BACKGROUND: Infective endocarditis (IE) in young adults can present with nonspecific symptoms mimicking alternative diagnoses.
CASE SUMMARY: A 29-year-old man presented to the emergency department with erythematous, pruritic papules on his shins and ankles with bilateral knee pain and swelling. After the initial work-up for septic or inflammatory arthritis, he was empirically treated for reactive arthritis with systemic steroids. A month later, he re-presented with headache, dark urine, and unintentional 40-lb weight loss. Imaging revealed multiple ischemic strokes in the bilateral cerebelli

Reactive arthritis (ReA) is a postinfectious inflammatory disorder that may be difficult to treat in refractory cases. We describe a 20-year-old man with HLA-B27-positive ReA presenting with arthritis, conjunctivitis, balanitis, and psoriasiform skin lesions. Conventional therapies, including antibiotics, nonsteroidal anti-inflammatory drugs, corticosteroids, and sulfasalazine, failed to control disease activity. Treatment with upadacitinib (15 mg/day), a selective Janus kinase 1 inhibitor (JAK1), led to rapid improvement of joint, ocular, and cutaneous manifestations within three weeks. Corti

Reactive arthritis, initially described at the beginning of the twentieth century, is characterized by a sterile articular inflammation occurring several days to weeks after a bacterial gastrointestinal or urogenital infection. The most common agents include Chlamydia trachomatis, Ureaplasma urealyticum, and Neisseria gonorrhea for the venereal type and Salmonella enteriditis, Shigella flexneri, Yersinia enterocolitica for the post-enteric type. The prevalence of reactive arthritis varies geographically but overall, worldwide, it is estimated as 1/1000 persons. It is more common in young adult

Low-quality evidence suggests that COVID-19 may trigger reactive arthritis one to four weeks after the infection. Post COVID-19 reactive arthritis resolves within a few days, and no additional treatment is required. Established diagnostic or classification criteria for reactive arthritis are missing, and a deeper understanding of the immune mechanism related to COVID-19 prompt us to further investigate the immunopathogenic mechanisms capable of promoting or contrasting the development of specific rheumatic diseases. Caution should be exerted when managing post-infectious COVID-19 patient with

OBJECTIVES: The aim of the study is to report a series of 17 cases of ankle bi-arthritis that occurred shortly after coronavirus disease 2019 RNA vaccination, and to discuss the potential role of these vaccines in the pathogenesis of this rheumatological manifestation.
METHODS: All patients were examined in the same department and received a full work-up to investigate the usual causes of ankle bi-arthritis. No rheumatic inflammatory disease occurred after 9 months of follow-up. A post-vaccination serological follow-up in search of anti-Spike antibodies was requested for all patients.

Reactive arthritis (ReA) is a sterile arthritis that occurs in genetically predisposed individuals secondary to an extra-articular infection, usually of the gastrointestinal or genitourinary tract. Sterile arthritis associated with instillation of intravesical bacillus Calmette-Guérin (iBCG) therapy used for bladder cancer can also be included under ReA based on the pathogenic mechanism. Similar to spondyloarthritis, HLA-B27 positivity is a known contributor to the genetic susceptibility underlying iBCG-associated ReA. Other genetic factors, such as HLA-B39 and HLA-B51, especially in Japanese

The introduction of the term reactive arthritis (ReA) for the joint inflammation observed after infection with Yersinia enterocolitica, in which "a causative pathogen cannot be isolated from the synovial fluid", and the association with the HLA-B27 were the historical milestones for a new classification and assignment to the spondylarthritides (SpA). The division into postinfectious and reactive arthritis proposed in 1976 was put into perspective in the 1990s because of investigations with the newly available molecular biological method of the polymerase chain reaction. Microbial products coul

A 43-year-old healthy male was diagnosed with symptomatic COVID-19. Soon after recovery, he experienced severe back pain, bilateral red eye, and a new penile lesion. He was diagnosed with reactive arthritis, given he presented with arthritis, conjunctivitis, and balanitis. The patient was treated with nonsteroidal anti-inflammatory drugs (NSAIDs), a short course of systemic steroids, and a local steroid cream on the penile lesion, followed by a local antifungal cream for two months. The patient responded well to the treatment and returned to his usual life activities.

Tunga penetrans is the smallest biting flea known. In cattle, infestation by T. penetrans (tungiasis) typically affects the skin of the distal legs, udder, prepuce, and perianal area. A detailed clinical and pathologic description of bovine tungiasis, together with electron microscopy and molecular diagnostics to establish the identity of the parasite are described. Ninety percent of the cows and heifers and 80% of the bulls in a herd in northwest Argentina had proliferative and ulcerative skin lesions affecting the coronary band, interdigital space, heels, and rudimentary toes of the fore and

Reactive arthritis is an inflammatory joint disease induced by a preceding, sometimes asymptomatic bacterial infection outside the joints. With an estimated prevalence of 40/100,000 inhabitants, the disease primarily affects adults between the ages of 20 and 40 years. The clinical presentation is typically characterized by monoarthritis to oligoarthritis of the lower extremities with possible accompanying enthesitis, bursitis, tenosynovitis, sacroiliitis, dactylitis and rare extra-articular manifestations. Because of the similar clinical symptoms and an association with HLA-B27, reactive arthr

PURPOSE OF REVIEW: The incidence of acute rheumatic fever (ARF) in the United States and Western Europe is decreasing and postStreptococcal reactive arthritis (PSRA) is more prevalent. It is not clear whether PSRA is a forme fruste of ARF or a separate disease entity. Therefore, this review explores similarities and dissimilarities in initial symptoms and signs, disease course and underlying pathophysiologic mechanisms.
RECENT FINDINGS: ARF and PSRA present differently. PSRA patients are generally older, have a longer interval between group A streptococcus infection and symptom onset, and res

Reactive arthritis (previously known as Reiters syndrome) is an inflammatory arthritis that is a type of spondyloarthropathy. The disease consists of the classical triad of arthritis, urethritis and conjunctivitis, but mucocutaneous abnormalities also frequently appear: balanitis circinata, keratoderma blennorrhagicum, aphthous ulcers in the mouth and nail disorders. These skin lesions are mainly found in reactive arthritis induced by Chlamydia trachomatis (Ct). Reactive arthritis is often triggered by a sexually transmitted infection (Chlamydia trachomatis) or an enteric infection (such as Sa

Intravesical instillation of bacillus Calmette-Guérin (BCG) is used in the treatment of patients with intermediate and high-risk superficial bladder carcinoma with efficacy and safety. The vast majority of patients do not present any side effects and only 5% of patients have mild and short-lived clinical manifestations such as malaise, low-grade fever, cystitis, and hematuria. Arthralgia and/or arthritis is one of the rare severe complications following intravesical BCG immunotherapy. We report here the case of a patient with reactive arthritis successfully treated with nonsteroidal anti-infla

OBJECTIVE: To study the incidence and clinical picture of Campylobacter-associated reactive arthritis (ReA) and other reactive musculoskeletal symptoms in the population.
METHODS: A questionnaire on enteric and extraintestinal, including specifically musculoskeletal, symptoms was sent to 870 consecutive patients with Campylobacter-positive stool culture and 1440 matched controls. Analysis of self-reported musculoskeletal symptoms with clinical examination was performed.
RESULTS: Forty-five of the patients (7%) had ReA and eight (1%) had reactive tendinitis, enthesopathy or bursitis. No child

Reactive arthritis (ReA) is definitely caused by an infection. Several observations suggest that the triggering microbe may persist in the tissues of the patient for a prolonged time. The obvious conclusion is to consider antibacterial treatment. In two instances antibacterial agents are of definite value: in the primary and secondary prevention of rheumatic fever and for early eradication of Borrelia burgdorferi in order to prevent development of the arthritis associated with Lyme disease. Altogether, clinical and experimental data exist to indicate that if antibacterial treatment of ReA can

In this review synonymous definitions of reactive arthritis are discussed first. Major clinical symptoms, their infectious etiology and epidemiology define post-dysenteric and post-veneral forms of reactive arthritis. Classical (smear, culture, biochemistry, antigen detection) and molecular (DNA and RNA detections) techniques are used in the routine microbial diagnosis that is retrospective in the majority of cases. In the pathomechanism of this disorder, HLA-B27 antigen positivity of patients is a frequent risk factor. Molecular mimicry between microbial and self-antigens, abnormal antigen pr
