Characterization of the organ donor with bacteremia.

INTRODUCTION: The presence of bloodstream infection in the donor is a common finding that could be transmitted to the recipient. To safely expand the donor pool, we examined its relevance. MATERIALS AND METHODS: We described the clinical characteristics of organ donors grafted in our center between 1997 and 2006 who had bacteremia detected in blood cultures obtained during organ procurement. RESULTS: Among 1353 organ donors, 75 were non-heart-beating donors type II and the others brain-dead donors. Only 186 donors (14%) showed bacteremia during retrieval. This mean age was 49.8 years (range

Clostridial bacteremia in the community hospital.

Anaerobic infections are not commonly studied in the community hospital. The aim of this study was to determine demographic factors, the portals of entry and underlying disorders for clostridial bacteremia and to determine whether appropriate (recommended) treatment is effective. Medical records were reviewed for 42 patients with clostridial bacteremia at 1 Florida, USA, hospital and 4 Dayton, Ohio, USA, hospitals. Fourteen (33.3%) of the patients had clostridial micro-organisms that were isolated in cultures with polymicrobial isolates. Only about half of the patients had fever at the onset o

Bacteremia in patients with hematologic malignancies.

The decision regarding the appropriate empirical antibiotic therapy for bacteremia in patients with hematologic malignancies must be individualized in each institution, by taking into account variations in the patterns of microbial isolates and their resistance profiles. Microbial isolates and antibiotic resistance patterns must be taken into account at each hospital. A shift from bacteremia due to infections caused by predominantly gram-negative organisms to predominantly gram-positive organisms in patients with hematologic malignancies has been noted in reports from other countries. We inves

Bacteremia in conjunction with endodontic therapy.

This study characterizes oral microorganisms believed to have spread from the root canal into the blood stream during and after endodontic therapy of teeth with Asymptomatic apical periodontitis. Microbiological samples were taken under aseptic conditions from the root canal of 26 single-rooted teeth in 26 patients. In the endodontic treatment of 13 of the patients (Group 1), the first 3 reamers, sizes 15, 20 and 25, were used to a level 2 mm beyond the apical foramen. In the other 13 patients (Group 2), the instrumentation ended inside the root canal 1 mm short of the apical foramen. Blood sa

Occult bacteremia: is there a standard of care?

The evaluation and management of patients with occult bacteremia is controversial. The purpose of this study was to define the prevailing practices in the emergency management of occult bacteremia. Short, anonymous surveys were mailed to all 517 members of the Section on Emergency Medicine at the American Academy of Pediatrics. Three hundred six (59%) of those surveyed returned completed questionnaires. Eleven different temperature cutoff points are used, and 105 (34%) consider occult bacteremia in patients with temperature above 39 degrees C. Seventeen different age intervals are used to defi

Incidence of bacteremia at dilation and curettage.

The American Heart Association does not recommend prophylactic antibiotics for patients undergoing dilation and curettage (D & C) except in the case of septic abortion or the presence of prosthetic valves. This recommendation is based on a retrospective analysis of case reports of infectious complications of D & C. To date we are unaware of prospective studies that answer this question. At a community hospital we studied 20 patients undergoing D & C for noninfectious indications. Aerobic and anaerobic blood cultures were performed before and during the D & C. All baseline cultures were negativ

Occurrence of bacteremia in hematologic patients.

In the present study we reviewed eighty-six episodes of bacteremia occurred in 60 neutropenic patients and thirty-one episodes occurred in 30 non-neutropenic patients. Twenty-four out of 60 neutropenic patients suffered from multiple episodes of bacteremia, while only one out of 30 non-neutropenic patients presented multiple episodes. In neutropenic patients, 29 episodes of bacteremia were polymicrobial, whereas only one non-neutropenic patient had polymicrobial bacteremia. Intravascular catheters were the most common source of bacteremia (23.2%) in neutropenic patients, as compared with infec

Bacteremia in patients with cirrhosis of the liver.

Infections are frequent in patients with liver cirrhosis, as their defenses against infectious agents are altered. But bacteremia occurring in cirrhotic patients has seldom been reported in the literature. From 1981 to 1986, we collected 197 cases with 228 episodes of bacteremia for this retrospective study. The incidence of bacteremia in cirrhotic patients was 8.8%; no significant difference was noted between cirrhotic patients with variant etiologies of HBV(+), HBV(-) and alcohol. But the incidence increased with the severity of the disease (1%, 4.8%, 17.1% in Child's A, B, C groups, respect

Erysipelothrix rhusiopathiae bacteremia in a horse.

Erysipelothrix rhusiopathiae serotype 5 was isolated from blood obtained antemortem from a horse with presenting problems of laminitis, uveitis, acute blindness, localized ventral edema and depression. The patient failed to respond to therapy and died 96 hours after the onset of clinical signs. Cultures of the lung postmortem yielded Erysipelothrix rhusiopathiae serotype 5, Beta-hemolytic Streptococcus sp., Escherichia coli, Proteus sp., and Klebsiella sp.

Bacteremia with group A streptococci in childhood.

Medical records of 60 patients with bacteremia caused by group A streptococci who were treated at the Yale-New Haven (Conn) Hospital from 1973 to 1986 and the Boston Children's Hospital Medical Center from 1977 to 1984 were reviewed. Seven children (12%) were immunocompromised, seven (12%) had varicella, and two (3%) had cavernous hemangiomas. Fifty-two children (87%) had an identifiable focus of infection. The most commonly documented sources of bacteremia were in the skin (22 children) and the respiratory tract (19 children). Metastatic foci of infection included osteomyelitis (nine children

Capnocytophaga bacteremia in the compromised host.

Capnocytophaga, a CO2-requiring gram-negative bacillus, is a recently recognized pathogen in the immunocompromised host. The authors present two cases with granulocytopenia, oral disease and bacteremia with Capnocytophage ochracea. The microbiology of this genus, its clinical association with periodontal disease, granulocytopenia and oral ulceration, and its implications for the compromised host are reviewed.

Bacteremia, endocarditis, and the Hancock valve.

Among 373 patients with porcine xenografts, there were 27 instances of exposure of the xenograft to bloodstream or endocardial infection in 22 patients. Nine patients underwent 10 separate insertions of xenografts for active infective endocarditis. There were no early infections or valve failures. Three patients returned with a late prosthetic valve endocarditis (PVE) due to a new infection. There were 6 instances of bacteremia early after xenograft valve insertion with no early infection, no valve dysfunction, and 1 instance of late PVE. Eleven patients had PVE on a porcine xenograft. Blood c

The absence of bacteremia during colonoscopy.

Seventy-nine blood cultures were obtained from 15 patients undergoing diagnostic colonoscopy. Aerobic and anaerobic culture technics showed no evidence of bacteremia during or after the procedure. The colonoscope was passed to the cecum in 12 of the 15 patients. The time of the procedure varied from 15-60 minutes with an average of 30 minutes. The results of this study supplies further evidence that bacteremia during colonoscopy is an infrequent occurrence.

Detection and quantitation of bacteremia in childhood.

Quantitative blood cultures were sought in 383 children, from whom routine blood cultures were obtained because of fever, by direct plating of 10 and 100 microliter blood onto solidified media. There were 14 positive cultures from 12 patients. These were 7 Hemophilus influenzae type b, 5 Streptococcus penumoniae, and 2 Staphylococcus aureus. The direct-plating technique permitted more rapid identification of positive cultures, and detected three episodes not identified by routine broth culture. Bacterial counts ranged from 20 to greater than 10(4) bacteria/ml blood. In the three cases of H. in

A case of Chryseobacterium taklimakanense bacteremia.

Chryseobacterium is an obligate aerobic, non-glucose-fermenting, Gram-negative bacillus belonging to the family Flavobacteriaceae. Chryseobacterium species have rarely been isolated from clinical samples. Chryseobacterium indologenes and Chryseobacterium gleum are the most frequently isolated species. Chryseobacterium taklimakanense was first reported in 2009. Reports of C. taklimakanense isolation from blood cultures are rare and no case reports exist, leaving no established susceptibility data. An 80-year-old man with multiple underlying conditions, including appendiceal cancer, chronic hear
