BACKGROUND: Cerebral vasospasm is the main cause of neurological mortality and morbidity following subarachnoid hemorrhage. Basilar artery vasospasm (BAVS) is associated with a high morbidity and may have multiple clinical presentations.
METHODS: We report the case of a 43 years-old man with BAVS presenting as a reversible locked-in syndrome (LIS) after stopping sedation.
RESULTS: The symptoms were successfully managed by intra-arterial infusion of vasodilators and balloon angioplasty. Magnetic resonance imaging did not reveal any brainstem lesion 48 h after the complication, demonstrating a

BACKGROUND: Intra-arterial thrombolysis may reduce mortality in acute basilar artery (BA) occlusion. We wanted to identify an easy-to-use pre-treatment MR-based predictor of clinical outcome in patients with acute BA occlusion treated with IAT.
METHODS: We analyzed 16 patients with acute BA occlusion prospectively. Vascular risk factors, as well as National Institutes of Health Stroke Scale (NIHSS) and Glasgow Coma Scale (GCS) scores were recorded on admission, together with a 10-point semiquantitative score reflecting the number of acute ischaemic brain lesions on diffusion-weighted imaging.

The aim of this study was to describe skew deviation and vertical nystagmus as the initial signs for basilar artery thrombosis, a life-threatening disease. A 51-year-old woman complained of vertical diplopia for more than 20 h. A computed tomography of the brain was normal, but subsequently the patient developed additional symptoms including nausea, ventilation problems (dyspnoea) and somnolence. Neuro-ophthalmological evaluation revealed a skew deviation and a vertical nystagmus. Magnetic resonance imaging allowed the diagnosis of basilar artery occlusion. An emergency intervention with cereb

Basilar artery (BA) stenting is an emerging technique with promising results in revascularization of severe arteriosclerotic stenoses. Because of the limitations associated with other noninvasive techniques, we applied transcranial color-coded sonography (TCCS) in the follow-up. Successful stent application in two cases of acute basilar occlusion caused by thrombosis on pre-existing severe midbasilar stenoses was initially confirmed by angiography. Later recanalization was controlled noninvasively by magnetic resonance angiography (MRA), computed tomographic angiography (CTA) and TCCS. Ultraso

Despite various presentations of vertebrobasilar artery ectasia, the true incidence and its distribution and the frequency with which it produces symptoms are still uncertain. We report on a case of Locked-in syndrome caused by an ectatic elongated basilar artery, which predisposed to thrombosis and brain stem infarction.

INTRODUCTION: Since 1966 when Levine and Payan observed that the Mongolian gerbil (Meriones unguiculatus) was unusually susceptible to cerebral infarction following occlusion of one or both common carotid arteries, the model has been used for the experimental study of cerebral ischemia.
OBJECTIVE: To assess the presence of anterior-posterior arterial communication in the Mongolian gerbil (Meriones unguiculatus).
MATERIAL AND METHODS: 26 animals were used and divided in the following groups: 1. Bilateral carotid ligation (n = 10); 2. Unilateral left carotid ligation (n = 5); 3. Unilateral car

BACKGROUND AND PURPOSE: This study was undertaken to examine the relationship between collateral flow and outcome after local intraarterial thrombolytic treatment for basilar artery thrombosis.
METHODS: Twenty-four patients with symptomatic basilar thrombosis were treated with intraarterial urokinase. Angiograms at the time of treatment were analyzed to characterize collateral flow. The number of posterior communicating arteries (PCoAs) and the degree of collateral filling of the basilar artery were then compared with symptom duration before treatment, with Glasgow Coma Scale (GCS) score at t

A 30-year-old female became comatose due to embolic occlusion of the basilar artery, caused by surgical injury to the origin of the vertebral artery during removal of a neurinoma in the upper thoracic paravertebral region. The basilar artery occlusion was treated by local fibrinolysis through a microcatheter. Two weeks later she recovered her consciousness but suffered mutism. Her speech disturbance was characterized by severe ataxic dysarthria known as "cerebellar mutism" but without cranial nerve paresis. The mutism gradually improved during the following 3 months. This is case of cerebellar

34 cases with infarcts of rostral basilar territory arteries, including posterior cerebral artery (PCA), rostral basilar artery (BA) and superior cerebellar artery (SCA) confirmed by CT and MRI were reported. The clinical manifestations might be divided into 3 groups according to the supplying artery: the cortical branch of PCA (21 cases), the perforated branch of PCA (22 cases) and the SCA (10 cases). However, combination was often seen. MRI is superior than CT in finding lesions of this region. The use of digital substraction angiograph, magnetic resonance angiography and transcranial Dopple

BACKGROUND AND PURPOSE: Without recanalisation, acute basilar artery (BA) occlusion has a mortality of 90%, which is reduced to 50% if recanalisation is achieved. Fast diagnosis of BA occlusion is necessary in order to start thrombolytic therapy without delay. We wanted to assess the role of CT angiography (CTA) in the diagnostic evaluation of suspected acute BA occlusion.
MATERIALS AND METHODS: Ten patients with clinically suspected BA occlusion were examined with conventional CT and spiral CT angiography. Spiral scanning extended from the foramen magnum to the tip of the basilar artery. For

We report a case of a dissecting basilar artery aneurysm, which presented with subarachnoid hemorrhage in a 34-year-old woman. Although the natural history has been described as severe with mortality approaching 80%, this patient did well and spontaneous healing was demonstrated on MR and angiography.

The authors describe 6 cases of giant aneurysm of the basilar artery diagnosed in the Neurology Department of the Hospital Clínico Universitario of Zaragoza over the last 8 years by cerebral CT, arteriography and NMR with the clinical, diagnostic, and evolutive features of the same being reported. In 2 cases the clinical manifestations initiated as subarachnoid hemorrhage, in another two as AIT, in the fifth case as cerebral infarction and in the sixth case with pseudotumoral clinical manifestations with a necropsy study being available in the latter case. Three of the 6 patients died and of t

The dolichoectatic basilar artery was found in 23 cases during a 10-year period. The 19 males and 4 females ranged in age from 30 to 69 years (mean: 55 years). Hypertension was noted in 17 patients. In seventeen (74%) of the present cases this anomaly could be visualized with CT scan. Seven patients (30%) presented with pontine infarction, which was identified on CT scan in all cases. Vertebro-basilar insufficiency was found in four patients. One patient had transient ischemic attacks. There were facial spasms in four patients and impairment of the lower cranial nerves in one. One patient exhi

Three patients, aged 65, 70, and 80 years, respectively, with hypoplastic basilar artery are presented. To our knowledge no similar case has been reported in the literature till now. In one of the cases, the hypoplasia of the basilar artery was an incidental finding. The other two patients had transient neurological symptoms considered characteristic of circulatory insufficiency in the vertebrobasilar system. One of the latter two patients died of acute myocardial infarction and the other patient died after developing occlusion in the cervical part of the right internal carotid artery. The exa

A patient rendered acutely quadriplegic in an automobile accident was shown by angiography to have occlusion of the left vertebral artery. One month later, he abruptly became unconscious, apneic and died. Autopsy showed an organised thrombus in the left vertebral artery and a fresh thrombus occluding the entire basilar artery. This case differs from previously reported cases of traumatic vertebral artery thrombosis wherein symptoms of brain stem infarction were more immediately evident.

A 57 year-old white hypertensive male presented with a five-month history of progressive dementia and a clinical picture suggestive of normal pressure hydrocephalus. A CT scan and vertebral angiogram demonstrated an ectatic basilar artery obstructing the posterior third ventricle; the resulting hydrocephalus was relieved by a ventriculo-peritoneal shunt and the patient's symptoms resolved completely.

Giant and fusiform aneurysms of the basilar artery (BA) are difficult to manage surgically, and they still present a challenge for the neurosurgeon despite progress in posterior fossa surgery. Guglielmi detachable coils have provided an alternative approach to treatment, but they have their own difficulties. We present a series of three patients with BA aneurysms treated with endovascular occlusion of the aneurysm and basilar trunk. Our experience suggests that, in select cases, BA occlusion may be not only safe but also necessary for the effective treatment of these aneurysms.
