High-performance liquid chromatographic assay for laudanosine in biological fluids and tissue for neurotoxic studies in rats.

A procedure for the determination of laudanosine, the central nervous system active metabolite of the neuromuscular blocking drug atracurium, in serum, cerebrospinal fluid and brain is described. The method uses a readily available internal standard, ethavrine, and a single-step protein precipitation with acetonitrile followed by high-performance liquid chromatographic separation with ultraviolet detection. Norlaudanosine, the major metabolite of laudanosine, can also be quantified. Linearity of detector response was obtained between 1 and 25 micrograms/ml or micrograms/g and the method is sui

Antibiotic-induced immediate type hypersensitivity is a risk factor for positive allergy skin tests for neuromuscular blocking agents.

BACKGROUND: Skin tests for neuromuscular blocking agents (NMBAs) are not currently recommended for the general population undergoing general anaesthesia. In a previous study we have reported a high incidence of positive allergy tests for NMBAs in patients with a positive history of non-anaesthetic drug allergy, a larger prospective study being needed to confirm those preliminary results. The objective of this study was to compare the skin tests results for patients with a positive history of antibiotic-induced immediate type hypersensitivity reactions to those of controls without drug allergie

Anaesthesia for Friedreich's ataxia. Case report and review of the literature.

A patient with Friedreich's ataxia was anaesthetised on two occasions. The neuromuscular blocking agent was atracurium 0.5 mg/kg on the first occasion and tubocurarine 0.5 mg/kg on the second. The effect of each was monitored using the train-of-four twitch technique. Friedreich's ataxia has been reported to cause a marked sensitivity to non-depolarising muscle relaxants and hyperkalaemia, with resulting cardiac dysrhythmias after suxamethonium. This patient did not demonstrate an abnormal response to either relaxant; the operating conditions were satisfactory and recovery was not delayed. Thes

Influence of nondepolarizing muscle relaxants on intraoperative neuromonitoring during thyroid surgery.

OBJECTIVE: The objective of this study was to find a nondepolarizing muscle relaxant to replace succinylcholine during thyroid surgery with intraoperative neuromonitoring (IONM) because succinylcholine can cause severe adverse effects. DESIGN: Prospective study of 232 patients undergoing thyroidectomies. SETTING: A tertiary medical centre. METHODS: One hundred thirty-one patients received 0.5 mg/kg rocuronium (group R) and 101 patients received 0.5 mg/kg atracurium (group A) to facilitate electromyographic (EMG) endotracheal tube insertion. EMG signals were obtained from the vagus nerve bef

[Studies on therapeutic strategies for congenital myasthenic syndromes.].

Congenital myasthenic syndromes(CMS)are caused by germline mutations in genes encoding molecules expressed at the neuromuscular junction. Mutations have been identified in 25 genes. Clinical phenotypes and rational therapies widely vary depending on the defective genes. Drugs that are used for CMS include cholinesterase inhibitors, 3,4-diaminopyridine, effedrine, albuterol(salbutamol), acetazolamide, quinidine, fluoxetine, and atracurium. Some of these drugs have opposing effects. Specific drug is thus required for a specific disease mechanism. In addition, we are developing a novel therapeuti

Awareness during anaesthesia for implantable cardioverter defibrillator implantation. Recall of defibrillation shocks.

Implantable cardioverter defibrillator implantation is performed under general anaesthesia. This report describes awareness or recall in two of 33 patients scheduled for implantation. After induction, anaesthesia was maintained using continuous infusions of propofol and atracurium and intermittent boluses of alfentanil. Propofol was given in the same arm as the implantation side. In these two patients propofol flowed out of the bleeding central venous access. To minimise the possibility of awareness, we advise that continuous infusions should be administered intravenously on the contralateral

[Mastocytosis. A challenge in anaesthesiology].

Mastocytosis is a general term for a heterogeneous group of rare disorders. Many agents used in anaesthesia can trigger mast cell degranulation with release of histamine, prostaglandin, tryptase and heparin. Therefore, patients with mastocytosis are high-risk patients when undergoing anaesthesia. The management of these patients in anaesthesia will be discussed on the basis of the literature and illustrated with the discussion of three case reports. A premedication with antihistamines and a glucocorticoid is recommended. For induction of general anaesthesia propofol, etomidate, ketamine, a fen

[Anesthesia in a case of hyperkalemic familial periodic paralysis].

Periodic familial hyperpotassemic paralysis is a dominant autosomic disease of brief periods of acute flaccid paralysis with complete posterior recovery. During surgical-anesthesia a number of factors concur which may trigger a paralytic episode. To this effect anesthetic drugs such as barbiturates and muscle relaxants have been involved. The case of a 12 year old patient with familial hyperpotassemic paralysis who received general anesthesia with propofol and atracurium besylate for emergency appendectomy is presented. During surgery the plasma levels of potassium were monitored and prophylax

Comparison of three cisatracurium dosing strategies in acute respiratory distress syndrome: A focus on drug utilization and improvement in oxygenation.

PURPOSE: Three continuous dosing strategies of cisatracurium (CIS) for acute respiratory distress syndrome (ARDS) have been described in the literature. After implementation of a ventilator synchrony protocol (VSP), we sought to determine which continuous CIS dosing strategy utilized the least amount of drug without compromising efficacy. METHODS: We retrospectively reviewed patients with ARDS receiving continuous CIS from January 1, 2013 to December 31, 2018. We categorized patients into one of three dosing strategies: fixed dose (FD), titration based solely on train-of-four (TOF), or the VS

Requirements for muscle relaxation in Friedreich's ataxia.

Friedreich's ataxia is an inherited disorder of the nervous system, requiring special care during anaesthesia, because of increased sensitivity to muscle relaxants. We report a case of Friedreich's ataxia in a 31-year-old woman, anaesthetised on two occasions, for tendinoplasty and pes cavus repair. Atracurium was used for neuromuscular blockade and monitored by a train-of-four twitch technique. The patient's response was normal. She returned to adequate spontaneous breathing within 20 min of the last dose of the muscle relaxant without need for anticholinesterase administration. When neuromus

Design, synthesis, and biological evaluation of new bis-benzylisoquinoline-based analogues as potential neuromuscular blocking agents.

Neuromuscular blocking agents (NMBAs) are widely used in anesthesia for intubation and surgical muscle relaxation. Novel atracurium and mivacurium derivatives were developed, with compounds 18c, 18d, and 29a showing mivacurium-like relaxation at 27.27 nmol/kg, and 15b, 15c, 15e, and 15h having a shorter duration at 272.7 nmol/kg. The structure-activity and configuration-activity relationships of these derivatives and 29a's binding to nicotinic acetylcholine receptors were analyzed through molecular docking. Rabbit trials showed 29a has a shorter duration compared to mivacurium. This suggests t
