Unicentric Castleman's lymphadenopathy presenting with Behçet's syndrome: a case report.

We describe a male patient with Behcet's syndrome and Castleman's lymphadenopathy. He was affected by severe oral and genital ulcers, bilateral uveitis, necrotic purulent nodules on both hands. A CT scan of the mediastinum showed the presence of a mediastinal mass. On the basis of histological and immunohistological findings, a diagnosis of Castleman's disease, angiofollicular type, was formulated. The lymphadenopathy was unicentric. After the surgical excision of the mediastinal mass (January, 1989) until now, Behcet's syndrome is in complete remission with a low dosage of prednisone.

Superior vena cava syndrome without thrombosis revealing Behçet's disease: Two cases.

Superior vena cava (SVC) thrombosis is a rare but well-recognized manifestation of Behcet's disease, whereas SVC syndrome due to vasculopathy, without evidence of thrombosis, has only been described a single time in the literature. We report two cases of this exceptional association. The two patients presented with swelling of the face and oral aphtosis, the SVC stenosis was evidenced by angio-CT and classical cavography. In each case, a combination of anticoagulant and immunosuppressive (corticosteroids and azathioprine) treatments allowed a significant improvement of SVC diameter. In one of

A case of severe MAGIC syndrome treated successfully with the tumor necrosis factor-alpha inhibitor infliximab.

We report a patient with an overlap of relapsing polychondritis and Behcet disease whose cartilaginous inflammation and genital lesions were refractory to corticosteroids and immunosuppressants, but fully remitted with the tumor necrosis factor-alpha inhibitor, infliximab. We believe this case represents the first report of a response to a tumor necrosis factor inhibitor in a patient with mouth and genital ulcerations with inflamed cartilage syndrome.

Manifestations of Neuro-Behcet's disease. Report of two cases and review of the literature.

Behcet's disease (BD) is a rare condition, more commonly seen in Turkey, the Middle East, the Mediterranean, and the Far Eastern countries. It is a multifaced disease involving the central nervous system (CNS), skin, joints, intestines and blood vessels. CNS involvement occurs in 10-25% of the patients with BD. We report two cases with Neuro-Behcet's disease. Both patients were females and they were in their second decade of life (early forties) when they suffered from relapsing retinitis and uveitis which led to blindness, despite treatment with steroids. They presented with clinical signs of

[Differential diagnosis of chronic inflammatory diseases of the central nervous system. Cerebrospinal fluid diagnosis and immunological parameters].

A number of neurological syndromes may be evoked by involvement of the nervous system due to systemic diseases such as lupus erythematosus, sarcoidosis, Behcet's disease, and Sjogren's syndrome. Because of different treatment strategies, it is important to distinguish between these different diseases. Neither clinical signs nor additional analyses such as serological findings or cerebrospinal fluid analysis are able to differentiate between the diseases with certainty. Nevertheless, diagnosis may finally be made taking all findings together. Here we compare typical clinical and cerebrospinal f

Mutation analysis of the TNFAIP3 in A20 haploinsufficiency: A case report.

INTRODUCTION: Haploinsufficiency of A20 (HA20) is a novel genetic disease presented by Zhou et al in 2016. A20 is a protein encoded by TNFAIP3. Loss-of-function mutation in TNFAIP3 will trigger a new autoinflammatory disease: HA20. HA20-affected patients may develop a wide range of clinical manifestations, such as Behcet disease, rheumatoid arthritis, rheumatic fever, juvenile idiopathic arthritis, and systemic lupus erythematosus. HA20 is rarely reported, thus remaining far from thoroughly understood. Sixty-one cases of HA20 have been reported worldwide, among which 29 cases were diagnosed wi

[Demyelinating process in systematic rheumatoid diseases (review)].

Multiple sclerosis (MS), systemic lupus erythematosus (SLE), antiphospholipid syndrome (APS), Sjogren's syndrome (SS) and Behcet disease (BD) are chronic, immune-mediated disorders affecting young adults, the pathogenesis of which is still largely unknown. MS and other demyelinating processes are sometimes difficult to differentiate from the CNS involvement in systemic rheumatic diseases (SRD). An acute isolated neurological syndrome presents the greatest diagnostic problem, since it is common not only in MS, but can also be the only feature or first manifestation in SLE, APS, SS, and BD. [Behcet's disease--clinical picture and treatment].

Behcet's disease is a systemic inflammatory disease of unknown etiology which usually occurs as a trait of symptoms: aphatous stomatitis, genital ulcerations and eye disease. Characteristic manifestations are frequent: erythema nodosum, arthralgias, arthritis, myalgias, phlebothrombosis and nervous system disorders as well as disorders of other organs and systems. Behcet's disease was discovered in 1937 for the first time by dermatovenerologist Hulusi Behcet (1889 - 1948) after whom it was named Behcet's disease or Behcet's syndrome. At the beginning of the disease the diagnosis is uncertain b

Neutrophilic phlebitis is characteristic of intestinal Behçet's disease and simple ulcer syndrome.

AIM: To characterize the histological features of intestinal Behcet's disease and simple ulcer syndrome and to clarify the possible mechanisms involved in their development by analysing the type of inflammatory infiltrates in the diseased intestine and the expression of adhesion molecules on endothelial cells. METHODS AND RESULTS: Tissue samples from 10 patients diagnosed as having intestinal Behcet's disease or simple ulcer syndrome were studied. Immunohistochemical studies were performed on paraffin-embedded tissue using a labelled streptavidin-biotin method. In all cases, phlebitis was rem

Immune-mediated conditions affecting the brain, eye and ear (BEE syndromes).

The triad of central nervous system symptoms, visual disturbance and hearing impairment is an oft-encountered clinical scenario. A number of immune-mediated diseases should be considered among the differential diagnoses including: Susac syndrome, Cogan syndrome or Vogt-Koyanagi-Harada disease; demyelinating conditions such as multiple sclerosis or neuromyelitis optica spectrum disorder; systemic diseases such as systemic lupus erythematosus, Sjögren syndrome or Behcet disease and granulomatous diseases such as sarcoidosis. In this article, we coin the term 'BEE syndromes' to draw attention to

Tumor necrosis factor alpha inhibitors for the treatment of dermatologic diseases.

Tumor necrosis factor alpha (TNFalpha) is involved in cell differentiation, mitogenesis, cytotoxic responses, inflammation, immunomodulation, and wound healing. Because of its numerous roles, it was thought that inhibition of TNF may aid in the treatment of certain dermatologic diseases such as psoriasis, hidradentitis suppurativa, pyoderma gangrenosum, Behcet 's syndrome, and graft versus host disease. The efficacy of these agents has proven impressive and short-term side effects have been few and relatively benign.

Immune response genes in uveitis.

Uveitis is defined as an intraocular inflammation induced by different etiologies. Though the precise pathogenesis is still unknown, accumulating evidence shows that both innate and adaptive immune responses may be predominant mechanisms involved in the development of uveitis. Toll-like receptors have been shown to be expressed in the human eye and play an important role in infectious uveitis. The NOD proteins, expressed mainly in the cytosol by APCs, recognize the products of bacteria and participate in the development of uveitis. HLA genes have been associated with some uveitis entities, inc

Behcet's disease presenting as malignant hypertension induced by renovascular hypertension.

Hypertension is an uncommon manifestation of Behcet's disease, which is also an uncommon cause of renovascular hypertension. We herein report a case of malignant hypertension associated with unilateral renal artery stenosis due to vascular Behcet's disease. A 19-year-old man, who had no significant medical history, was referred to ophthalmology at our hospital because he was suspected to have uveitis and Vogt-Koyanagi-Harada syndrome. In addition to poor eyesight, he had been aware of a fever, loss of appetite, and weight loss for a month. He was admitted with markedly elevated blood pressure

[Severe flare of Behcet's disease with intense mucucutaneous manifestations].

Behcet's disease is a multisystemic inflammatory disease with various manifestations at the skin level, especially in the form of acne-like lesions or erythema nodosum. Susceptibility to this disease is strongly linked to the presence of the HLA_B51 allele, although certain environmental factors such as infectious agents have also been implicated in its pathogenesis. We present the case of a patient with Behcet's disease who developed a severe flare characterized by lesions similar to those seen in Sweet's syndrome. Histologically we observed granulomatous vasculitis.

Mucosal lesions may be a minor complication of SAPHO syndrome: a study of 11 Japanese patients with SAPHO syndrome.

Since the term synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome was proposed by Chamot et al. (Rev Rhum Mal Osteoartic 54:187-196, 1987), clinical reviews concerning this syndrome have been mainly reported from Europe. We carried out a retrospective analysis of 11 Japanese patients with SAPHO syndrome, and reviewed the clinical features of our series in comparison with those in a European large case study. In this study the major features of SAPHO syndrome were chronic osteitis of the anterior chest wall and pustulotic arthro-ostitis with middle age onset, and mucosal lesions s

Certain Autoimmune Manifestations Are Associated With Distinctive Karyotypes and Outcomes in Patients With Myelodysplastic Syndrome: A Retrospective Cohort Study.

Autoimmune manifestations (AIMs) are common in patients with myelodysplastic syndrome (MDS). This study aimed to investigate whether AIMs are associated with a specific cytogenetic abnormalities and worse survival in patients with MDS. A total of 67 MDS patients with AIMs and 134 age- and sex-matched MDS patients without AIMs, all of whom received medical care at Seoul National University Hospital from January 2000 through July 2014, were enrolled. The clinical features, chromosomal abnormalities, and outcomes were examined. The effect of AIMs on mortality was estimated after adjusting for age
