Anatomical factors associated with overuse sports injuries.

Overuse injuries develop when repetitive stress to bone and musculotendinous structures damages tissue at a greater rate than that at which the body can repair itself. A combination of extrinsic factors, such as training errors and environmental factors, and intrinsic or anatomical factors, such as bony alignment of the extremities, flexibility deficits and ligamentous laxity, predispose athletes to develop overuse injuries. Malalignant of the lower extremity, including excess femoral anteversion, increased Q angle, lateral tibial torsion, tibia vara, genu varum or valgum, subtalar varus and e

Dorsally displaced fractures of the distal radius: a critical appraisal of the DRAFFT (distal radius acute fracture fixation trial) study.

The results of the DRAFFT (distal radius acute fracture fixation trial) study, which compared volar plating with Kirschner (K-) wire fixation for dorsally displaced fractures of the distal radius, were published in August 2014. The use of K-wires to treat these fractures is now increasing, with a concomitant decline in the use of volar locking plates. We provide a critical appraisal of the DRAFFT study and question whether surgeons have been unduly influenced by its headline conclusions.

Stimulation of bone union with dynamic beams of Konzal's "R" external fixator.

BACKGROUND: Treatment of septic long bone non-union remains a complex therapeutic problem. External stabilisation with Konzal's "R" fixator has been used in the Orthopaedic Department of CPME for years and allows for rigid stabilisation of bone fragments and good mutual alignment. Tried and tested in the treatment of osteitis, the fixator, however, offers limited possibilities for dynamisation and interfragmental compression. The following article presents a modernised design of Konzal's "R" with dynamic beams. MATERIAL AND METHODS: The efficacy of dynamisation of the fixator with the moderni

[Femoral head epiphyseolysis].

Slipped capital femoral epiphysis could be regarded as exceeding of a physiological process in puberty in which overweight of the patients seems to be the essential factor. Early clinical and radiological diagnosis are important to perform simple surgical procedures stopping further lysis and avoiding later osteoarthritis. Fixation of the femoral epiphysis with K.-wires are emphasized worldwide because of the lower complication rate. In case of severe slip osteotomies for realignment are more problematic. Long term follow-up studies have shown poor results mostly because of chondrolysis and fe

Minimizing the complications of intramedullary nailing for distal third tibial shaft and metaphyseal fractures.

BACKGROUND: The complications of intramedullary nailing of distal third tibial shaft and metaphyseal fractures have a direct impact on ankle and hind foot function. METHODS: We retrospectively evaluated 28 patients. Unreamed nail was negotiated across the well reduced fracture till subchondral bone and fixed with 2 to 3 distal locking screws in different planes. RESULTS: Fracture union rate was 85%. Three out of 28 patients had malalignment. Mean ankle, hindfoot functional score was 85. CONCLUSION: Complications can be minimized by impacting the unreamed nail till the subchondral bone while

One-stage tibial deformity correction and ankle arthrodesis for ankle osteoarthritis and tibial malalignment after low tibial osteotomy.

INTRODUCTION: There are no reports on one-stage corrective tibial opening wedge osteotomy and arthrodesis for osteoarthritis of the ankle and tibial malalignment after distal tibial osteotomy. PRESENTATION OF CASE: The patient was a 70-year-old woman who presented with complaints of ankle pain and lower limb deformity after tibial osteotomy performed for ankle arthritis 17-18 years earlier. Clinical examination revealed marked swelling around the ankle joint and pain and tenderness at the joint line. Imaging showed tibial malalignment and severe osteoarthritic changes in the ankle. The patien

Computer assisted knee arthrodesis in a primary case of septic arthritis: a case report.

We report a case of a 78 year old patient with primary septic arthritis who was treated with knee arthrodesis using computer navigation. Use of computer navigation avoided violating the medullary canal and its associated risks. Also, it offered the advantage of making precise bone cuts in all the three planes, thus minimizing the risk of malalignment. At 3 months, our patient showed good appositional contact of the bone ends, stable fusion, with no sign of infection and was mobilized with a simple brace. At 6 months review, the patient was walking painlessly without support. We achieved a mech

Imaging features of discospondylitis in two horses.

Two horses with discospondylitis are described with emphasis on the imaging modalities used and their contribution to the final diagnosis and outcome. Radiographic findings were vertebral endplate lysis with sclerosis in both horses, with additional vertebral subluxation and ventral spondylosis in one horse. Ultrasonographic findings included vertebral malalignment and obliteration of the intervertebral disc by spondylotic bone in one horse and irregular endplates, a widened disc space, a hypoechoic paravertebral abscess and hypoechoic, atrophic adjacent musculature with loss of linear fiber p

[Iso Kikuchi syndrome (C.O.I.F. syndrome). A report on 2 cases and a review of 44 cases in the literature (author's transl)].

Attention was drawn to congenital onychodysplasia of the index fingers (C.O.I.F.) by Iso, and later by Kikuchi who collected several cases. Micronychia, polyonychia, anonychia, hemionychrogryphosis and malalignment are the observed index finger defects. Five classical criteria were given to characterize the C.O.I.F. syndrome: 1 degree congenital, 2 degrees involvement of index fingers, 3 degrees variability in nail appearance, 4 degrees non-familial, non-hereditary, 5 degrees without bone and joint abnormalities. Alternative criteria are suggested for this syndrome. These are: 1 degree congeni

Correction of Femoral Torsional Deformities by Rotational Guided Growth.

Background: Femoral torsional malalignment is a common cause of in-toeing and out-toeing in children, often leading to gait disturbances, functional limitations, and increased risk of falls. Traditionally, osteotomy was the only surgical option for correction. A minimally invasive technique known as rotational guided growth (RGG) has recently been introduced to address these malalignments. This study aims to assess the effectiveness of rotational femoral malalignment correction by rotational epiphysiodesis with tension band 8-plates (Orthofix, Verona, Italy). Methods: Eleven patients with in-t

Combined external fixation frames and autologous bone graft reconstruction in thumb proximal phalanx bone loss: A case series.

BACKGROUND: Proximal phalangeal fractures with significant bone loss are inherently unstable, often resulting in significant malalignment secondary to the high degree of instability. Often significant bone loss is not amenable to surgical fixation alone and bone grafts are required to maintain length and reconstruct the given defect. OBJECTIVE: This cases series exhibits three cases of fixation of proximal phalanx fractures of the thumb with associated bone loss using an independently constructed external fixation frame in combination with autologous iliac crest bone graft to restore bone sto

Coronal alignment parameters of the knee predict osteoarthritis development : a Coronal Plane Alignment of the Knee classification-based analysis using the Multicenter Osteoarthritis Study data.

AIMS: Mechanical varus or valgus alignment measures have been demonstrated to be closely related to knee osteoarthritis (KOA) development, but few attempts have been made to correlate the Coronal Plane Alignment of the Knee (CPAK) classification and disease development. This study hypothesized that certain CPAK types or deviations in CPAK parameters might enhance the risk of KOA development. METHODS: KOA was defined as a Kellgren-Lawrence grade of 2 or higher. Data from 2,541 knees without KOA at baseline from the Multicenter Osteoarthritis Study (MOST) over five years of follow-up were exami

Management of rotational malalignment following operative treatment of fractures of the lower extremities. A scoping review.

BACKGROUND: Rotational malalignment after operative fracture treatment of the lower extremity may be associated with increased pain and functional impairment. Despite its clinical relevance, there are no uniform management guidelines. The aim of this scoping review is to provide an overview of all available evidence to diagnose and treat rotational deformities of the lower extremity following operative fracture treatment. METHODS: This scoping review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. A literature search was carri

[Clinical picture and diagnosis of knee osteoarthritis].

The principal symptom of osteoarthritis of the knee is pain. As the disease progresses, attack-like symptoms with knee pain, effusion, activated osteoarthritis, increasing restriction in knee mobility and reduced walking range occur more frequently. Secondarily joint malalignments also develop. Activities such as climbing stairs or sitting for long periods with bent legs are named as sources of pain for patients with patellofemoral osteoarthritis. Medial and/or lateral osteoarthritis of the knee is very probable when the pain is described as being more lateral or medial. The standard x-ray sho

Reasons for and Complications Following Revision Tibial Tubercle Osteotomy.

BACKGROUND: While tibial tubercle osteotomy (TTO) is a highly effective treatment for patellofemoral malalignment, some patients may have recurrent symptoms after surgery. To date, there are little data on revision TTO (rTTO) and its outcomes. PURPOSE: To evaluate the (1) reasons for and (2) complications following rTTO. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This was a retrospective review of patients who underwent rTTO from a single high-volume patellofemoral surgeon between 2016 and 2023. All included patients had a minimum of 3 months of follow-up data. Demographic ch

Operative management of periprosthetic femur fractures in the elderly using biological fracture reduction and fixation techniques.

Treatment of periprosthetic femur fractures often requires deviation from standard fixation techniques due to the presence of associated arthroplasty components, however, the use of adjuvant bone grafts and bone graft substitutes remain controversial. 59 patients (average age, 74 years) with either a periprosthetic femoral shaft about an arthroplasty stem (n=29) or supracondylar fracture above a total knee arthroplasty (n=30) were treated with biological open-reduction internal-fixation techniques without the use of bone grafts or bone-graft substitutes. All except one patient healed after the

Neurologic complications of surgery for cervical compression myelopathy.

Neurologic complications resulting from surgery for 384 cases of cervical myelopathy (cervical soft disc herniation, spondylosis, ossification of the posterior longitudinal ligament) were reviewed. Surgical procedures performed included 134 anterior interbody fusions (Cloward or Robinson-Smith technique), 70 subtotal corpectomies with strut bone graft, 85 laminectomies, and 95 laminoplasties. Twenty-one patients (5.5%) sustained neurologic deterioration related to surgery. The deterioration was classified into two types on the basis of the neurologic signs observed: deterioration of spinal cor

Biomechanical investigation of two long bone growth modulation techniques by finite element simulations.

Implants used to correct pathological varus-valgus deformities (VVD) and leg length discrepancies (LLD) may not be optimized for the specific treatment, as suggested by their off-label use. Detailed analysis of this issue has been limited by the poorly understood mechanical behavior of the growing physis and ignorance of the loads acting on the implants. The aim of this study was to predict and compare the loading conditions of a growth modulation implant in VVD and LLD treatments. Idealized finite element (FE) models of the juvenile distal femur treated with the Eight-Plate implant were devel

Tendon injuries in dance.

Professional ballet dancers require an extraordinary anatomic, physiologic, and psychologic makeup to achieve and sustain their level of ability and activity. They are subject to a myriad of injuries as a result of the extreme demands of this profession. Tendon injuries are common and often coexist with other pathologies of the bone, ligaments, and psyche. It is critical that the dance doctor not examine the tendon injury in isolation, but rather the cause of the injury, either intrinsic from anatomic malalignment or from external sources, including poor form.

Malalignment after minimally invasive plate osteosynthesis in distal femoral fractures.

INTRODUCTION: Although minimally invasive plate osteosynthesis (MIPO) is a preferred operative treatment for fractures of the distal femur, malalignment is a significant concern because of indirect reduction of the fracture. The purpose of this study, therefore, was to evaluate radiologic alignment after MIPO for distal femoral fractures. PATIENTS AND METHODS: Of the 138 patients with fracture of the distal femur who underwent MIPO, we enrolled 51 patients in whom bilateral rotational alignment could be assessed by postoperative computed tomography (CT). The patients included 32 men and 19 wo
