to enhance memory for details central to an episode but impair memory for peripheral details. However, new research suggests that arousal induced thematically (i.e., through involvement with an unfolding event) produces only memory enhancements. This article examines whether consciously controlled elaborative processing in the aftermath of an arousing experience is responsible. A dual task manipulation was used to prevent participants from ruminating over a video that depicted an abduction and attack. Several indices of recall showed greater memory for emotional e

Arousal modulation in females with fragile X or Turner syndrome.

The present study was carried out to examine physiological arousal modulation (heart activity and skin conductance, across baseline and cognitive tasks, in females with fragile X or Turner syndrome and a comparison group of females with neither syndrome. Relative to the comparison group, for whom a greater increase in skin conductance was associated with poor arithmetic performance and less risk taking behavior, females with fragile X displayed a minimal increase in heart activity that was nevertheless associated with poor performance on mental arithmetic. In contrast, no arousal-cognitive per

On a characteristic of cortical arousals in individuals with obstructive sleep apnea.

STUDY OBJECTIVE: To identify the frequency and duration of those arousals exceeding fifteen seconds in length, in patients with obstructive sleep apnea. METHODS: The polysomnographic tracings from fifty consecutive patients with obstructive sleep apnea were carefully analyzed to delineate the duration of each arousal event. These data were then collated to define the frequency of arousals of differing durations, and the impact thereof on the total time spent in an arousal state. RESULTS: Respiratory event related prolonged arousals (> or = 15 seconds) were identified in 100% of the patients.

Role of arginase in the male and female sexual arousal response.

The NO-cGMP pathway plays a key role in the male and female genital sexual arousal response. Nitric oxide synthase (NOS) utilizes L-arginine and oxygen as substrates to produce nitric oxide (NO) and citrulline. Arginase is a metalloenzyme that catalyzes the hydrolysis of L-arginine to produce L-ornithine and urea. It is proposed that arginase competes for L-arginine and reduces NOS activity in genital tissues, thus modulating sexual function. Using 2 transition state analogue inhibitors of arginase, 2(S)-Amino-6-boronohexanoic acid (ABH) and S-(2-boronoethyl)-L-cysteine (BEC), we have characte

Objective assessment of sexual arousal in women with a history of hysterectomy.

OBJECTIVE: The potential contribution of psychological and anatomical changes to sexual dysfunction following hysterectomy is not clear. Radical hysterectomy for cervical cancer causes surgical damage to the autonomic nerves which are responsible for the increased vaginal blood flow during sexual arousal. Simple hysterectomy causes more limited nerve disruption. Photoplethysmographic assessment of vaginal pulse amplitude objectively measures vaginal blood flow during sexual arousal. We hypothesised that damage of the autonomic nerves results in a disrupted vaginal blood flow response during se

Effects of prematurity on arousal from sleep in the newborn infant.

The incidence of sudden infant death syndrome has been found to be consistently higher in preterm and low birth weight infants than in infants born at term. Failure to arouse from sleep is one possible mechanism for sudden infant death syndrome. This study compared the arousal responses to nasal air-jet stimulation in a longitudinal study between groups of healthy preterm and term infants. Preterm infants (n = 9) were born at 31-35 wk gestation with normal birth weights for gestational age and studied on three occasions: a preterm study at 36 wk, at 2-3 wk post-term, and at 2-3 mo post-term. Influence of uvulopalatopharyngoplasty on alpha-EEG arousals in nonapnoeic snorers.

Arousals are more numerous in heavy snorers than in nonsnorers and might be a cause of excessive daytime sleepiness (EDS) in these patients. The present study investigated whether treatment of snoring by uvulopalatopharyngoplasty (UPPP) had an influence on sleep microstructure in nonapnoeic snorers. The polysomnographic records of 10 nonapnoeic snorers were reviewed retrospectively and arousals scored according to the American Sleep Disorders Association (ASDA) 3 s definition. Scores for snoring, EDS and polysomnographic data were compared before and after UPPP (mean (+/-SD) time interval 249
