We describe a 31-year-old patient with missed abortion, thrombocytopenia, and clinical, laboratory, and radiologic evidence of hepatic infarction. On evaluation, she was found to have the lupus anticoagulant. The association between enhanced thrombosis and the lupus anticoagulant is discussed, and previously reported thrombotic complications are described. The etiology, clinical course, and radiologic features of liver infarction are summarized, and the importance of recognizing and treating this form of hypercoagulability is stressed. To our knowledge, this is the first description of liver i

A major complication associated with prosthetic heart valves is valve thrombosis and embolization. Depending on the valve design these complications should be almost eliminated by anticoagulants or aspirin. However, valve thrombosis and embolism may occur despite apparently adequate anticoagulation. The question is whether these thrombi arise as a result of local factors such as the foreign surface and disturbance of flow at the valve site, or as a result of alteration of the hemostatic balance within the blood favoring thrombus formation. Such a disturbance, known as hypercoagulability, is th

The levels of coagulation factors II + VII + X and of blood platelets (thrombocytes) as well as of cytokines and soluble cytokine receptors were studied in the patients with malaria or meningococcal infections. The coagulation factors were decreased particularly in the meningococcal patients, while thrombocytes were lowest in the Plasmodium falciparum malaria patients. There was no correlation between factors II + VII + X and thrombocytes, but plasma levels of coagulation factors II + VII + X were found to correlate inversely with levels of soluble interleukin-2 receptor (sIL-2R) and soluble t

Hemophiliacs are X-linked inherited bleeding disorders. The treatment of hemophilia is based on periodic replacement treatment with coagulation factor products to avoid the development of hemophilic arthropathy, as well as to control fatal bleeding. However, because of the coagulation factor's incredibly short half-life, the medication must be often given intravenously throughout one's life. Extended half-life coagulation factor preparations have lowered the burden of treatment by enhancing pharmacokinetics. However, the patients remain at risk of developing inhibitors to significantly interfe

Streptococcus oralis (S. oralis) has been recognized as a fatal pathogen to cause multiorgan failure by contributing to the formation of microthrombus. Coagulation and fibrinolysis systems have been found under the control of circadian clock genes. This study aimed to explore the correlation between BMAL1 and coagulation factor biosynthesis in S. oralis infection. Mice were administered S. oralis to induce sepsis, and HepG2 cells were also infected by S. oralis. The expression of BMAL1 of hepatocytes was downregulated in the S. oralis infection group, leading to the downregulation of coagulati

Coagulation test results are influenced by many pre-analytical variables, including clinical sample storage conditions. This study investigated the effects of storage time and temperature on coagulation factor (FII, FV, FVII, FX, FVIII, FIX, FXI, FXII, and FXIII) and natural anticoagulant (antithrombin, protein C, and free protein S) activities. Blood samples were collected from 78 healthy individuals, and 3 samples were used for each coagulation factor and natural anticoagulant. Plasma was tested after centrifugation and storage at room temperature (18 to 25 °C) or refrigerated (2 to 8 °C) fo

The standard hemostasis workup [quick time (QT), and activated partial thrombin time (APTT)] is very commonly prescribed but its interpretation is often difficult for practitioners who are not specialized in hemostasis. Here, we review the principles of the diagnostic approach to these tests. Only a very basic knowledge of the coagulation cascade is necessary to identify which clotting factor tests to prescribe and to interpret the results. Deficiency in several clotting factors suggests liver dysfunction, disseminated intravascular coagulation (DIC) or vitamin K deficiency. If a single factor

Coagulation studies were performed on samples taken from day 6 to day 22 after the onset of symptoms from 32 patients with Argentine hemorrhagic fever, grouped by clinical form into 17 mild cases, nine moderate cases, and six severe cases. The profiles of the coagulation factors during the course of disease were similar in all patients. Partial thromboplastin time activated with kaolin was prolonged in the early stages (78.0 +/- 5.8 sec on day 6 and 47.5 +/- 1.9 sec on day 14). Values for factors II and VII-X remained in the normal range, but factor V activity was greatly enhanced (257.6% +/-

The deficiency of fibrinogen, prothrombin, factor V (FV), FVII, FVIII, FIX, FX, FXI, and FXIII, called rare coagulation disorders (RCDs), may result in coagulopathies leading to spontaneous or posttrauma and postsurgery hemorrhages. RCDs are characterized by a wide variety of symptoms, from mild to severe, which can vary significantly from 1 disease to another and from 1 patient to another. The most typical symptoms of all RCDs are mucosal bleedings and bleeding at the time of invasive procedures, whereas other life-threatening symptoms such as central nervous system bleeding and hemarthroses

Hemophilia B is a rare blood coagulation disorder. Complications such as bleeding and hematoma can cause necrosis of flaps, wound disruption, and the disturbance of wound healing. In particular, guidelines for flap operations in hemophilia B patients have still not been defined, and case reports are rare. We reconstructed the heel of a 41-year-old male hemophilia B patient using a reverse sural artery flap operation. The patient presented with mild hemophilia, having 27% of the normal value of coagulation factor IX. Coagulation and the changing value of the coagulation factor were regularly me

BACKGROUND: Thromboelastography (TEG) is a widely utilized clinical testing method for real-time monitoring of platelet function and the thrombosis process. Lipid metabolism disorders are crucial risk factors for thrombosis. The lipid metabolism characteristics of hamsters resemble those of humans more closely than mice and rats, and their relatively large blood volume makes them suitable for studying the mechanisms of thrombosis related to plasma lipid mechanisms. Whole blood samples from golden Syrian hamsters and healthy humans were obtained following standard clinical procedures. TEG was e

Vitamin K is well known for its role in the synthesis of a number of blood coagulation factors. During recent years vitamin K-dependent proteins were discovered to be of vital importance for bone and vascular health. Recommendations for dietary vitamin K intake have been made on the basis of the hepatic requirements for the synthesis of blood coagulation factors. Accumulating evidence suggests that the requirements for other functions than blood coagulation may be higher. This paper is the result of a closed workshop (Paris, November 2002) in which a number of European vitamin K experts review

The objectives of the present study were to evaluate the analytical sensitivity of the recently developed Clot Formation and Lysis (CloFAL) global assay for factor VIII (FVIII) deficiency, both in vitro and ex vivo, to determine whether this global assay is influenced by FVIII inhibitors, and to investigate the coagulative response to FVIII replacement in haemophilia A patients using the CloFAL assay in comparison with FVIII activity. Among adults and children alike, the CloFAL assay coagulation index (CI) was significantly decreased in FVIII-deficient vs. healthy subjects (adults median CI: 2

Rabbits were given polyclonal anti-tissue factor (TF) immunoglobulin G (IgG) before an injection of endotoxin to test the hypothesis that TF triggers disseminated intravascular coagulation (DIC) after endotoxin. The rabbits had been prepared with cortisone to develop DIC after one injection of endotoxin. Anti-TF IgG substantially reduced the falls in fibrinogen, factors V and VIII, and platelets noted in control rabbits given preimmune IgG before endotoxin. At autopsy 24 hours later, fibrin was present in glomerular capillaries of 4 of 5 control rabbits, but in none of 11 rabbits given anti-TF

BACKGROUND: Postpartum hemorrhage (PPH), a major cause of maternal mortality, has several known risk factors but frequently occurs unexpectedly. PPH incidence and related maternal morbidity and mortality are rising worldwide.
OBJECTIVE: To evaluate the impact of defined prepartum blood coagulation parameters on postpartum blood loss.
METHODS: This single-center, prospective cohort study analyzed prepartum activities of coagulation factors II and XIII and fibrinogen levels in 1300 women. Blood samples were obtained at labor onset and analyzed only after the last patient had delivered, to prev

BACKGROUND: Circulating levels and functionality of coagulation- and fibrinolysis-related proteins are critical in bleeding and thrombosis. Traditional assays lack multiplexing and adequate standardization and require substantial plasma volumes. Quantitative protein mass spectrometry (QPMS) enables multiplexed protein quantitation, addressing clinical and research gaps.
OBJECTIVES: This study aimed to develop and validate a QPMS test for multiplexed quantitation of 5 coagulation proteins.
METHODS: A QPMS test was developed to measure plasma fibrinogen (FBG), von Willebrand factor (VWF), fact
