BACKGROUND: The perception of illness and health-seeking behaviours, including rabies differ from one culture to another. Depending on the cultural setting of the society in which people live, the definition of the causal factors of illness may range from natural biological causes to supernational causes which greatly influence subsequent health-seeking behaviour. To ensure best health practices and plan effective health interventions for the control of dog-mediated-human rabies, we explored how school children and adult communities perceive and respond to illnesses, including rabies in southe

BACKGROUND: High mortality rates following out-of-hospital cardiac arrest (OHCA) make identification of its etiology vital for prevention. In low-resource settings, limited access to diagnostics and clinical autopsy often prevents definitive conclusions. This study aimed to describe the presumed etiology of OHCA cases presenting to a tertiary emergency department (ED) in Bhutan.
METHODS: A prospective cohort study was conducted from August 1, 2023, to July 31, 2024, in the ED of the National Referral Hospital, Bhutan. All patients presenting with OHCA were included and followed until hospital

BACKGROUND: Rabies is endemic in southern Bhutan, associated with 1-2 human deaths and high post exposure prophylaxis (PEP) costs annually. Evaluation of clinicians' management of human cases potentially exposed to rabies could contribute to improving PEP prescribing practices to both reduce unnecessary costs associated with PEP and reach the target of zero human deaths due to rabies by 2023.
METHODS: A cross-sectional survey of 50 clinicians' management of human cases potentially exposed to rabies was conducted in 13 health centers in high-rabies-risk areas of Bhutan during February-March 20

In Bhutan, Capture-Neuter-Vaccinate-Release (CNVR) programs have been implemented to manage the dog population and control rabies, but no detailed evaluation has been done to assess their coverage and impact. We compared estimates of the dog population using three analytical methods: Lincoln-Petersen index, the Chapman estimate, and the logit-normal mixed effects model, and a varying number of count periods at different times of the day to recommend a protocol for applying the mark-resight framework to estimate free-roaming dog population abundance. We assessed the coverage of the CNVR program

Although the Numerical Rating Scale (NRS) is commonly recommended for use in research and clinical practice internationally, recent research in non-western countries questions its utility in some countries. We aimed to evaluate validity and utility of four commonly used pain intensity measures in a sample of participants with musculoskeletal pain in Bhutan. Two hundred forty-seven adults with musculoskeletal pain visiting three hospitals rated their one-week recalled worst and average pain using all four scales, and indicated their preferred scale. For analysis, participant responses were clas

The diversity and prevalence of canine vector-borne pathogens (VBPs) in Bhutan have to date remained unexplored, whilst recent epidemiological surveys in other South Asian nations have found diseases caused by VBPs to be rife in local dog populations. Importantly, many of such VBPs can infect people as well, with a building body of evidence identifying potentially zoonotic rickettsial organisms infecting humans in Bhutan. Given the lack of data on canine pathogens in Bhutan we employed a suite of deep-sequencing metabarcoding methods using Oxford Nanopore Technologies' MinION™ device to holist

The unprecedented experience of national lockdowns and uncertainty of academic career due to the COVID-19 pandemic has multifaceted impacts on mental health among university students worldwide. This study determined its impact on depression and anxiety level, and associated risk factors among engineering students studying at College of Science and Technology (CST), Phuentsholing, Bhutan during the first lockdown in the country. Self-reported depression and anxiety levels were assessed using Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7) respectively. Data was c

BACKGROUND: There are approximately 35,000 human deaths from rabies in Asia annually. Rabies can be prevented through timely post-exposure prophylaxis (PEP) consisting of wound washing, rabies vaccine, and in some cases, rabies immunoglobulin (RIG). However, access to rabies PEP often remains limited to urban areas and is cost-prohibitive. There is little information on procurement, distribution, monitoring, and reporting of rabies PEP.
METHODS: We interviewed key informants in the public sector from various levels in Bangladesh, Bhutan, Cambodia, and Sri Lanka between March 2017 and May 2018

BACKGROUND: Urine sampling may offer a less invasive solution than cervical sampling to test for human papillomavirus (HPV) for HPV vaccine impact monitoring.
METHODS: Paired samples of urine and exfoliated cervical cells were obtained for 89 women with history of high-risk (HR) HPV-positive normal cytology in Bhutan. Urine sampling protocol included self-collection of first-void urine immediately into a conservation medium and procedures to optimize DNA yield. Colposcopical abnormalities were biopsied. Two HPV assays were used: a multiplex type-specific PCR (E7-MPG) and a less analytically s

In Bhutan, HIV testing uptake among key populations remains low, with most cases diagnosed late. Evidence shows index testing and partner notification services (PNS) enhance diagnosis among partners of people living with HIV (PLHIV). However, traditional PNS are limited by HIV related stigma, fear of disclosure, and partner reluctance to visit health facilities. Integrating HIV self-testing (HIVST) within the PNS provides an alternative for earlier diagnosis. This qualitative study used in-depth interviews to explore the feasibility and acceptability of index testing using HIVST among PLHIV. D

INTRODUCTION: Timely admission and treatment of critically ill patients to Intensive Care Units (ICU) is shown to reduce premature mortality. However, low- and middle-income countries face many challenges in providing ICU services. This study describes the profile of patients admitted to ICU and their outcomes at the Central Regional Referral Hospital in Bhutan.
METHOD: This was an observational study with a review of records of patients from 2021 to 2023. The data were extracted into a pro forma, entered into EpiData Entry 3.1, and analysed in STATA 18.
RESULTS: There were 287 patients admi

Bovine tuberculosis (bTB) is a globally neglected zoonotic disease. In several low- and middle-income countries (LMIC), limited bTB surveillance and technologies to minimise milk-borne zoonoses put dairy consumers at high risk of bTB infection. Surprisingly, LMIC consumers are seldom investigated, which represents a significant knowledge gap that undermines existing efforts to mitigate zoonotic tuberculosis. We conducted a cross-sectional study in Bhutan where bTB control measures are lacking and raw dairy is widely consumed to assess dairy consumers' awareness and their knowledge, attitudes,

Tobacco use remains a major public health challenge globally. In Bhutan, despite the implementation of strict tobacco control measures, the tobacco uses among the students continue to be alarmingly high compared to neighboring countries. This study aims to analyze the trends and correlates of tobacco use among students in Bhutan, utilizing the nationally representative Global Youth Tobacco Survey (GYTS) data from multiple survey years. Secondary analyses of GYTS data collected during 2004-2019 with 12,594 students aged 11-18 years were employed. Use of tobacco was defined as either smoked or s
