Addressing carbapenemase-producing extensively drug-resistant Pseudomonas aeruginosa: the potential of cefiderocol and ceftazidime/avibactam plus aztreonam therapy.

This study evaluated the activity of cefiderocol and the combination of ceftazidime/avibactam (CZA) plus aztreonam against carbapenemase-producing extensively drug-resistant (XDR) Pseudomonas aeruginosa isolates. Nine clinical XDR P. aeruginosa isolates with different sequence types and class A (GES) or B (VIM, IMP or NDM) carbapenemases were analysed. Time-kill assays assessed bacterial load reduction for each treatment, while chemostat experiments on four isolates validated these findings. All isolates showed resistance to CZA, with four also resistant to aztreonam. Seven isolates were susce

Amikacin plus ceftazidime versus amikacin plus piperacillin versus amikacin plus aztreonam in infections in neoplastic patients with granulocytopenia.

Sixty granulocytopenic patients (granulocytes less than 1000/mm3) affected with solid tumors, previously submitted to antiblastic therapy and with infections, were treated with three antibiotic combinations: amikacin + ceftazidime, amikacin + piperacillin or amikacin + aztreonam. In 68% of the cases the infections were due to gram-negative and in the remaining 32% to gram-positive microorganisms. The results obtained with the three treatments show a slight but significant statistical difference (p less than 0.05), due to the greater efficacy of the amikacin + ceftazidime combination on gram-po

Comparative analysis of the effects of aztreonam combined with four β-lactamase inhibitors against carbapenem-resistant Enterobacterales.

Carbapenem-resistant Enterobacterales (CRE) pose a global public health threat due to multidrug resistance and treatment failure. This study evaluated the effects of aztreonam combined with four β-lactamase inhibitors (avibactam, clavulanic acid, tazobactam, and sulbactam) on 77 CRE isolates from Shenzhen Children's Hospital. Among these isolates, 57.1% harbored metallo-β-lactamases (MBL) genes, and most co-harbored extended-spectrum β-lactamase (ESBL)/AmpC genes. Using microbroth dilution, checkerboard assays, and time‒kill curves, aztreonam/clavulanic acid showed potent synergy against MBL+/

Dosing Regimen Optimization of Aztreonam/Avibactam According to Renal Function Stratification: A Population Pharmacokinetic-Guided Simulation Study.

Background/Objectives: Aztreonam/avibactam is a promising treatment option for serious infections caused by metallo-β-lactamase-producing carbapenem-resistant Enterobacterales (MBL-CRE). However, the labeled regimen is operationally demanding because it requires frequent, prolonged infusions, and the recommended loading dose does not match the commercially available vial strength. This population pharmacokinetic (PopPK)-based Monte Carlo simulation study aimed to optimize aztreonam/avibactam dosing across renal function strata while maintaining pharmacokinetic/pharmacodynamic (PK/PD) target at

Cefiderocol Heteroresistance in NDM-Producing Enterobacterales: A Complex Polymicrobial Infection Successfully Salvaged With Ceftazidime-Avibactam Plus Aztreonam.

BACKGROUND Heteroresistance among New Delhi metallo-ß-lactamase (NDM)-producing carbapenem-resistant Enterobacterales (CRE) is an emerging but underrecognized mechanism of treatment failure. Although cefiderocol is a preferred therapy for metallo-ß-lactamase-producing organisms, real-time development of discordant susceptibility within the same patient remains rarely described. We present a case demonstrating site-specific cefiderocol heteroresistance with divergent microbiologic responses, highlighting critical diagnostic and therapeutic implications. Synergistic effect of ceftazidime-avibactam and aztreonam combination against carbapenem resistant Klebsiella spp., E.coli and Pseudomonas spp.

Carbapenem-resistant Gram-negative bacilli (CR-GNB) represents an escalating threat to global public health due to its resistance to last-resort of antibiotics. One potential treatment strategy to combat these resistant microorganisms is the combination therapy like ceftazidime-avibactam (CZA) and aztreonam (ATM). The study evaluated the in vitro activity of the CZA-ATM combination against 156 CR-GNB including Klebsiella spp., Escherichia coli (E. coli), and Pseudomonas spp. using E-strip disk diffusion, broth disk elution (BDE), and disk stacking plus micro-elution (DSE) method at the Departm

Evaluation of gradient strip diffusion for susceptibility testing of aztreonam-avibactam in metallo-β-lactamase-producing Enterobacterales.

The emergence of metallo-β-lactamase (MBL)-producing Enterobacterales presents unique clinical treatment challenges. Recently developed β-lactam/ β-lactamase inhibitor combination agents, while effective against other carbapenemase-producing organisms, are notably ineffective against MBL producers. While MBLs do not hydrolyze monobactams (aztreonam), many MBL-producing organisms are resistant to aztreonam through alternate mechanisms, leaving cefiderocol as the sole monotherapy treatment option recommended for MBL producers. Recent guidelines for the treatment of MBL-harboring organisms have a

Characterization of Escherichia coli NDM isolates with decreased susceptibility to aztreonam/avibactam: role of a novel insertion in PBP3.

OBJECTIVES: The spread of NDM-1 amongst Enterobacteriaceae has highlighted a significant threat to the clinical management of serious infections. The combination of aztreonam and avibactam, a non-β-lactam β-lactamase inhibitor, may provide a much-needed therapeutic alternative. This combination was potent against most NDM-containing Enterobacteriaceae, although activity was diminished against many Escherichia coli isolates. These E. coli isolates were characterized to elucidate the mechanism of decreased susceptibility to aztreonam/avibactam. METHODS: MIC determinations were performed using b

Global Dissemination of blaKPC into Bacterial Species beyond Klebsiella pneumoniae and In Vitro Susceptibility to Ceftazidime-Avibactam and Aztreonam-Avibactam.

The Klebsiella pneumoniae carbapenemase (KPC), first described in the United States in 1996, is now a widespread global problem in several Gram-negative species. A worldwide surveillance study collected Gram-negative pathogens from 202 global sites in 40 countries during 2012 to 2014 and determined susceptibility to β-lactams and other class agents by broth microdilution testing. Molecular mechanisms of β-lactam resistance among carbapenem-nonsusceptible Enterobacteriaceae and Pseudomonas aeruginosa were determined using PCR and sequencing. Genes encoding KPC enzymes were found in 586 isolates

[Transfer of resistance to 3rd generation cephalosporins and aztreonam in strains of Klebsiella pneumoniae producing extended spectrum beta-lactamases].

The authors demonstrated the transferability of antibiotic resistance genes in nosocomial strains of Klebsiella pneumoniae, isolated from newborn children at the Paediatric University Hospital in Bratislava. Strains were resistant to cefotaxime, ceftazidime and aztreonam. The determinants of resistance (carbenicillin, cephaloridine, cefotaxime, ceftazidime and aztreonam) were transferred to recipient strains of Escherichia coli K-12 and Proteus mirabilis P-38. The transfer of resistance determinant from donor strains was demonstrated by the analysis of the resistance spectrum in transconjugant

Synergistic efficacy of ceftazidime/avibactam and aztreonam against carbapenemase-producing Pseudomonas aeruginosa: insights from the hollow-fiber infection model.

BACKGROUND: Combination therapy is an attractive therapeutic option for extensively drug-resistant (XDR) Pseudomonas aeruginosa infections. Existing data support the combination of aztreonam and ceftazidime/avibactam (CZA) against class serine-β-lactamase (SBL)- and metallo-β-lactamase (MBL) - producing Enterobacterales. However, data about that combination against SBL- and MBL-producing P. aeruginosa are scarce. The objective of the study was to assess the in vitro activity of CZA and aztreonam alone and in combination against SBL- and MBL-producing XDR P. aeruginosa isolates. METHODS: The c

New Multitarget Inhibitor Potentiates Antimicrobial Activity of Aztreonam against Metallo-β-lactamase-Producing Pseudomonas aeruginosa Including IMP-Types.

Combination of aztreonam (ATM) and avibactam (AVI) was proven to be synergistic against metallo-β-lactamase (MBL)-producing Enterobacteriaceae. However, in the case of Pseudomonas aeruginosa (PA), ATM-potentiation by AVI can hardly be achieved because PA has complex resistance mechanisms. In this study, we identified 3,7-bis-O-substituted difluoroquercetin derivatives 9 and 12 as new ATM-potentiating agents against the MBL-producing PAs through the simultaneous inhibition of NDM-1, OXA-10, and efflux pumps.
