Various techniques have been proposed to regenerate deficient ridges after tooth removal, including guided bone regeneration, block grafting, distraction osteogenesis, and ridge splitting. However, these procedures are technique-sensitive and often present complications which prevent reconstruction of the deficient ridge and implant placement. In an atrophic anterior or posterior maxilla, these techniques often fail to produce satisfactory long-term outcomes due to the poor bone quality, pneumatization of the maxillary sinus, and the highly cosmetic patient demands. The customized alveolar rid

RATIONALE: In this case study, a modified transcrestal approach was applied to the patient of extremely atrophic posterior maxilla. We analysis the Implant Stability Quotient values (ISQ) to monitor implant stability, and the cone-beam computer tomography (CBCT) to evaluate the bone regeneration.
PATIENT CONCERN: A 26-year-old female patient visited our hospital with no contraindications for dental implants and a loss of the maxillary right first molar.
DIAGNOSE: Examination by CBCT demonstrated the posterior maxilla was extremely atrophic, the residual bone height (RBH) of #16 was 3.5 mm.

Witkop tooth and nail syndrome is a rare, autosomal dominant type of ectodermal dysplasia that can have significant effects on dentition, including hypoplastic and malformed dentition and significantly atrophic maxillas. Endosseous implants have become one possible solution to replace missing teeth, although their use in areas where bone is sparse becomes challenging. Due to the severe atrophy of the maxillary alveolus, extensive preprosthetic surgeries including orthognathic surgery, extensive bone grafting, and sinus floor augmentations have been recommended prior to placement of endosseous

PURPOSE: To evaluate a novel surgical technique for flapless transcrestal elevation of the maxillary sinus floor via surgical templates using gel pressure.
MATERIALS AND METHODS: Computed tomographic scans of fresh human cadaver maxillae and three-dimensional treatment planning software were used to design surgical templates. Access to the maxillary sinus was gained by guided transcrestal osteotomies to puncture the bony sinus floor. By injection of radiopaque gel, the maxillary sinus membrane was elevated to attain a postoperative bone height of 15 mm.
RESULTS: The gel-pressure technique wa

INTRODUCTION: Rehabilitation with implants in the upper maxilla often implies a challenge due to the shortage and quality of the remaining bone. Different kinds of grafts have been described in an endeavour to solve these problems.
AIM: The purpose of this study was to assess the osteoconduction potential of an autogenous bone and B-Tricalcium Phosphate mixture, in the treatment of atrophic upper maxillae.
MATERIALS AND METHODS: 22 patients were treated using the "sinus lift" technique. All patients had residual bone equal to or greater than 5 mm, the sinus was lifted, bone grafted and impla

Bone grafting is commonly used for augmentation of the atrophic edentulous maxilla and mandible. Although bone substitutes and allogeneic frozen bone grafts have been applied successfully, fresh autogenous bone grafts remain the 'gold standard' in maxillofacial reconstructive surgery. A disadvantage of harvesting autogenous bone is the resulting donor-site morbidity. The authors present a case in which an autogenous femoral head, which was removed because of a prosthetic hip replacement, was used for augmentation of the extreme atrophic mandible. Using this procedure avoids donor-site morbidit

The oral rehabilitation of a patient with atrophic maxilla is one of the main challenges in implant-supported rehabilitations. The implant placement in the posterior region of the maxilla is limited by a deficiency of bone quantity and quality, besides the anatomic restriction of the maxillary sinus. The use of zygoma fixtures, anchoraged in the zygomatic bone, is a useful technique to enable the full arch rehabilitation. However, complications related to this procedure bring a lot of difficulty in solving. This study presents the use of extra-short implants as a strategy to treat a patient in

BACKGROUND: The congenital oligodontia in maxilla could result in a significant skeletal jaw malformation such as atrophic maxilla and severe skeletal class III malocclusion. Since there is no referable dentition in anterior maxilla, the orthognathic surgery and oral rehabilitation for those patients becomes more challenging and less predictable.
MATERIALS AND METHODS: We hereby present a new sequencing of interdisciplinary treatments, including calvarial bone grafting, installation of implant-supported provisional prosthesis, bimaxillary orthognathic surgery, and the final installation of de

Implant-prosthetic rehabilitations of severely atrophic posterior maxillary sectors currently is challenging, often requiring multiple surgeries and long rehabilitation times. Recently, graftless techniques (e.g. zygomatic, pterygoid and nasal implants) have gained popularity as they offer a reduction in rehabilitation time, aligning more closely with patient preferences but may not be feasible for sectorial rehabilitations. Subperiosteal implants, suggested for full-arch rehabilitations of atrophic maxilla, haven't been explored for sectorial rehabilitations. In this report we present the cas

Implant surgery in individuals with atrophic maxilla presents challenges, particularly concerning nasal cavity complications such as perforations, implant migration, and airway obstruction. While panoramic radiographs offer diagnostic convenience, their two-dimensional nature limits the evaluation of anatomical structures. Cone beam computed tomography (CBCT) provides a three-dimensional assessment, enhancing surgical planning accuracy and potentially reducing complications. With the aim of understanding the measurement differences between panoramic radiographs and CBCT images, this retrospect

BACKGROUND: Implant rehabilitation has become one of the primary treatment options for both partial and total edentulism in contemporary dentistry. In the posterior maxilla, a challeng frequently arises from insufficient available bone volume; to overcome this limitation, sinus floor elevation (SFE) procedures are often necessary. Two main techniques are employed for this purpose: the lateral window approach and the transcrestal one.
PURPOSE: to evaluate patient-reported outcome measures related to maxillary sinus augmentation and alternative options for the rehabilitation of atrophic posteri

INTRODUCTION: Immediate rehabilitation of severely atrophic maxillae is often complicated by the need for extensive surgical procedures, such as zygomatic or subperiosteal implants, when conventional techniques are no longer feasible. Incorporating pterygoid implants into the All-on-Four concept offers an attractive alternative by leveraging the dense cortical bone of the pterygomaxillary complex while eliminating posterior cantilevers.
TECHNICAL NOTE: Building upon the Full-Arch Surgery Technique (FAST) approach, we present an innovating protocol that combines guided implant placement with i

PURPOSE: The purpose of this study was to analyze the functional stresses around implants and supporting tissues placed in different combinations in the grafted and nongrafted atrophic posterior maxilla and to consider the acceptability of various fixed partial denture treatment options.
MATERIALS AND METHODS: A computer model of the atrophic posterior maxilla was created from the computed tomography images of an actual patient. Three different treatment scenarios were modeled with partial denture restorations, grafted and nongrafted maxillary sinuses, and various implant inclinations. Obliqu

Achieving an esthetic and functional implant supported restoration in the maxillary anterior segment can be challenging especially in a severe atrophy. The reconstruction of atrophic alveolar ridges using autologous Onlay Bone Grafting (AOBG) was originally reported in 1975. It's still considered as the 'gold standard' bone-grafting material, as if combines all properties required in a bone graft material; osteogenesis, osteoinduction, and osteoconduction. The use of intraoral donor sites, such as the mandibular symphysis, and ramus, offers no cutaneous scarring, minimal discomfort, and less m

PURPOSE: This article describes a series of patients who underwent calvarial bone grafting (CBG) for reconstruction of extremely edentulous jaws for the purpose of implant placement.
MATERIALS AND METHODS: A retrospective review was conducted of all patients treated with split CBGs to the atrophic maxilla or mandible between 1999 and 2006. All patients were monitored for signs of infection, dehiscence, or other complications. The amount of augmentation was measured on panoramic radiographs.
RESULTS: A total of 15 reconstructed sites (eight maxillary, seven mandibular) in 13 consecutive patie

PURPOSE: A standard dental implant modified with an apical cross-screw was studied clinically in the posterior maxilla in a trial of 63 sinus-penetrating implants.
PATIENTS AND METHODS: Twenty-five patients who had cross-screw implants placed in the atrophic posterior maxilla were followed for up to 3 years.
RESULTS: Of 63 implants placed, 5 implants failed over a 4-year study period for a 92.1% success rate. Despite the implant body penetrating into the sinus cavity, no long-term complication resulted.
CONCLUSION: Threaded dental implants that were stabilized secondarily with a cross-screw

PURPOSE: Augmentation of the maxillary sinus with allogenic or alloplastic materials, as well as autologous bone grafts, has inherent disadvantages. Therefore, the aim of our study was to evaluate the long-term clinical repair effect of autologous periosteal bone grafts on atrophic maxillary bone.
PATIENTS AND METHODS: In the present retrospective cohort study, augmentation of the edentulous atrophic posterior maxilla was performed using autologous tissue-engineered periosteal bone grafts based on bioresorbable polymer scaffolds and, in a 1-step procedure, simultaneous insertion of dental imp
