22q11 deletion syndrome: is that what they used to call . . . ?

PURPOSE: 22q11 deletion syndrome (22q11DS) is a very common, but commonly overlooked, disorder that has many variable features, such as serious heart defects, speech and articulation problems, immune compromise, learning problems, pervasive developmental disorders, and a late appearing phenotype of this condition--mental illness. CONCLUSION: Persons with 22q11DS are likely to be medically complex patients with pervasive developmental disorders and late appearing mental problems. PRACTICE IMPLICATIONS: Establishing this diagnosis is useful to families and healthcare providers to anticipate an

Heterogeneity of short-term memory deficits in children with dyslexia.

Many studies have highlighted short-term memory (STM) impairment in dyslexic individuals. Several studies showed deficits for both item and serial order aspects of verbal STM in dyslexic individuals. These group-based studies, however, do not inform us about the prevalence of these deficits and, importantly, their potential heterogeneity at the individual level. The present study examined both group-level and individual STM profiles in dyslexic and age-matched non-dyslexic children. While confirming previous group-based results of both item and serial order STM deficits, individual analyses in

Intensive treatment of speech disorders in robin sequence: a case report.

PURPOSE: To describe the speech of a patient with Pierre Robin Sequence (PRS) and severe speech disorders before and after participating in an Intensive Speech Therapy Program (ISTP). METHODS: The ISTP consisted of two daily sessions of therapy over a 36-week period, resulting in a total of 360 therapy sessions. The sessions included the phases of establishment, generalization, and maintenance. A combination of strategies, such as modified contrast therapy and speech sound perception training, were used to elicit adequate place of articulation. The ISTP addressed correction of place of produc

Electropalatography in home training of retracted articulation in a Swedish child with cleft palate: effect on articulation pattern and speech.

The aim was to evaluate the effectiveness of electropalatography (EPG) in home training of persistent articulation errors in an 11-year-old Swedish girl born with isolated cleft palate. The /t/ and /s/ sounds were trained in a single subject design across behaviours during an eight month period using a portable training unit (PTU). Both EPG analysis and perceptual analysis showed an improvement in the production of /t/ and /s/ in words and sentences after therapy. Analysis of tongue-contact patterns showed that the participant had more normal articulatory patterns of /t/ and /s/ after just 2 m

The incidence of centric slides in healthy individuals and TMD patients.

Controversy exists as to whether centric slide is an etiological factor for temporomandibular disorders. In this cross-sectional study the magnitude of centric slides, obtained with 2 different articulations and one with a millimetre ruler was compared. The study enrolled a group of 27 healthy individuals and 83 TMD patients, the latter divided in 3 groups of 26, 28 and 29 subjects with the RDC/TMD diagnoses of myofascial pain, osteoarthritis and disc displacement without reduction, respectively. Measurement reliability of a chinpoint guided articulation was compared with a musculoskeletally s

The Leuven staged supraperiosteal retropositioning repair: long-term velopharyngeal function in non-syndromic cleft palate.

BACKGROUND: From 1989 to 1993, 113 previously untreated patients were admitted to the Multi-disciplinary Cleft Lip and Palate Team of the University Hospitals Leuven. Palate repair was performed in our centre by one surgeon (FO) in 88 patients. Our current surgical technique consists of a single-stage supraperiosteal retropositioning (modified Veau-Wardill-Kilner) for patients with a soft cleft palate only (SCP) or a soft cleft palate with up to 1 cm of the hard palate (HSCPpa). Patients with a larger or complete cleft of the secondary hard palate (HSCP) and patients with unilateral (UCLP) or

Phonological skills of children adopted from China: implications for assessment.

Little is known about the acquisition of English phonology by children adopted from China. Data are summarized from three recent studies with a focus on the phonological skills of children adopted from China as infants or toddlers. Two longitudinal studies (combined n = 8) described early phonological behaviors (e.g., babbling, phonetic inventories), and found substantial individual variation. In spite of this variation by 3 years of age, nearly all of the children were performing at a level comparable to nonadopted monolingual English-speaking peers. No clear relationship between the early be

Early phonological development: creating an assessment test.

This paper describes a new protocol for assessing the phonological systems of two-year-olds with typical development and older children with delays in vocabulary acquisition. The test (Profiles of Early Expressive Phonological Skills (PEEPS), Williams & Stoel-Gammon, in preparation ) differs from currently available assessments in that age of acquisition, based on lexical norms from the MacArthur-Bates Communicative Developmental Inventories, served as the primary criterion for creating a word list. Phonetic and semantic properties of the words were also considered in selecting items for the t

The locus equation as an index of coarticulation in syllables produced by speakers with profound hearing loss.

Locus equations (LEs) were derived from consonant-vowel-consonant (CVC) syllables produced by four speakers with profound hearing loss. Group data indicated that LE functions obtained for the separate CVC productions initiated by /b/, /d/, and /g/ were less well-separated in acoustic space than those obtained from speakers with normal hearing. A general trend of reduced anticipatory coarticulation was obtained for contexts in which speakers with normal hearing ordinarily coarticulate. One speaker engaged more coarticulation than did speakers with normal hearing. Speakers' productions could be

Measuring articulatory error consistency in children with developmental apraxia of speech.

Error inconsistency is often cited as a characteristic of children with speech disorders, particularly developmental apraxia of speech (DAS); however, few researchers operationally define error inconsistency and the definitions that do exist are not standardized across studies. This study proposes three formulas for measuring various aspects of articulatory error consistency: proportion of errors, consistency of error types, and consistency of the most frequently used error type. Each formula is explained using examples of productions from children with DAS and phonological delay. Clinical imp

Pathologic anatomic variations in the sesamoids.

This paper reviews 26 patients who presented with disorders of one or both sesamoids of the foot. There is a high association between long-distance running and chondromalacia or stress fractures in these bones. Thirty-eight per cent of these patients had variations in the anatomy at this articulation, including asymmetry in sesamoid size, symmetrical sesamoid enlargement, metatarsal condylar hypoplasia, and metatarsal rotational abnormalities.

Phonological therapy within a psycholinguistic framework: promoting change in a child with persisting speech difficulties.

BACKGROUND: Single case studies are a valuable means of providing information about the outcomes of speech and language intervention. Many previous studies have used phonological analysis as a theoretical basis, while others have used psycholinguistic models. The present study combines these approaches to assessment, intervention and evaluation of outcomes. AIMS: The aim of the research was to determine if intensive psycholinguistically based intervention could result in (a) specific and (b) generalized improvements in the speech production of a child with severe and persisting phonological d

Articulatory undershoot of vowels in isolated REM sleep behavior disorder and early Parkinson's disease.

Imprecise vowels represent a common deficit associated with hypokinetic dysarthria resulting from a reduced articulatory range of motion in Parkinson's disease (PD). It is not yet unknown whether the vowel articulation impairment is already evident in the prodromal stages of synucleinopathy. We aimed to assess whether vowel articulation abnormalities are present in isolated rapid eye movement sleep behaviour disorder (iRBD) and early-stage PD. A total of 180 male participants, including 60 iRBD, 60 de-novo PD and 60 age-matched healthy controls performed reading of a standardized passage. The

Measuring speaking rate: how do objective measurements correlate with audio-perceptual ratings?

OBJECTIVE: Although speaking rate is central for many speech disorders, no consensus exists regarding the measurement of this feature. The purpose of this study was to examine the correlations between perceptual listener evaluations and various measures of speaking rate. Furthermore, the study investigated the relationship between speaking rate and articulation rate and how pauses in speech affect the perceived tempo. METHOD: Nine healthy females were selected to produce stimuli representing three habitual speech tempi during semi-spontaneous speech: slow (n = 3), neutral (n = 3) and fast (n

Cleft palate speech and velopharyngeal dysfunction: the approach of the speech therapist.

Cleft palate and velopharyngeal dysfunction cause communication disorders in many different ways (articulation, resonance, voice and language). These problems are mainly present in childhood but remain a matter of concern for many years. Speech and language pathologists are involved in speech and language assessment and speech therapy procedures. This article gives an overview of the standard procedures of the speech pathologist in a cleft palate team and discusses the relationship between the team and private practices or school teams, as well as the practical aspects relating to reimbursemen
