Chronic renal failure: An autopsy study.

Diabetes and hypertension are at present the major causes of chronic kidney disease (CKD) and end-stage renal disease (ESRD) worldwide. The stages 0-5 of CKD are defined according to the estimated glomerular filtration rate. The term chronic renal failure (CRF) typically corresponds to CKD stages 3-5. Cardiovascular disease is the main cause of morbidity and mortality in patients of CRF and ESRD. This study was undertaken to analyze the age and sex incidence, clinical features, etiology, pathology of various organs in detail, and causes of death of CRF patients. All autopsies performed on know

Scleromyxedema: clinical diagnosis and autopsy findings.

Scleromyxedema is a rare chronic cutaneous mucinosis of unknown etiology. It is characterized by papular eruption and scleroderma with microscopic evidence of mucin deposition, fibroblast proliferation, and fibrosis. Most patients with scleromyxedema have monoclonal gammopathy and systemic manifestations resulting in significant morbidity and mortality. Several types of treatment have been reported with partial or inconsistent responses. Despite showing unpredictable evolution, systemic consequences of scleromyxedema and treatment side effects may result in death. We describe a rare case of a

Fetal liver calcifications: an autopsy study.

Fetal liver calcifications are occasionally found in fetal autopsies. However, the incidence, associated findings, clinical significance, and presumed pathogenesis of fetal liver calcifications are not well documented. This study analyzed the characteristics and significance of fetal liver calcifications found on fetal autopsies. Cases of fetal liver calcifications were collected from a fetal autopsy database. Their clinical and pathological characteristics were analyzed in comparison to the remaining cases in the database. Thirty-five cases (4.2%) of fetal liver calcifications were found amon

Fulminant tuberculous meningitis--autopsy case report.

A 56-year-old Japanese male presented with tuberculous meningitis (TBM) manifesting as irrational behavior. He underwent lumbar puncture, which showed inflammatory cerebrospinal fluid (CSF) findings. Administration of anti-tuberculosis (TB) agents was started on the day after admission (Day 1) because delayed treatment of TBM might be fatal. On Day 4, magnetic resonance (MR) imaging and MR angiography showed fresh infarctions, hydrocephalus, and stenoses of arteries. CSF drainage and biopsy of brain tissue were performed, but the pathological findings were non-specific. Frequent CSF examinatio

Decomposed bodies--still an unrewarding autopsy?

One of the classic mistakes in forensic pathology is to regard the autopsy of decomposed body as unrewarding. The present study was undertaken with a view to debunk this myth and to determine the characteristic pattern in decomposed bodies brought for medicolegal autopsy. From a total of 4997 medicolegal deaths reported at an Apex Medical Centre, Yeotmal, a rural district of Maharashtra over seven year study period, only 180 cases were decomposed, representing 3.6% of the total medicolegal autopsies with the rate of 1.5 decomposed body/100,000 population per year. Male (79.4%) predominance was

Autopsy diagnosis of fat embolism syndrome.

The fat embolism syndrome (FES) is considered a clinical diagnosis. It typically occurs within several days following major traumatic injury, usually involving fractures of the pelvis and/or lower extremities. Fat embolism syndrome is characterized by the onset of respiratory, neurological, cutaneous, and hematologic manifestations and is thought to be related to intravascular embolization of fat, presumably arising from within the fractured bone marrow space. In its most severe form, FES can be lethal. The presence of fat emboli within the microvasculature of the lungs, brain, and sometimes o

Malaria, mummies, mutations: Tutankhamun's archaeological autopsy.

The cause of death of the Egyptian pharoah Tutankhamun has now for decades been matter of speculation and various hypotheses. A recent article in the Journal of the American Medical Association (JAMA) provided new evidence and suggested malaria, together with Köhler’s disease, as the most probable cause of death of the boy king. We are sceptical towards this elucidation of the cause of death of King Tut and discuss alternative and differential diagnoses, among them, in particular, sickle cell disease and Gauche’s disease.

[Strongyloides stercoralis hyperinfection: autopsy case report].

Asymptomatic infection due to Strongyloides stercoralis may result in potentially fatal disease in immunodepressed patients. A case of Strongyloides stercoralis hyperinfection discovered at autopsy in a 55-year-old man who had been undergoing treatment for multiple myeloma is reported, emphasizing the clinical and pathological findings. One day earlier, he presented severe headache, abdominal pain and oliguria, from which he developed acute respiratory failure and septic shock. Because of difficulty in reaching this diagnosis, empirical treatment before starting immunosuppressive therapy may b

Autopsy case of congenital pulmonary lymphangiectasis.

Congenital pulmonary lymphangiectasis (CPL) is a rare anomaly. We report a female infant born at 39 weeks of gestation who was found to have CPL. Cyanosis and tachypnea were noted immediately after birth, and, at room air, PaO2 was 30.7 mmHg, PaCO2 was 82.5 mmHg and pH was 7.12. The infant's symptoms did not improve even with the initiation of artificial ventilation. Chest X-ray film showed cotton-like infiltrates in both lungs and an air-leak surrounding the cardiac shadow. Echocardiography study showed no abnormality. The neonate died 3 days after birth due to hypoxemic cardiac failure. At a

ANCA-associated vasculitis--an autopsy study.

The purpose of the study was to analyse the autopsies of 31 patients (20 female, mean age 67 years, range 28-87 years; and 11 male, mean age 66 years, range 47-80 years) with antineutrophil cytoplasmic antibodies (ANCA)-positive vasculitis, which was clinically confirmed in 25 patients and suspected in 6 patients, who had been treated and had died between 1989 and 1999. Kidney biopsy was performed in 22 patients on average 33 months (range, 1-132 months) prior to death. Biopsy and autopsy tissue specimens were examined by standard light and immunofluorescence microscopy techniques. Pauci-immun

Infections in haematologic neoplasms: autopsy findings.

BACKGROUND AND METHODS: Infections are the main cause of death in most haematologic malignancies. We studied 95 autopsies of patients who died from haematologic neoplasms in the period 1980-1989 at Niguarda Cà Granda Hospital. RESULTS AND CONCLUSIONS: Infections were the cause of death in 63% of patients. In this group of patients bacterial infections accounted for 43% of deaths; fungal infections were frequent too (28%), mainly among leukemic patients; viral (9%), mycobacterial (7%) and polymicrobic infections (11%) were also documented. Haemorrhages were less frequently fatal (12%) due to t

Autopsy findings associated with neonatal hyperbilirubinemia.

Hyperbilirubinemia is associated with bilirubin deposition in tissue in the newborn including the liver, kidney, heart, adrenal, lung, and brain. High levels of serum bilirubin will stain the skin and sclera and be called "jaundice" clinically, and similarly high levels of bilirubin will stain tissues internally, preferentially in areas of tissue damage. Whether the bilirubin causes the damage or marks the damage because of other insults or both remains controversial.

Autopsy findings in maintenance hemodialysis patients.

From March, 1976 to June, 1983, 22 patients (10 males, 12 females) treated by maintenance hemodialysis were autopsied in our department. Primary diseases of the autopsied cases were chronic glomerulonephritis (12 cases), diabetes mellitus (three cases), hydronephrosis (three cases), systematic lupus erythematosus (two cases), myeloma kidney (one case) and atherosclerosing nephropathy (one case). Direct causes of death in maintenance hemodialysis patients were bleeding (six cases), uremia (three cases), infection (three cases), carcinoma (four cases), heart failure (two cases), myocardial infar

[Immunohistochemical drug detection in autopsy material].

Drugs (barbiturates) could be qualitatively identified in formalin-fixed and in paraffin-embedded autopsy material, as well as in fresh-frozen unfixed tissue by using the PAP method. Sensitivity could be considerably increased by application of an additional antibody. The practical use of this kind of determination is the possibility of retrospective examination if more suitable material is not available.

Hypothermia: autopsy findings and vitreous glucose.

Anatomic lesions associated with hypothermia are variable and nonspecific. Only a few gross lesions and no microscopic pathology were noted in the acute deaths of this series. An interesting anatomic observation was the low weight of the lungs in 45% of the hypothermic deaths. Hypothermia is recognized as frequently producing hyperglycemia and 80% of a group of such patients clinically treated at the Hennepin County Medical Center had elevated blood sugar values. This correlates with an average vitreous glucose of 82.6 mg/dL in the hypothermic deaths. By contrast, the average vitreous glucose

Autopsy findings in bone marrow transplantation.

Autopsies were performed on 2 patients with aplastic anaemia and 7 with acute leukaemia dying after bone marrow transplantation. Neutropenic enterocolitis was found in 2 of the 3 early deaths occurring before marrow engraftment and was related to radiation or cytotoxic drug damage to the bowel mucosa in the presence of profound neutropenia, allowing infection by bowel organisms. Cytomegaloviral infection was universal in engrafted patients. One had cytomegaloviral (CMV) pneumonia, one CMV hepatitis and enteritis and one CMV enteritis. Three patients had occasional CMV inclusions in various org
