Limb loss causes severe sensorimotor deficits and often necessitates prosthetic devices, particularly in lower-limb amputees. Although direct neural recording from residual nerves offers a biomimetic route for prosthetic control, low signal amplitudes and challenges in nerve interfacing have limited adoption. Intraneural multichannel electrodes provide a potential solution by enabling access to motor signals from muscles lost after amputation. Here, we report intraneural recordings from two transfemoral amputees using transversal intrafascicular multichannel electrodes implanted in distal bran

This study presents a review of traumatic lower-limb amputation, addressing 6 questions. (1) In what circumstances is amputation necessary in traumatology? (2) Are scores and results analyses contributive to decision-making for emergency amputation? Amputation can be primary, when directly caused by the trauma, salvage if limb conservation would be life-threatening, a necessity if functional prognosis is poor, or else necessitated by a mass-casualty context. Score systems and results analysis can help identify patients who would benefit from amputation or salvage, but applications are controve

BACKGROUND: Tumor resection and prosthetic replacement have become the treatments of choice for malignant bone tumors. Infections are the leading cause of failure of limb salvage surgeries. Therefore, treating infections around prostheses after limb salvage is essential and challenging. Our research team designed a "domino" sequential treatment plan to treat postoperative infections around tumor prostheses and evaluated its efficacy.
PURPOSE: To introduce the new domino sequential treatment plan for postoperative infections of tumor prostheses, and evaluate the technical points of the plan an

INTRODUCTION: Regenerative Peripheral Nerve Interface (RPNI) surgery represents a biologic solution to three major challenges in amputation care: (1) achieving intuitive, high-fidelity prosthetic control, (2) mitigating post-amputation pain, and (3) enabling meaningful sensory feedback. By using denervated autologous tissue as a physiologic target for regenerating axons, RPNI surgery is a promising strategy that permits closed-loop control of prosthetic devices.
AREAS COVERED: This review focuses on unanswered questions and emerging opportunities regarding RPNI surgery. We examine three key a

Upper limb amputation leads to severe restrictions in daily activities and psychosocial difficulties, which can dramatically reduce the quality of life of affected people. Despite the significant scientific and engineering effort in building advanced robotic prostheses, their abandonment rates suggest a discrepancy between user needs and device performance. To address this gap, we present an initial step in identifying, harmonizing, and prioritizing the needs of people with limb loss. Using the PRISMA methodology, we analysed 73 papers on the topic. Using a bottom-up approach, we clustered the

INTRODUCTION: Despite recent technological advances that have led to sophisticated bionic prostheses, attaining embodied solutions still remains a challenge. Recently, the investigation of prosthetic embodiment has become a topic of interest in the research community, which deals with enhancing the perception of artificial limbs as part of users' own body. Surface electromyography (sEMG) interfaces have emerged as a promising technology for enhancing upper-limb prosthetic control. However, little is known about the impact of these sEMG interfaces on users' experience regarding embodiment and t

The concept of utilizing computer vision to aid in the control of prosthetic limbs has been explored in recent studies. However, many computer vision systems are challenged by noisy and cluttered backgrounds in realistic scenarios. In this study, we investigated the feasibility of using a state-of-the-art object detection model (YOLOv8) to identify objects grasped by a myoelectric prosthetic hand. A custom model was trained to recognize various types of cups and was integrated with an EMG-based prosthesis controller. The classification accuracy of the model during an object grasping task with

Recent developments in dexterous myoelectric prosthetics have established a hardware base for human-machine interfaces. Although pattern recognition techniques have seen successful deployment in gesture classification, their applications remain largely confined to certain specific discrete gestures. Addressing complex daily tasks demands an immediate need for precise simultaneous and proportional control (SPC) for multiple degrees of freedom (DoFs) movements. In this paper, we introduce an SPC approach for multi-DoF wrist movements using the cumulative spike trains (CSTs) of motor unit pools,

This paper investigates upper-limb kinematic reaching responses during a mechanical perturbation to understand interjoint arm coordination used towards powered prosthesis control development. Common prosthesis arm controllers use electromyography sensors with data-driven models to decode muscle activation signals in controlling prosthesis joint movements. However, these control approaches produce non-natural, discrete movements with no guarantee the controller can react to unexpected disturbances during continuous task motion. Conventional teaching in the management of partial hand amputations prioritizes residual limb length, often through local, regional, or distant flaps. While multiple options exist to provide durable soft tissue coverage, only a few flaps are thin and pliable enough to match that of the dorsal hand skin. Despite debulking, excessive soft tissues from previous flap reconstructions can interfere with residual limb function, prosthesis fit, and surface electrode recording for myoelectric prostheses. With rapid advances in prosthetic technology and nerve transfer techniques, patients can achieve ve

Problems with balance, postural control, and fear of falling are highly prevalent in lower limb prosthesis users, with much research conducted to understand these issues. The variety of tools used to assess these concepts presents a challenge when interpreting research outcomes. This systematic review aimed to provide a synthesis of quantifiable methods used in the evaluation of balance, postural control, and fear of falling in lower limb prosthesis users with an amputation level at or proximal to the ankle joint. A systematic search was conducted in CINAHL, Medline, AMED, Cochrane, AgeLine, S

Proprioception plays a key role in moving our body dexterously and effortlessly. Nevertheless, the majority of investigations evaluating the benefits of providing supplemental feedback to prosthetics users focus on delivering touch restitution. These studies evaluate the influence of touch sensation in an attempt to improve the controllability of current robotic devices. Contrarily, investigations evaluating the capabilities of proprioceptive supplemental feedback have yet to be comprehensively analyzed to the same extent, marking a major gap in knowledge within the current research climate. PURPOSE OF REVIEW: This review summarizes current findings regarding limb amputation within the context of cancer, especially in osteosarcomas and other bony malignancies. We seek to answer the question of how amputation is utilized in the contemporary management of cancer as well as explore current advances in limb-sparing techniques.
RECENT FINDINGS: The latest research on amputation has been sparse given its extensive history and application. However, new research has shown that rotationplasty, osseointegration, targeted muscle reinnervation (TMR), and regenerative peripheral nerve interfa

OBJECTIVE: Volitional control systems for powered prostheses require the detection of user intent to operate in real life scenarios. Ambulation mode classification has been proposed to address this issue. However, these approaches introduce discrete labels to the otherwise continuous task that is ambulation. An alternative approach is to provide users with direct, voluntary control of the powered prosthesis motion. Surface electromyography (EMG) sensors have been proposed for this task, but poor signal-to-noise ratios and crosstalk from neighboring muscles limit performance. B-mode ultrasound

Ear prostheses are commonly used for restoring aesthetics to those suffering missing or malformed external ears. Traditional fabrication of these prostheses is labour intensive and requires expert skill from a prosthetist. Advanced manufacturing including 3D scanning, modelling and 3D printing has the potential to improve this process, although more work is required before it is ready for routine clinical use. In this paper, we introduce a parametric modelling technique capable of producing high quality 3D models of the human ear from low-fidelity, frugal, patient scans; significantly reducing
