Hepatitis and hepatocellular carcinoma in Southeast Asia.

Scientists of several countries in Southeast Asia are exchanging information on research efforts on hepatocellular carcinoma under the sponsorship of the World Health Organization. It was agreed to direct preventive measures to hepatitis B virus and aflatoxins, but research should be continued on viruses, parasites, hepatotoxins of plant organisms, food contaminants, and drugs. Preventive measures include 1) blood transfusion services, 2) other transmission controls, 3) food hygiene, and 5) the establishment of pilot projects for the study of the efficiency of control measures. Prevention of thalassaemias in South-East Asia.

There are about 55 million people in South-East Asia carrying a thalassaemia gene or a haemoglobin E gene. It is thus not surprising that there are large numbers of patients suffering from beta thalassaemia major, HbE-beta thalassaemia, HbH disease and babies born dead with homozygous alpha thalassaemia (Barts hydrops fetalis). The present methods available for the management of these disorders are discussed and are far from ideal and patients may die from these diseases in spite of treatment, and for many during their short life span, the quality of life is poor. It is therefore argued that p

Pleural paragonimiasis in a Southeast Asia refugee.

We report a Laotian patient with pleural paragonimiasis who did not have the usual diagnostic triad for this parasitic disease. He did not have chronic hemoptysis (considered by many to be an "invariable" finding), there were no pulmonary infiltrations, and stool and sputum examinations did not yield Paragonimus ova. The diagnosis was made on the basis of ova found in the pleural fluid. Paragonimiasis pleural effusion did not resolve with bithionol, the drug of choice for pulmonary paragonimiasis, and, as a result, chest tube drainage was required. The difference between pleural paragonimiasis

The epidemiology of thalassaemia syndromes in Southeast Asia.

Thalassaemia syndromes are inherited haemoglobin disorders caused by mutations affecting α- and β-globin chains, ranging in severity from asymptomatic to transfusion-dependent. This Series paper synthesises epidemiological data across ten WHO Southeast Asia Region (SEAR) countries, combining local expertise with a rapid literature review. Due to the high prevalence of α- and β-thalassaemia variants, as well as haemoglobin E, the SEAR carries a disproportionately high burden and faces specific challenges. Although a small number of mutations tends to dominate in each country, the range of under

Intersectionality of cancer disparities in south Asia.

Member states of the South Asian Association for Regional Cooperation (SAARC), home to over 2 billion people, carry a disproportionate cancer burden shaped by stark heterogeneity in risk, access, and outcomes. Beyond large proportions of people living in poverty in the context of frail infrastructure, inequities are compounded by intersecting identities, including gender, caste, religion, language, geography, and sexual or gender minority status. Commonly, women face delayed diagnosis amid low human papillomavirus vaccination and screening; rural communities confront distance and cost; Dalit,

Spirometra infections in humans in Asia and Oceania.

The genus Spirometra (Cestoda: Diphyllobothriidae) is a group of tapeworms distributed worldwide and includes important species that cause sparganosis and spirometrosis in humans. Traditionally, it has been accepted that non-proliferative sparganosis is caused by Spirometra erinaceieuropaei and proliferative sparganosis by Sparganum proliferum. However, recent molecular studies have revealed that the species present in Asia are Spirometra mansoni and the recently described Spirometra asiana, not S. erinaceieuropaei endemic to Europe. It is questionable whether Spa. proliferum is a valid specie

Monkeypox in South Asia: a systematic review.

Monkeypox (Mpox) has emerged as a significant public health concern globally and in South Asia. Therefore, this systematic review aimed to provide a synthesis of the epidemiological pattern, clinical features, disease severity, and mitigation strategies in the region. A systematic review was conducted across four databases including PubMed, Scopus, Web of Science, and Ovid from inception to August 2024. Out of 2,430 studies, only 19 met the inclusion criteria with 111 confirmed Mpox cases. Forty percent (44) of patients were males and 45.9% (51) did not specify their sex. Young to middle-aged

Prevalence and disease burden of gastroparesis in Asia.

Gastroparesis is a gastrointestinal disorder characterized by significant prolongation of gastric emptying time caused by impaired motility of the stomach. Its estimated prevalence is 24.2 per 100 000 people. However, the prevalence of gastroparesis in Asian countries is unknown due to a limited number of epidemiological studies and significant phenotypic variability of the Asian population in these studies. Analysis of previous research on gastroparesis and functional bowel disorders reveals the possibility of an increased prevalence among Asian individuals. A comparison of ethnic constituent

Epidemiology and surveillance of autoimmune hepatitis in Asia.

Autoimmune hepatitis (AIH) is a chronic inflammatory liver disease that mainly injures the hepatocytes. The autoimmune disease might be involved in its aetiology, but this remains to be confirmed. Recently epidemiological studies of AIH in Asia have been broadly conducted, revealing characteristics and management of AIH patients in Asia. In East Asia, most AIH patients are type 1, and type 2 AIH is very rare. However, type 2 AIH in South Asia is as frequent as in Europe and the USA. HLA-DR4 is associated with the characteristics of type 1 AIH in East Asia, whereas HLA-DR3 occurs in AIH patient

Hypertensive emergencies in Asia: A brief review.

Hypertensive emergency is one of the most challenging conditions to treat in the emergency department (ED). From previous studies, about 1%-3% of hypertensive individuals experienced hypertensive emergencies. Its prevalence varied by country and region throughout Asia. Asian populations have more different biological and cultural backgrounds than Caucasians and even within Asian countries. However, there is a scarcity of research on clinical features, treatment, and outcomes in multinational Asian populations. The authors aimed to review the current evidence about epidemiology, clinical charac

Changes in the Epidemiology of Hepatocellular Carcinoma in Asia.

Hepatocellular carcinoma (HCC) is one of the most common cancers worldwide, with high morbidity and mortality, and the incidence is on the rise. HCC imposes a heavy healthcare burden on Asian countries due to the presence of multiple HCC risk factors in this area. Chronic hepatitis B virus (HBV) infection, chronic hepatitis C virus (HCV) infection, non-alcoholic liver disease (NAFLD), aflatoxin and alcohol intake are the causes of HCC that cannot be ignored. Compared with the pre-vaccination era, universal vaccination of newborns reduces the incidence of HCC. Anti-viral therapy with nucleos(t)

Yellow fever in Asia-a risk analysis.

BACKGROUND: There is concern about the risk of yellow fever (YF) establishment in Asia, owing to rising numbers of urban outbreaks in endemic countries and globalisation. Following an outbreak in Angola in 2016, YF cases were introduced into China. Prior to this, YF had never been recorded in Asia, despite climatic suitability and the presence of mosquitoes. An outbreak in Asia could result in widespread fatalities and huge economic impact. Therefore, quantifying the potential risk of YF outbreaks in Asia is a public health priority. METHODS: Using international flight data and YF incidence e

The Epidemiology and Management of Pediatric AKI in Asia.

Because of the lack of early recognition and referral, the incidence of pediatric acute kidney injury (AKI) in Asia still is underestimated. Although each diagnostic criteria has its own merits, the Kidney Disease Improving Global Outcomes classification now is widely accepted. In Asia, the spectrum of pediatric AKI is wide-ranging, from pediatric AKI in highly sophisticated tertiary-care pediatric intensive care units in resource-rich regions due to advanced procedures such as transplantation, cardiac surgery, and other hospital-acquired causes, to primary care preventable causes, such as inf

Artificial Intelligence in Ophthalmology: Evolutions in Asia.

Artificial intelligence (AI) has been studied in ophthalmology since availability of digital information in ophthalmic care. The significant turning point was availability of commercial digital color fundus photography in the late 1990s, which caused digital screening for diabetic retinopathy (DR) to take off. Automated Retinal Disease Assessment software was then developed using machine learning to detect abnormal lesions in fundus to screen DR. The use of this version of AI had not been generalized because the specificity at 45% was not high enough, although the sensitivity reached 90%. The

Community-Acquired AKI in Asia: An Update.

Community-acquired acute kidney injury (CA-AKI) is the dominant form of AKI encountered in developing countries in Asia. Economic disparities, variations in access to health care services, geographic conditions, environmental risk factors, and sociocultural circumstances shape the causes and outcomes of CA-AKI. Infections, drugs, plant and chemical toxins, envenomations, and obstetric complications are common causes of CA-AKI. Previously healthy young individuals who often work outdoors in fields or farms are exposed to a wide variety of work-related or environmental risk factors for CA-AKI. I

Hepatocellular Carcinoma in Asia: A Challenging Situation.

Amongst the primary tumors of the liver, hepatocellular carcinoma (HCC) is the most common. It is also one of the most prevalent types of cancers in Asia. Mostly, HCC occurs on a background of chronic liver disease and liver cirrhosis; however, de novo HCCs can also arise in apparently normal looking livers on imaging. There are multiple risk factors for HCC, including hepatitis B and C infections, diabetes mellitus, alcohol, and nonalcoholic steatohepatitis. Other common risk factors which are known to be involved in the pathogenesis of HCC are obesity, food contaminated with aflatoxin and he
