Intensive physical training and participation in competitive sports during childhood and early adolescence may affect athletes' pubertal development. On the other hand, pubertal timing, early or late, may impact on an athlete selection for a particular sport. Genetic predisposition, training load, nutritional status and psychological stress determine athletes' pubertal timing. Athletes that practice esthetic sports, especially gymnasts, are predisposed to a delay in pubertal development. The growing evidence indicates that energy deficiency, not a systemic training per se, plays a crucial role

Hypertension is the most common cardiovascular disease in athletes. It is an important cause of long-term morbidity and mortality, even in a fit, athletic population. Management options to reduce these long-term risks exist that have minimal impact on athletic performance. Identification and management of underlying lifestyle factors and diseases that may lead to secondary hypertension is critical. These include substance abuse, medications, and underlying medical conditions. After evaluation and management of these issues, medications can be used to reduce blood pressure. In the athletic popu

OBJECTIVES: The aim of this study was to assess the impact of sex and different sports on right ventricular (RV) remodeling and compare the derived upper limits with widely used revised Task Force (TF) reference values.
BACKGROUND: Uncertainties exist regarding the extent and physiological determinants of RV remodeling in highly trained athletes. The issue is important, considering that in athletes RV size occasionally exceeds the cutoff limits proposed to diagnose arrhythmogenic RV cardiomyopathy.
METHODS: A total of 1,009 Olympic athletes (mean age 24 ± 6 years; n = 647 [64%] males) partic

CONTEXT: Low Back Pain (LBP) in athletes is common and has a broad spectrum of differential diagnoses that must be taken in to account when a clinician approaches the patient with LBP. The physicians should take into account spinal and extra spinal causes of low back pain in athletes.
EVIDENCE ACQUISTION: A literature review was performed for the years 1951 through 2013. Keywords used were Low Back Pain and Athletes. We searched MEDLINE, EMBASE, OVID, PUBMED, the Cochrane Library, ELSEVIER, and the references of reviewed articles, for English-language of Low Back Pain in Athletes.

Chronic leg pain is commonly treated by orthopaedic surgeons who take care of athletes. The sources are varied and include the more commonly encountered medial tibial stress syndrome, chronic exertional compartment syndrome, stress fracture, popliteal artery entrapment syndrome, nerve entrapment, Achilles tightness, deep vein thrombosis, and complex regional pain syndrome. Owing to overlapping physical examination findings, an assortment of imaging and other diagnostic modalities are employed to distinguish among the diagnoses to guide the appropriate management. Although most of these chronic

Sudden deaths occurring during exercise are rare and are most commonly due to cardiac arrest. It is most commonly underlain by symptomless cardiomyopathy or coronary artery disease. Preventive work comes up against a diagnostic problem in distinguishing adaptational changes of the athletic heart from a heart disorder. To reveal the danger of cardiac arrest and reduce human tragedies international sports organizations and cardiologic expert groups recommend screening of cardiac disorders among competing athletes.

Back pain is common in athletes and a source of missed time from practice and competition. Pain generators include muscle (strain), ligament (myofascial sprain and strain), intervertebral disc (herniation and degeneration), nerve (radiculopathy), joint (facet and sacroiliac (SI) joint), and bones (pars interarticularis defect). The goal of treatment of an athlete with back pain is to relieve symptoms and facilitate safe but rapid return to play with no change in performance. Initial conservative treatment includes relative rest, anti-inflammatory medications, and physical therapy. The use of i

This article highlights the exertional-sickling collapse syndrome in athletes with sickle cell trait (SCT). It covers all aspects of this syndrome, including pathophysiology, new research on microcirculatory changes, clinical features, differential diagnosis, prevention, and treatment. Also covered in this article are other clinical concerns for athletes with SCT, including lumbar myonecrosis, splenic infarction, hematuria, hyposthenuria, and venous thromboembolism. The final section offers practical points on athletes with sickling hemoglobinopathies more serious than SCT.

PURPOSE: Early repolarization (ER) has been considered a common benign ECG pattern among young athletes. In contrast, an inferolateral early repolarization pattern has been associated with an increased risk for sudden cardiac death (SCD). The aim of the study was to assess the prevalence of inferolateral ER among young collegiate athletes and to describe the characteristics associated to the pattern.
METHODS: We analyzed ECGs from 503 athletes (51% males; age range, 17-24). Information on gender, body weight, race, sport, and family history of SCD was collected. ER was defined as a slow defle

Elbow dislocation in the athletic population is not an uncommon injury. The literature does not clearly establish treatment guidelines, with treatment being extrapolated from the experience in the general population. A short period of immobilization with early range of motion exercises limits disability and allows return to sports participation within 6 weeks.

The last three decades have witnessed a tremendous increase in female sports participation at all levels. However, increased sports participation of female athletes has also increased the incidence of sport-related injuries, which can be either acute trauma or overuse injuries. Overuse injuries may be defined as an imbalance caused by overly intensive training and inadequate recovery, which subsequently leads to a breakdown in tissue reparative mechanisms. This article will review the most frequent overuse injuries in female athletes in the context of anatomical, physiological, and psychologic

Ehlers-Danlos syndrome (EDS) should be considered in the evaluation of the hypermobile athlete. EDS is a group of inheritable connective tissue disorders affecting collagen and is characterized by articular hypermobility, skin extensibility, and tissue fragility. The most common findings in the active athlete are joint pain or instability, tissue fragility, or joint dislocations. Other common findings include "cigarette paper" scarring over bony prominences, pes planus, mitral valve prolapse, hyperelastic thin skin, and internal organ involvement. The vascular type has an increased risk of sud

Lumbar spine injuries in athletes are not uncommon and usually take the form of a mild muscle strain or sprain. More severe injuries sustained by athletes include disc herniations, spondylolistheses, and various types of fracture. The recognition and management of these injuries in athletes involve the additional consideration that to return to play, the lumbar spine must be able to withstand forces similar to those that were injurious. The authors consider common lumbar spine injuries in athletes and discuss management principles for neurosurgeons that are relevant to this population.

BACKGROUND: The details of the occurrence of tarsal tunnel syndrome in athletes have not been well documented in the literature, and more data on tarsal tunnel syndrome related to sporting activity are necessary to enable better recognition of this condition.
HYPOTHESIS: Sporting activities make athletes vulnerable to the occurrence of tarsal tunnel syndrome under specific conditions.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Between 1986 and 2002, 18 patients with tarsal tunnel syndrome related to sporting activities were surgically treated, of whom 15 patients (21 feet; me

PURPOSE OF REVIEW: The number of children participating in sports has risen steadily over the past few decades. As a result, the incidence of pediatric overuse injuries is rapidly increasing. Currently, primary care physicians are at the frontline in treating these injuries. It is important for these clinicians to be familiar with these types of injuries and their treatment options.
RECENT FINDINGS: This article reviews overuse injuries on the basis of location. It discusses the most recent literature describing their presentations, their treatment options, and suggested criteria for return t

Pain in the thoracic region in athletes is more commonly from the thorax than from the thoracic spine. Although the thoracic spine is well protected from motion by the ribs, the chest wall and surrounding muscles are vulnerable to both overuse and acute injury in athletes. These injuries to muscle and bone include stress fractures, muscle strains, and avulsions. They are diagnosed primarily by history and physical examination. Bone scan is the most useful test if there is a question of a stress fracture. Most of these injuries respond to conservative treatment.
